kalmeco is indicated for treatment of peripheral neuropathy due to vitamin B12 deficiency.
Pharmacokological
kalmeco contains mecobalamin, a homogeneous substance of vitamin B12.
mecobalamin participates in the metabolism of protein and nucleic acid.
Mecobalamin is the only homogeneous substance of B12 involved in the methylation reaction in the human body. Mecobalamin facilitates the methylation process of T-RNA, which is very important in the process of protein synthesis and homocystein conversion into methionin.
Precaessing studies on animals with mice show that mecobalamin has an impact on the activity of the nervous system.
mecobalamin promotes recovery processes for atrophy muscles by stimulating large hip nerves. There is also the effect of repairing damaged nerve cells in case of encephalitis and nerve inflammation in animals.
These effects are due to mecobalamine that enhances the synthesis of protein and nucleic acid of the nerve and facilitates the process of spinal cord formation.

Single dose
When using a single dose of methyl-cobalamin (Mecobalamin) for volunteers who are healthy men with a dose of 120 µg and 1500 µg by oral, the peak concentration of vitamin B12 in plasma is achieved after 3 hours and depends on the concentration.

Methyl cobalamin repeated dose for volunteers is a healthy male with a daily dose of 1500 µg for 12 consecutive weeks and determines the change of vitamin B12 concentration to 4 weeks after taking the last dose.
The serum concentration increases in the first 4 weeks of taking the drug, reaching the concentration of about 2 times the original value.
After that, the drug concentration increased slowly and reached the peak concentration of about 2.8 times the initial value after 12 weeks of taking the drug. The concentration of the drug decreased after the last dose (12 weeks) but still about 1.8 times higher than the original value after 4 weeks of stopping the drug.

Dosage
Adults
The total dose is 1500 µg per day, dividing 3 times (each time 1 tablet 500 µg).

Interactive drug
The absorption of the drug may be reduced when there is the appearance of para-aminosalicylic acid, colchicin, biguanide, neomycin, cholestyramin, potassium chloride, methyldopa and cimetidine.
Patients treated with chloramphenicol may be less responding to Kalmeco.
Serum concentration may be reduced when taking oral contraceptives. These interactions do not seem to cause clinical effects.
