Uses
Indications
Letrozsun is the first therapy in metastatic breast cancer/progressing in menopausal women (positive or unknown hormone receptors).
Pharmacokology
Letrozole is a selective Non-Stomatase enzyme inhibitor. Letrozole inhibits aromatase enzyme by connecting with the heme of this enzyme P450 cell pigment, leading to a reduction in estrogen biosynthesis in all tissues.
Letrozole is effective against cancer by taking away the breast cancer dependent of estrogen of tumor stimulation agents. In menopause, estrogen is mainly synthesized by the activity of aromatase enzyme, which converts adrenal androgen (mainly androstenedione and testosterone) into Estrone (E1) and Estradiol (E2). The process of inhibiting estrogen synthesis in peripheral tissues and the tumor itself can be achieved by the specific inhibition of the enzyme.
In healthy menopause, single dose 0.1; 0.5 and 2.5mg Letrozole inhibit the estrone and estradiol levels of plasma, 75 - 78% and 78% respectively. Maximum inhibition achieved after 48 - 78 hours.
In patients with menopause with progressive breast cancer, daily dose 0.1 to 5mg helps inhibit serum levels of oestradiol, oestrone, and oestrone sulphate to 78 - 95% in all treated patients.
Letrozole has no effect on serum androgon concentration (Androstenedione and Testosterone) in healthy menopausal women after single dose 0.1; 0.5 and 2.5 mg, which are the doses that inhibit estrogen biosynthesis, did not lead to Androgenic money accumulation. Does not affect the synthesis of adrenal steroids.
Dynamic pharmacokinetics
Letrozole is absorbed quickly and completely through the digestive tract (absolutely used about 99.9%). Food reduces the absorption rate but the absorption rate is unchanged. The small change of the absorption rate is not considered to have a clinical effect, so Letrozole can be used before, during or after meals.
Letrozole's serum binding is about 60%, mainly albumin (55%). Letrozole concentration in erythrocytes is about 80% of Letrozole concentrations in plasma.
Metabolizes into carbinol transformers without pharmacological activity is the main excretion path of Letrozole but relatively slow when compared to the blood rate through the liver. ISoenzyme P450 3A4 and 2A6 are found to be able to change the Letrozole into the metabolic substance on In vitro but the contribution of these factors has not been set on in Vivo.
The apparent sale time is 2 days. After daily dosage of 2.5mg, the stable concentration is reached within 2-6 weeks.
Before taking Letrozsun 2.5mg Sun Pharma supports breast cancer treatment (2 blisters x 14 tablets)
How to use
Letrozsun drug used oral.
Dosage
recommended dose for Letrozsun is 2.5 mg/time/day. Treatment with Letrozsun should be continued depending on the response of the tumor. Stop drug if the tumor stops growing.
No dose adjustment for the elderly.
No dose adjustments for patients with liver failure or renal failure from mild to medium level if creatinine clearance> 10 ml/min.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? In special cases, the highest single dose is 62.5 mg or using 25 tablets. While no dangerous side effects are reported in these cases (due to limited data), there is no sure instructions for treatment. However, vomiting reaction may be available if the patient is irritated. Normally, regular care and monitoring of life signs is necessary in the study of single doses, the highest dose is 30 mg, in multi -dose test, the highest dose can be used is 10 mg.
In an emergency, call the 115 emergency center immediately or go to the nearest local health station.
What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
Letrozol -related side effects are usually mild to medium and rare cases of serious cases. Many side effects may be related to the disease itself or the normal pharmacological consequences of the lack of estrogen (such as flushing, thinning hair).
Side reactions often occur such as musculoskeletal pain, joint pain, headache, fatigue, nausea, shortness of breath, peripheral angio, cough, constipation, vomiting, chest pain, viral infection, diarrhea, rash, abdominal pain, indigestion, anorexia.
Less: dizziness, weight gain, itching.
Notice the doctor the side effects encountered when taking the drug.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Letrozsun drug is contraindicated in the following cases:
Be cautious when used
in patients with breast cancer with average liver dysfunction, no need to adjust the dose. However, it is necessary to be cautious because the disposal of Letrozole is mainly dependent on the inner transformation clearance.
Renal failure (Creatinine clearance coefficient of 20 - 50ml/minute): does not affect the Letrozole concentration in plasma in a stable state when using a dose of 2.5mg or 5mg. Therefore, there is no need to adjust the dosage in patients with renal function. Letrozole may be removed by hemorrhage because Letrozole is weak with plasma proteins. Should carefully consider the risk and benefits achieved before treatment with Letrozole.
The effect of the drug on the ability to drive and operate machinery
When treated with Letrozole, some cases may be tired and dizzy. Therefore, patients need to be notified as physical and mental ability necessary for operating machinery or driving can be affected.
Use drugs for women during pregnancy and lactation
have not had a complete and well -controlled research on the use of Letrozole in pregnant women. Do not use Letrozole for pregnant women.
It is still not known whether Letrozole is excreted through breast milk. Because other drugs are excreted in milk, we should not use Letrozole for breastfeeding women.
Drug interaction
Clinical interactive studies with cimetidine and warfarin show that when used simultaneously letrozole with the above drugs does not cause any significant interaction with clinical significance although cimetidine is known as an ISOenzyme inhibitors Cytochrome P450 of Letrozole transformation on Vitro.
Storage
Leave a cool place, avoid light, temperature below 30⁰C.
