Uses

indications

HasanLar 500 mg drug is indicated for penicillin in people who are allergic to penicillin in mild to medium -sized bacterial infections (due to sensitive bacteria strains) such as:

  • Lower respiratory infections: Manchu bronchitis with exacerbations, community pneumonia.
  • Upper respiratory infection: acute sinusitis, sore throat, tonsillitis, otitis media.
  • Skin infections and soft tissue.

Infections of opportunities on HIV -infected patients due to Mycobacterium Avium or M. Aviumcomplex (Mac).

Mycoplasma pneumoniae and Legionella pneumonia, diphtheria and the early stages of the pertussis.

Used in combination with proton pump inhibitors (such as omeprazol) or a h2 h2 receptor antagonist or sometimes another antibacterial drug to treat Helicobacter pylori in progressive peptic ulcer disease.

    Pharmacokic

Clarithromycin is a semi -synthetic macrolid antibiotic. Clarithromycin often has bactericidal effects, which can have bactericidal effects in high doses or for very sensitive strains. Clarithromycin inhibits the synthesis of protein in sensitive bacteria by attaching a 50S ribosom subunit.

Clarithromycin has a stronger effect than erythromycin for Moraxella (Branhamella) CatVrhalis and Legionella spp. Very strong effects on Chiamydia spp, ureaplasma urealylicum and more than other macrolids for mycobacterium avium intracellular. Clarithromycin also works with M. Leprae. In some documents, it refers to the Vitro application effect with Toxoplasma Gondii and has some effects with Cryplosporidis.

Metabolic 14 - Hydroxy Clarithromycin is active and can be in vitro with mother medicine to significantly increase the activity of clarithromycin clinically for Haemophilus Influenzae. The metabolites with T1/2 are from 4 - 9 hours.

Clarithromycin is tolerated in the stomach - better intestines than erythromycin. Clarithromycin is affinity for CYP3A4 lower than erythromycin and therefore, the drug interaction is less important than clinically. However, clarithromycin is contraindicated with astemizol, cisaprid and terfenadin.

    Pharmacokinetics

    absorption

Clarithromycin, when taken, is absorbed quickly through the gastrointestinal tract and is subjected to the first metabolism at a high level, making the birth of the mother medicine decrease to about 55%. The level of absorption is almost not affected by food.

Clarithromycin's peak concentration and main metabolites 14 - hydroxy clarithromycin is about 0.6 - 0.7 micrograms/ml, after taking a single dose of 250 mg. In the dynamic equilibrium at the same dose level for the peak concentration of about 1 microgram/ml.

Clarithromycin pharmacokinetics are not linear and dependent on doses. Large doses can create peak concentrations without proportion due to saturated drug metabolism.

    Distribution

Clarithromycin and main metabolites are widely distributed and the concentration in tissue exceeds the serum concentration due to a part of the drug is collected into the cell.

    Metabolism and elimination

Metabolic drugs are high in the liver and release stool through biliary tract. A significant part is discharged through urine. About 20% and 30% corresponding to the dose of 250 mg and 500 mg are discharged in urine in the form of non -metabolized.

14 - Hydroxy Clarithromycin as well as other metabolites are also discharged through urine. Clarithromycin, 2 times/day, and about 5-7 hours when the patient takes 500 mg, 2 times/day. T1/2 is prolonged in patients with renal failure.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

HasanLar 500 mg drug is contraindicated in the following cases:

  • People are sensitive to macrolids.
  • Absolutely contraindicated with Cisaprid, Pimozid, Terfenadin and Astemizol because it can cause QT interval and arrhythmia, including ventricular, ventricular and torsion.
  • Be cautious when used

Precautions when used for patients with impaired liver or kidney function due to clarithromycin excreted mainly through the liver and kidneys.

Long -term and repeated clarithromycin treatment can cause fungal infections or sensitive bacteria. If superinfection occurs, Clarithromycin is discontinued and properly therapy.

In some patients, H. Pylori may become resistant to clarithromycin.

The ability to drive and operate machinery

    no.

    Pregnancy

The safety of the drug during pregnancy has not been determined. Only use Clarithromycin when really necessary and must be monitored carefully.

    The period of breastfeeding

The safety of the drug during breastfeeding has not been determined. Only use Clarithromycin when really necessary and must be monitored carefully.

    Drug interaction

like other macrolid antibiotics, clarithromycin has the ability to inhibit cytochrom P450. Therefore, the serum concentration of drugs can therefore be like Phenytoin, Carbamazepin, Theophyllin, Digoxin, Wartarin, Triazolam, Lovastatin, Cyelosporin, Disopyramid. Therefore, it is necessary to monitor the plasma concentration of these substances and adjust the dose accordingly.

Used with cisaprid and terfenadin to extend the QT, torsion, ventricular vibration. Clarithromycin reduces zidovudine absorption on adults infected with HIV. This situation can be avoided by taking Clarithromycin and Zidovudin 1-2 hours away. Do not record this reaction in children.

No dose adjustments when used simultaneously with omeprazol, maalox or ranitidine although Clarithromycin concentration may increase in plasma.

    Storage

Store in dry places, below 30 ° C. Avoid light.
