Uses

indications

Lodimax 5 pills are indicated in the following cases:

  • Treatment Hypertension , chronic stable angina. Calcium through cell membranes into myocardial cells and blood vessel smooth muscles. Amlodipine's hypotension mechanism due to the effect of direct dilatation of blood vessel muscles. The exact mechanism of Amlodipine reduces angina has not been fully determined but Amlodipine reduces the overall anemia burden due to the following two effects:
  • Amlodipine relaxes peripheral arteries and thus reduces the peripheral resistance total of the heart (reducing post -burden). Because the heart rate does not change, this burden reduces the heart to reduce energy consumption and oxygen needs of the heart muscle.
  • The mechanism of action of Amlodipine may also include the relaxation of the main coronary arteries and coronary arteries, both in anemia and normal areas. This vasodilation increases the supply of oxygen to the heart muscle in patients with coronary artery spasms (Prinzmetal angina) and reduces smoking coronary spasms.

In patients with hypertension, a daily dose/day significantly reduces clinical blood pressure in both lying and standing in 24 hours. Due to the slow starting characteristic, Amlodipine avoids the drop in blood pressure when used.

In patients with angina, 1 -day use/day amlodipine increases the overall exertion time, the starting time of angina and the ST segment time to 1 mm, reducing both the frequency of angina and reducing the amount of nitroglycerine to be used.

Amlodipine is not related to any adverse effects on metabolism or changes in blood lipids and is suitable for use in asthma patients, diabetes and gout.

    pharmacokinetics

    absorption:

Amlodipine is well absorbed with peak plasma concentrations reaching about 6–12 hours after drinking. Absolute bioavailability is estimated at 64–80%. The absorption of drugs is not affected by food.

    Distribution:

The distribution volume is approximately 21 l/kg. The ratio of cohesion to plasma protein is about 97.5%.

    Metabolism:

amlodipine is metabolized largely in the liver into non -active metabolites.

    Era:

The disposal time of the drug is about 35–50 hours. 10% of the initial substance and 60% of metabolic substances are eliminated in the urine.

    How to use

    oral medication.

    Dosage

    adults:

For hypertension and angina, the starting dose is usually 5 mg x 1 time/day, which can increase to a maximum dose of 10 mg x 1 time/day depending on the response of each patient.

    Hypertension:

Amlodipine is simultaneously used with thiazide diuretics, alpha blockers, beta blockers or angiotensin transfer enzymes.

    Angina:

Amlodipine can be used for monomers or simultaneously used with other anti -angina drugs in patients with angina nitrate derivatives and/or appropriate beta blockers.

No need to adjust the dose when used simultaneously amlodipine with thiazide diuretics, alpha blockers, beta blockers or angiotensin enzyme inhibitors.

Children and teenagers 6–17 years old:

    Hypertension:

The recommended starting dose is 2.5 mg x 1 time/day, may increase to 5 mg daily if the desired blood pressure is not achieved after 4 weeks.

Dosage over 5mg daily has not been studied in children.

    Children

    No report.

    Elderly:

The ability to tolerate the elderly and young patients is the same when using the same Amlodipine dose. The usual dose is recommended for the elderly and cautious when increasing the dose.

    Patients with liver failure:

The recommended dose has not been set for patients with mild to moderate liver failure, so it is necessary to be cautious when choosing the dose and starting treatment with the lowest dose effectively. For patients with severe liver failure, start taking the drug at the lowest doses and then increase the doses slowly.

    Patients with renal failure:

    Recommended use of doses.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? A few cases need gastric lavage.

In case of severe hypotension, a positive cardiovascular support must be taken including regular monitoring of cardiovascular and respiratory functions, high limbs, paying attention to the volume of circulation and urine. Vascular medications can be used to recover blood vessels and blood pressure. Calcium gluconate can be injured to reverse the effect of calcium channel blockers.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Instructions on how to handle ADR

Notify the doctor with unwanted effects when using the drug.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

Lodimax 5 drugs contraindicated in the following cases:

  • Sensitive to the derivatives of dihydropidine, amlodipine or any ingredients of the drug.
  • Severe hypotension, shock (including cardiovisa).
  • Conclusion of the output of the left ventricle (for example: severe aorta stenosis).
  • Unstable hemodynamic heart failure after acute myocardial infarction.

    Be cautious when used

Amlodipine has been reported related to the increase in pulmonary edema ratio when using drugs in heart failure patients.

The half -life of the drug is prolonged in patients with liver failure, so it is necessary to be cautious when taking amlodipine in these patients.

The ability to drive and operate machinery

drugs hardly impair the ability to drive and operate machinery.

    Pregnancy

Amlodipine's safety in pregnant women has not been set. Medication should only be used during pregnancy when there is no way to replace it more safely and when the benefits are out of risk.

    The period of breastfeeding

The safety of amlodipine in breastfeeding women has not been set. Amlodipine is distributed to breast milk.

    Drug interaction

Amlodipine is used safely with thiazide diuretics, alpha receptor inhibits, beta receptor inhibitors, angiotensin transfer inhibitors, prolonged nitrate, nitroglycerine under the tongue, non -steroid anti -inflammatory drugs, antibiotics and oral oral medications.

Amlodipine does not affect the cohesion of plasma proteins of digoxin, phenytoin, warfarin and indomethacin.

Amlodipine when used with simvastatin may increase the concentration of simvastatin. For patients using amlodipine, the dose limit of simvastatin is 20 mg/day.

Do not use amlodipine with grapefruit juice because it can increase the effect of lowering blood pressure.

Strong CYP3A4 inhibitors (Ketoconazole, Itraconazole , Ritonavir) may increase the concentration of amlodipine in the blood larger than Diltiazem. Be cautious when using amlodipine along with CYP3A4 inhibitors.

Clarithromycin may increase the risk of hypotension when used simultaneously with amlodipine. Monitor patients closely if they combine these two drugs.

Simultaneous use amlodipine with CYP3A4 induction ( rifampicin ) can reduce the concentration of amlodipine in the blood.

    Storage

Store at a temperature of 15–30ºC.
