Uses

Indications

Clopidogrel is indicated to reduce events due to atherosclerosis such as myocardial infarction, stroke and blood vessel death in patients who have just suffered from stroke, have just suffered from myocardial infarction, or peripheral artery disease and has been identified.

Pharmacological

Clopidogrel is a strong and specific platelet aggregation inhibitor.

The role of platelets has been determined in the pathophysiology of atherosclerosis and cases of thrombosis.

Clopidogrel selectively inhibits adenosine diphosphate (ADP) to platelet receptor. And the consequence of this mount is the ADP intermediate activation of the GPIIB/LLA complex, thereby inhibiting platelet aggregation.

Dynamic pharmacokinetics

After taking oral repeated dose of 75 mg/day, Clopidogrel is quickly absorbed. However, the plasma concentration of the mother molecule is very low and below the specified limit (0.00025 mg/l) after 2 hours of use. The minimum absorption is 50%, based on the excretion through the urine of clopidogrel metabolites.

Clopidogrel is metabolized mainly in the liver, and inactive metabolites are carboxylic acid derivatives that account for about 85% of the drug component circulating in plasma.

This metabolic substance reaches a plasma peak concentration (approximately 3 mg/l after the dose of 75 mg repeated by oral) about 1 hour after taken.

When compared to a young healthy volunteer, the plasma concentration of the main metabolites is significantly higher in the elderly (> 75 years old).

However, this higher concentration in plasma is not associated with the difference in platelet aggregation and bleeding time. No need to adjust the dose for the elderly.

Before taking Infartan 75 OPV drugs prevent arterial thrombosis (3 blisters x 10 tablets)

Dosage

recent myocardial infarction, recent stroke, or peripheral artery disease has been identified

recommended dose is 75 mg, orally once a day during or outside meals.

acute coronary syndrome

Patients with acute coronary syndrome without ST wave difference (unstable angina/myocardial infarction without Q wave): Clopidogrel should be started with a single dosage of 300 mg and then continue at a dose of 75 mg once a day. Aspirin (75 mg - 325 mg once a day) should be started and continued in a combination with clopidogrel.

Patients with acute myocardial infarction with st differentia waves: Clopidogrel dose recommends 75 mg once a day, used in combination with aspirin, attached or not with thrombolytic drugs. Can start clopidogrel with load or not.

What do

do when overdose? Should consider appropriate treatment if bleeding. There is no antidote to the pharmacological activity of clopidogrel. If you need to adjust the bleeding time of clopidogrel, platelet transmission can limit the effect of clopidogrel.

What to do when you forget the dose? However, if it is almost time for the next dose, it is advisable to skip the forgotten dose. Take care not to use double the prescribed dose.

Side Effects

When using Infartan 75, you may experience unwanted effects (ADR).

  • Stomach bleeding, intestines, abdominal pain, diarrhea, constipation, vomiting, liver enzyme, blood clotting disorders - bleeding, skin rash, itching.

Notify the doctor with unwanted effects when using the drug.

  • Warnings

    Contraindicated

Infartan 75 Contraindications used in the following cases:

  • Sensitive to any ingredients of the drug.
  • Pathological bleeding is active as stomach bleeding, retinal hemorrhage or intracranial.
  • Be cautious when used

Caution for patients at risk of bleeding due to trauma, surgery or other bleeding diseases (such as ulcers).

Should stop clopidogrel 7 days before surgery.

Experience treatment for patients with severe liver disease is limited, should be careful to use clopidogrel for these patients.

Blood thrombocytopenic decrease (TTP) is very reported rarely using clopidogrel, sometimes occurring after a short time, manifested by plateletal reduction and capillary hemolytic anemia accompanied by nerve signs, renal or fever impairment.

Demonstration of hemorrhage reduction in thrombus cranes is likely to be treated immediately, including plasma filter treatments.

Experience for patients with severe renal failure is limited. Therefore, be careful when using clopidogrel for these patients.

Patients may be more susceptible to bruising and the bleeding time lasts longer than usual when using clopidogrel (alone or in combination with aspirin). Patients should notify the doctor for any abnormal bleeding (location or time).

    During pregnancy or lactation

There are no adequate and good controls on pregnant women. It is known that clopidogrel and/or its metabolites pass through the placenta in mice and pregnant rabbits, although there are no evidence in the detected of embryo poisoning or mice and rabbits at oral doses up to 500 mg and 300 mg/kg/day.

Because the above studies are not always a basis for predicting human influence, Clopidogrel is not recommended during pregnancy.

    Women who are breastfeeding

Mouse research shows Clopidogrel and/or metabolites are excreted in milk. Therefore, Clopidogrel should not be used for breastfeeding women.

The ability to drive and operate machinery

Be careful when driving or operating machines until you know how Clopidogrel affects you.

The safety and effectiveness of clopidogrel in children has not been set.

    Drug interaction

clopidogrel increases the effectiveness of aspirin on platelet training on collagen. Safety when used for long term aspirin and clopidogrel have not been set up.

Be cautious about using nonsteroidal anti -inflammatory drugs for patients who are taking clopidogrel due to the risk of stomach bleeding.

The risk of increased bleeding if used simultaneously clopidogrel with drugs that affect the hemostasis such as platelet inhibitors, thrombolytic drugs, anticoagulant drugs and clinical platelets clinically significant.

Digoxin, Theophylline, acid neutralizing drugs: There is no pharmacokinetic change of digoxin or theophylline when used simultaneously with clopidogrel. Acid neutralization does not change the absorption of clopidogrel.

    Storage

    Store at temperatures from 15 ° C - 30 ° C, in a dry place, avoiding light.

Expiry date: 36 months from the date of manufacture. Do not use drugs when expiry
