Indications

Levofloxacin 500 mg is indicated in the following cases:

Treatment of infections caused by sensitive bacteria, including:

  • Pneumonia has a community.
  • Chronic prostatitis caused by bacteria.
  • Nephritis - Pyelonephritis and urinary tract infections have complications.
  • Skin infections and complicated skin structure.
  • Preventive after exposure and thorough treatment of coal diseases.
  • Acute bacterial infections of chronic bronchitis.
  • Pharmacology

levofloxacin is a broad spectrum antibiotic belonging to Quinolon group (fluoroquinolon). Levofloxacin has bactericidal effects due to inhibiting the Topoisomerase II (DNA-GYRASE) and/ or Topoisomerase IV, which are essential enzymes of bacteria involved in the process of copying, transcription, DNA repair of bacteria.

levofloxacin is acting on gram -positive bacteria and anaerobic bacteria than other fluoroquinolon (such as ciprofloxacin, enoxacin, lomefloxacin, norfloxacin, ofloxacin). However, Levofloxacin has a Vitro effect on Pseudomonas Aeruginosa weaker than Ciprofloxacin.

    pharmacokinetics

    absorption:

The drug is absorbed quickly and almost completely. The peak concentration in plasma is usually achieved after 1-2 hours, absolutely used by approximately 99% - 100%. Food less affects the absorption of the drug

    Distribution:

Levofloxacin is widely distributed in the body such as bronchial mucosa, alveolar lining, alveolar, lung tissue, skin (the fluid of blistering spots), prostate tissue, urine and difficult to penetrate into the cerebrospinal fluid. The ratio of plasma proteins is 30-40%.

    Metabolism:

levofloxacin is very little metabolized in the body. The metabolites of levofloxacin are desmethyl-levofloxacin and levofloxacin n-oxide less biological effects

    Era:

The drug is eliminated in urine about 87% in the form of unchanged and crossing about 12.8%. The sale time of levofloxacin from 6 to 8 hours.

Before taking Levofloxacin 500mg Imexpharm treatments for pneumonia, prostatitis, nephritis (1 blister x 10 tablets)

    Dosage

Patients with normal renal function (ClCR> 50 ml/minute):

  • Pneumonia with community, skin infection and skin structure with complications: 500 mg/time x 1-2 times/day, for 7-14 days.
  • Chronic prostatitis caused by bacteria: 500 mg/time/day, for 28 days.

  • Nephritis - Pyelonephritis, urinary tract infections have complications: 500 mg/time/day, for 7-14 days.
  • Preventive after exposure and thorough treatment: 500 mg/time/day 8 weeks.
  • Acute bacterial infections of chronic bronchitis: 500 mg/day/day for 7-10 days.

  • Acute sinusitis caused by bacteria: 500 mg/day/day for 10 - 14 days.
  • Patients who have responded well when treating with levofloxacin intravenously can be used in the form of tablets to finish a treatment.

Patients with renal failure (Clcr ≤ 50 ml/minute):

The initial dose does not change 500 mg/24 hours when used for patients with renal failure, the next dose should be adjusted based on creatinine clearance.

    Patients with liver failure:

No need for dose do the dose due to levofloxacin is not metabolized through the liver but mainly through the kidneys.

    Elderly:

No dose should be adjusted if normal kidney function. However, be careful when choosing the dose and should monitor the kidney function.

    Children:

Levofloxacin is contraindicated for children and adolescents in the development stage.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose? Symptoms of levofloxacin overdose on the nervous system include: taste disorders, convulsions, hallucinations, tremor.

    How to handle:

  • Without specific antidote.
  • In case of overdose, need to treat symptoms, remove smoking from the stomach, fully compensate for patients. Monitor the electrocardiogram because the drug causes the interval of QT. Give patients to drink Antacid to protect the stomach - intestinal mucosa.
  • Hematomopia and peritoneal fertilizer continuously not effective levofloxacin from the body.
  • What to do when forgetting a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

  • neurological disorders: drowsiness, tremor, taste disorders.
  • Instructions on how to handle ADR

Levofloxacin should be stopped in the following cases: Starting skin rash or any signs of sensitive reaction or adverse reaction on central nervous system. It is necessary to monitor patients to detect fake colitis and take appropriate treatments when diarrhea appears while using levofloxacin.

When signs of tendonitis appear, it is necessary to stop the drug immediately, leave two heel tendons with appropriate fixed tools or heel brace and specialized consultation.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Levofloxacin 500 mg contraindicated in the following cases:

  • There is a history of hypersensitivity to levofloxacin, other quinolones or any ingredients of the drug;
  • epilepsy;

    History of disease in the tendon caused by fluoroquinolon;

  • Patients under 18 years old;
  • Pregnant and lactating women.

    Be cautious when used

Achille tendonitis, which can lead to tendons and appear on both sides, appearing at the first 48 hours after starting the medication. Be careful when using the drug and adjust the dose in risk objects (people over 65 years old, are using corticosteroids including inhalation form) according to glomerular filtration level.

levofloxacin can cause cartilage degeneration in joints.

Levofloxacin 500mg medicine causes many unwanted effects, so it is necessary to be cautious in patients in the following cases:

  • Patients with myasthenia gravis because the manifestations may get worse.
  • As soon as used at the first dose, there may be adverse nerve reactions such as mental disorders, increased intracranial pressure, central nervous stimulation leading to convulsions, trembling, restlessness, headache, insomnia, depression, confusion, hallucinations, nightmares, intentions or suicide action (rare).
  • People with central neurological diseases such as epilepsy, brain sclerosis can increase the risk of convulsions.

  • When there are the first signs of sensitive reaction, even anaphylaxis.
  • Diabetes people are using levofloxacin simultaneously with a hypoglycemic drug or with insulin that can cause sugar metabolic disorders.

  • Clostridium difficile colitis may occur at all levels from mild to life -threatening.
  • Sensitive to light so it is necessary to avoid direct contact with strong light or UV rays during treatment and 48 hours after treatment.

  • Patients with prolonged QT interval, patients with hypokalemia, patients are using anti -arrhythmic drugs in group IA, III, are in the condition of arrhythmia such as slow rhythms and acute myocardial ischemia due to drugs that increase QT.
  • Do not use levofloxacin to treat defined or suspected infections caused by the MRSA strain, unless the antibiotic results determine the bacterial strain is also sensitive to levofloxacin.
  • Patients with renal impairment need to adjust the dose.
  • Patients with serious skin reactions such as Stevens-Johnson syndrome and poisoned epidermal necrosis.
  • Patients who are being treated with anti -vitamin K anticoagulants (such as warfarin) due to levofloxacin increases blood coagulation indicators and increases the risk of bleeding in this object.
  • Patients with severe diseases for example, for example, blood infections can lead to liver necrosis or liver failure threatening life.
  • Improving visual patients or having other eye problems.

Quantitative tests in the urine can give a fake positive result in patients being treated with levofloxacin.

Serious reactions are capable of not recovering and causing disabilities, including tendonitis, tendons, peripheral neuropathy and adverse effects on the central nervous system.

Tartrazin excipients in the drug can also cause allergic reactions in patients.

The ability to drive and operate machinery

levofloxacin can cause some unwanted effects that directly affect the central nervous system (such as headache, dizziness, blurred vision, hallucinations, ...). Therefore, ships should not be driven or operate machinery during treatment with levofloxacin.

    Pregnancy

Do not use levofloxacin on pregnant women.

    The period of breastfeeding

The drug has many risks of articular cartilage damage on young children, not breastfeeding when using levofloxacin.

    Drug interaction

  • antacid, sucralfate, metal ions, multivitamin, didanosin: When used simultaneously, it can reduce the absorption of levofloxacin, need to drink away from Levofloxacin at least 2 hours.
  • Theophylin: Need to closely monitor theophylin concentration and adjust the dose if necessary when used simultaneously with levofloxacin. Warfarin and other anti -anticoagulant drugs: Levofloxacin increases the anticoagulant effect of these drugs, needs closely monitoring.

Steroid anti -inflammatory drugs: capable of increasing the risk of central nerve stimulation and convulsions when used simultaneously with levofloxacin.

Hypoglycemic drugs: Concomitant use with levofloxacin may increase the risk of blood sugar disorders, need closely monitoring.

BCG, Mycophenolat, sulfonylura, Korean vaccine: reduces the effects when using simultaneously with levofloxacin. fenbufen: Levofloxacin concentration increases by about 13%. Probenecid and cimetidin: levofloxacin is reduced excretion through the kidneys when used simultaneously with probenecid (34%reduction) or cimetidine (down 24%) due to these two drugs inhibit the excretion through the renal tubules of levofloxacin. Therefore, be careful in patients with renal failure.

ciclosporin: Ciclosporin's waste time increases about 33% when used simultaneously with levofloxacin.

Anti -arrhythmia, three -round antidepressants, macrolid antibiotics, psychotic anti -psychotic drugs: extend the QT interval on the center of electrolyte.
