Uses

Indications

Irbesartan 150 are indicated for treatment in the following cases:

  • Hypertension: Irbesartan is used alone or in combination with other hypertension treatment groups in the treatment of hypertension. Removing sugar: Irbesartan is used to treat diabetes with diabetes manifested by an increase in serum and protein -urinary creatinine (protein secretion in urine exceeds 300 mg/day) in patients with type 2 diabetes and hypertension.

    Code ATC: C09CA04

Irbesartan is an oral drug that has a selective antagonistic effect on Angiotensin II receptor (type AT1).

    Mechanism of impact

Prevent all the effects of Angiotensin II through the AT1 receptor intermediaries, regardless of the origin or synthetic path of Angiotensin II.

Selective antagonistic effect on the AT1 receptor leads to increased plasma renin and decreased serum aldosterone levels.

Serum concentration is not significantly affected when using solitary Irbesartan in the recommended dose.

Irbesartan does not inhibit the transferred enzyme (Kininase II), an enzyme that produces angiotensin II and bridge bradykinin into inactivated metabolites.

Irbesartan does not need to be converted into metabolites for this effect.

    Dynamic pharmacokinetics

Irbesartan is quickly absorbed by gastrointestinal tract with oral bioavailability of 60 - 80%.

The peak concentration of plasma of Irbesartan is reached from 1.5 to 2 hours after drinking. About 96% Irbesartan binds to plasma proteins.

The drug is metabolized through the liver, mainly by cytochrome isoenzym CYP2C9 into non -activity metabolites. The drug is eliminated in a constant form and metabolic in bile and urine.

The final selling time is about 11 - 15 hours.

    How to use

Irbesartan Stella is used by oral and does not depend on meals.

    Dosage

    Hypertension

In adults: Irbesartan's usual starting dose is 150 mg x 1 time/day for patients not to be impaired inner volume. If the starting dose is still in the beginning, the blood pressure is still not able to lower the desired, can increase the dose up to 300 mg x 1 time/day or may use additional diuretics. Irbesartan can also be used in combination with other hypertension medications.

For people over 75 years old, patients with impaired internal volume and patients are secretly bloody: Starting treatment at low dose of 75 mg per day.

    Kidney disease due to diabetes

In the treatment of kidney disease in patients with type 2 diabetes with hypertension, it is recommended to start with the dose of Irbesartan 75 mg 1 time/day, up to 300 mg 1 time/day as a maintenance dose.

Patients with kidney failure, liver failure

No dose adjustments in patients with renal failure, liver failure.

    Children

Irbesartan's usual starting dose in children 6-12 years old or 13-16 -year -old teenagers with hypertension are 75 mg or 150 mg x once a day.

If the response of blood pressure is not enough for the starting dose, the dose may be increased to 150 mg or 300 mg x 1 time/day for children or teenagers. Do not use higher doses.

    What to do when overdose?

    Symptoms

There is no data overdose in humans. However, the daily dose of 900 mg for 8 weeks has been well tolerated. When overdose, symptoms are more likely to occur, especially hypotension and tachycardia, slow heart rate can also occur.

    Treatment

There is no specific therapy in the treatment of Irbesartan overdose. Patients should be monitored and symptomatic treatment and supportive treatment. Irbesartan cannot be excluded by hemolysis.

What to do when you forget the dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using Irbesartan 150, you may experience unwanted effects (ADR).

Unwanted effects of Irbesartan's reports are usually mild and transient, including dizziness, headache and hypotension depending on the dose.

Hypotension can occur especially in patients with intravascular volume impairment (for example, patients taking high doses of diuretics).

impaired renal function, and rare rash, urticaria, itching, angioedema and liver enzyme can occur. Hyperbonia, muscle pain and joint pain have been reported.

Other undesirable effects have been reported for Angiotensin II receptor antagonists including respiratory disorders, back pain, digestive disorders, fatigue and neutropenia. Explanation of muscle rarely occurs.

    Warnings

    Contraindicated

Irbesartan 150 Contraindicated for the following cases:

  • hypersensitively to Irbesartan or any ingredients of the drug.

    Be cautious when used

Starting treatment with hypertension may cause symptomic hypotension in patients with impaired intravascular volume or sodium decrease, for example, patients use strong diuretic therapy or fertilizer.

This fluid decrease should be adjusted before taking Irbesartan or the starting dose is lower.

In general, in patients with blood vessel and renal function depends mainly on the activity of the renin-angiotensin-aldosteron system (such as severe congestive heart failure or kidney disease, including kidney stenosis), treatment with enzyme inhibitors or Angiotensin II drug antagonistic drugs that affect this system that can cause acute blood pressure, rare urine, minimize urine, minimize urine, minimize urine More with acute renal failure.

As well as all other antihypertensive drugs, excessive hypotension in patients with ischemic heart disease can lead to myocardial infarction or stroke.

Irbesartan Stella contains lactose excipients. This drug should not be used for patients with rare genetic problems galactose intolerance, total lactase enzyme deficiency or poorly absorbed glucose-galactose.

    pregnancy or breastfeeding

    Pregnant women

Medicines directly acting on the Renin-Anotensin system can cause disease and death for fetuses and babies when used in the second and third quarter of pregnancy. Irbesartan should be stopped as soon as possible when being diagnosed with pregnancy.

    breastfeeding women

When driving and operating machinery, it should be noted that dizziness or fatigue may occur during treatment.

    Interaction

Diuretics and other hypertension drugs: may increase the hypotension effect of Irbesartan.

Potassium supplements and potassium -saving diuretic supplements: can lead to increased serum potassium levels.

Lithi: Need to carefully monitor serum lithium concentration if this combination is necessary.

Non -steroid anti -inflammatory drugs: reduce hypertension.

There is no significant pharmacokinetic or pharmacological interaction in studies on interactive with hydrochlorothiazid, digoxin, warfarin and nifedipine.

Based on in vitro data, there is no possibility of interaction with drugs that their metabolism depends on Cytochrom P450 Isoenzyme 1A1, 112, 2A6, 2B6, 2D6, 2E1 or 3A4.
