Hasanlor 5 drugs are indicated in the following cases:
Treatment of hypertension.
Symptomatic treatment of chronic stable angina.
Treatment of Prinzmetal angina.
Pharmacology
Pharmacological classification: Dihydropyridine derivative, anti -angina, anti -hypertension, antagonistic calcium.
Mechanism of action:
Amlodipin is a calcium channel antagonist and inhibits the movement of calcium ions into heart muscle cells and blood vessel smooth muscles. The mechanism of hypotension through direct relaxation of blood vessel smooth muscles. Amlodipine's exact mechanism that reduces angina has not been fully evaluated, but has the main role:
coronary artery dilatation both in ischemic area and normal blood supply area, this vasodilation increases oxygen supply to patients with cramped angina (Prinzmetal angina).
In patients with hypertension, taking a one -time dosage daily has a good blood pressure to reduce both standing and lying for 24 hours. Because amlodipine is slowly acting, it is less likely to cause acute hypotension or reflex tachycardia. Amlodipine does not adversely affect lipid concentration in plasma or metabolic, so it can be used amlodipine to treat hypertension in patients with asthma, diabetes, gout.
pharmacokinetics
absorption:
After drinking, amlodipin absorbs well, the peak concentration in plasma is achieved after 6 - 12 hours. Amlodipin's absolute bioavailability when taken orally is about 64 - 80% and is not affected by food.
Distribution:
The distribution volume is approximately 21 l/kg. In vitro research shows about 97.5% amlodipine linked to plasma proteins.
Metabolism:
Amlodipine is widely metabolized by the liver into non -activity metabolites.
Era:
Amlodipine is excreted in the urine in the form of intact (10%) and metabolites (60%). Selling time in plasma is about 35 - 50 hours.
Pharmacokinetics on some special clinical subjects:
Elderly:
Time to reach the peak of plasma in the elderly similar to adults. Amlodipine's clearance tends to decrease leading to an increase in AUC and the sale time.
Patients with liver failure:
Very few clinical data involves the use of amlodipine in patients with liver failure. Liver failure reduces the clearance of amlodipine leading to extending the sale time and increasing AUC by 40 - 60%.
Before taking Hasanlor 5 Hasan treats hypertension, angina (10 blisters x 10 tablets)
How to use
Take the whole pill with a sufficient amount of water, can take the medicine before or after meals. Should take the drug at the same time every day. Do not use the drug with grapefruit juice.
Do not throw away drugs in wastewater or domestic waste. Consult the pharmacist how to quit smoking is not used anymore. These measures will help protect the environment.
Dosage
Adults:
Children (from 6 -17 years old):
Hasanlor 5 is not specified for the starting dose of 2.5 mg/day/day by breaking half of the capsule.
Dosage on some special clinical subjects:
Patients with renal failure: Amlodipine plasma changes are not related to the level of renal failure, so the dosage is similar to adults. Amlodipine is not eliminated by a separation.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Symptoms:
There is little overdose when using amlodipine. Symptoms of overdose may include excessive peripheral vasodilation that causes an empty tachycardia, prolonged systemic hypotension that can lead to shock and death.
How to handle:
Positive cardiovascular support treatment, including regular monitoring of cardiovascular and respiratory functions, should pay attention to ensure the volume of circulation and the amount of urine released.
Vascular -causing therapy may be helpful in restoring blood vessels and blood pressure, as long as not contraindicated. Calcium gluconate intravenous injection may be helpful in reversing the effects of the impact of calcium channel blockers.
Gastric lavage may be useful in some cases. Research in healthy people, using activated carbon up to 2 hours after taking Amlodipin 10 mg has shown to reduce the absorption rate of amlodipine.
Valid is not effective and amlodipine is strongly linked to plasma proteins.
What to do when forgetting a dose? If the time of remembering is close to the time of taking the next dose, ignore whether you have forgotten and continue to drink as usual. Do not take double dose to compensate for the forgotten dose.
Side Effects
When using Hasanlor 5, you may experience unwanted effects (ADR).
Common, ADR> 1/100
Digestive system: Nausea, abdominal pain. Other: edema, fatigue, ankle swelling. Uncommon, 1/1000 Digestive system: Vomiting, indigestion, digestive disorders (diarrhea, constipation), dry mouth. Skin and subcutaneous tissues: hair loss, hemorrhage, skin discoloration, increased sweat, itching, rash. Other: Tinnitus, hypotension, shortness of breath, rhinitis, cramps, back pain, muscle pain, joint pain, urination disorders, night urination, frequent urination, impotence, large breasts in men, chest pain, weakness, pain, discomfort, increase or weight loss. Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Hasanlor 5 contraindications in the following cases:
Unstable hemodynamic heart failure after acute myocardial infarction.
Caution when using
Patients with heart failure: Caution when treating in patients with heart failure. In long -term research, controlled in patients with severe heart failure (degree III and IV according to the classification of NYHA), the lung edema event is higher than the placebo group. Calcium channel blockers, including amlodipine, should be used carefully in patients with congestion heart failure, because it may increase the risk of cardiovascular complications and mortality rates.
Patients with liver failure: Amlodipine has prolonged sale time and the area under the curve (AUC) is higher in patients with liver failure, the recommended dose has not been researched. Amlodipin should start with a low dose of the usual recommended dose, increase slow doses and careful monitoring in patients with severe liver failure.
Patients with renal failure: Amlodipine can be used in patients with kidney failure in normal doses. The changes in the concentration of amlodipine in plasma are not related to the level of renal failure. Amlodipine cannot be separated.
Elderly: In the elderly, increasing the dose should be carefully monitored.
Need to be used cautiously with aortic stenosis and porphyrin metabolism.
The ability to drive and operate machinery
Amlodipine may have a slight or medium impact to the ability to drive, operate machinery, work on high or other cases. Patients using amlodipine may become dizzy, headache, fatigue or nausea that reduce the ability to react, especially at the beginning of treatment.
Pregnancy
Calcium channel blockers can inhibit early uterine contractions, but the drug has no adverse effects on the delivery process. The risk of oxygen deficiency must be taken into account if causing hypotension in the mother, because there is a risk of reducing placental perfusion. This is a common risk when taking medication to treat hypertension, the drug can change the amount of blood due to peripheral vasodilation. In testing animals, calcium channel blockers can cause teratogenic, bone defects. Therefore, avoid using amlodipine for pregnant women, especially in the first 3 months of pregnancy.
The period of breastfeeding
It is impossible to determine whether amlodipine is excreted in breast milk or not. The decision to continue to stop breastfeeding or continue to stop treatment should be considered based on the benefits of breastfeeding compared to the benefits of amlodipine treatment for the mother.
Drug interaction
The effect of other drugs on amlodipine:
CYP3A4 inhibitors: Using strong or moderate CYP3A4 inhibitors (protease inhibitors, antifungal drugs, Azol groups, macrolid antibiotics such as erythromycin, clarithromycin), Verapamil or Diltiazem can significantly increase amlodipine pharmacodicity, increasing blood pressure risk. Clinical changes can be more clear in the elderly.
CYP3A4 induction drugs: Concomitance the use of CYP3A4 induction drugs (rifampicin, aluminum plants) causes low concentration of amlodipine in the blood. .
Grapefruit or grapefruit juice: increase the bioavailability of amlodipine in some patients, leading to increased hypotension effect.
dantrolen (infusion): Volunteers and cardiovascular collapse due to hyperkalemia death in animals when using Verapamil and venous dantrolen. Due to the risk of hyperkalemia, avoiding other calcium channel blockers such as amlodipine with dantrolen, especially for patients sensitive to malignant body hyperplasia and in controlling malignant body hyperthermia.
Anesthesia: Increases the anti -hypertension effect of amlodipine and can make blood pressure more sharply.
High -linked drugs with protein (such as Coumarin, Hytantoin, ...): Be careful when coordinating with amlodipin, because amlodipine is also highly linked to protein, the concentration of the above drugs is free (non -linked) that can change in serum.
The effect of amlodipine on other drugs:
Amlodipine increases the hypotension effect when combined with other hypotension drugs.
In clinical studies, Amlodipin does not affect the pharmacokinetics of Atorvastatin, Digoxin, Ethanol (Alcohol), Wafarin or Cyclosporin.
Simvastatin: Use a combination of amlodipin 10 mg with simvastatin 80 mg, causing 77% of simvastatin concentration compared to when using SIMVASTATIN single therapy. Patients who are using amlodipin should only limit the dose of simvastatin is 20 mg/day.
tacrolimus: Amlodipine increases blood concentration of tacrolimus, should monitor the concentration of tacrolimus in the blood and adjust the dose if necessary.
Lithi: When used with amlodipine, it can cause nerves, nausea, vomiting, diarrhea.
Non -steroid anti -inflammatory drugs, especially indomethacin, can reduce the anti -hypertension effect of amlodipine due to prostaglandin synthesis inhibitors and/or hold sodium and fluid.
