Uses

indications

Lordivas 5mg drug is indicated in the following cases:

  • Hypertension.
  • Chronic stable angina. Medications inhibitors of calcium ion transport through the membrane to the heart muscle and smooth muscle walls.

Amlodipine's hypotension mechanism due to the direct active effect of smooth muscle relaxation. The exact mechanism of relief of amlodipine angina has not been clearly defined. However, amlopidine reduces wholeheartedly anemia through 2 mechanisms:

Amlodipin relaxes peripheral arteries and thus reduces the total peripheral resistance (post -load) that hinder the heart activity. When the heart rate maintains stable, the post -load reduction reduces the consumption of energy and oxygen of the heart.

Amlodipin relaxes 3 astronaus and coronary artery, both in normal and ischemic region. Vascular dilatation increases oxygen transport in coronary artery patients (Prinzmetal angina).

Due to the slow onset, no amlodipine in case of acute hypertension.

In patients with angina, one daily dose increases the time of strengthening the angina, and the time leads to the difference of the ST about 1 mm, while reducing the frequency of angina and the need to use glyceryltrinitrate.

amlodipine is not related to harmful metabolic reactions and does not change lipid level in plasma. Therefore, Amlodipin can be used in asthma patients, diabetes and gout.

    Dynamic pharmacokinetics

Absorption: After oral, Amlodipin is well absorbed, the peak concentration is achieved after 6 -12 hours. Absolute bioavailability about 64 - 80%.

Distribution: The distribution volume is about 21 kg. In vitro studies show that 97.5% amlodipine in plasma is linked to protein. Amlodipine's bioavailability is not affected by food.

Metabolism: Amlodipine is strongly metabolized in the liver into inactive metabolites.

Elimination: When using the dose once a day, the last elimination time is about 35 - 50 hours. About 10% amlodipine is not metabolized and 60% of amlodipine metabolic substances are excreted in the urine.

Pharmacokinetics in some special clinical subjects:

Patients with liver failure: There is very little clinical data on the use of amlodipine in patients with liver failure, impaired liver function reduces amlodipine clearance.

Elderly patients: Amlodipine's peak concentration time in similar plasma in young people and the elderly. Amlodipin's clearance tends to decrease leading to an increase in AUC and the sale time in the elderly.

Children: A pharmacokinetic study was conducted on 74 children with hypertension from 1-17 years old (34 children 6- 12 years old and 28 children 13- 17 years old) taking amlodipin dose of 1.25- 20 mg, divided 1 or 2 times/day. In this group of children, the clearance of 22.5-27.4/hour in men and 16.4-21.3 hours in women has a big difference in the ability to absorb each individual. There is little information in children under 6 years old.

    How to use

Lordivas 5mg Tablets for oral tablets.

    Dosage

    adults

In most cases of hypertension and angina, the dose of 5 mg/time/day, the dose may be increased to 10 mg/day depending on the patient's response.

In hypertension patients, Lordivas 5 mg is often combined with thiazid diuretics, alpha blockers, beta blockers or enzyme inhibitors (ACE).

For angina, Lordivas 5 mg is used for monomers or combined with persistent angina treatments, nitrate and/ or beta blockers with appropriate doses.

No need to adjust Lordivas 5 mg dose when used with thiazid diuretics, beta blockers and enzyme inhibitors.

Dosage in some special clinical subjects:

Elderly: Elderly and young people tolerate the same dose. Lordivas 5 mg the same.

Normal recommendations are similar in young people, but be careful if the dose increases.

Patients with liver failure: There is no recommendation for patients with mild to moderate liver failure, so be careful when used and should start at the lowest dose allowed. Amlodipin's pharmacokinetics have not been studied in patients with severe liver failure, so the lowest starting dose should be used and increased slowly.

Patients with renal impairment: The change of amlodipine concentration in plasma is not correlated with the degree of renal failure, so the recommended use of normal doses of amlodipine is not fertilized.

    Children

  • Children 6 - 17 years old: The starting dose is 2.5 mg/time/day, the dose can be increased to 5 mg/time/day if the target blood pressure is not reached within 4 weeks. The dose level is greater than 5 mg/day has not been studied in children.

Research data shows that amlodipine overdose can cause excessive foreign vasodilation and fast heart reflection. There is a percentage of prolonged systemic hypotension, including shock leading to death.

    How to handle

Hospitalized to regularly monitor cardiompapia, high presents, pay attention to the volume of circulation and the amount of urine released.

Plee -causing drugs may be helpful to restore vessels and blood pressure, in the absence of contraindications for calcium gluconate intravenously can reverse the effects of calcium blockers.

Gastric lavage may be useful in some cases. In healthy volunteers, using activated carbon 2 hours after taking 10 mg of amlodipine reduces the absorption rate of amlodipine.

amlodipine is highly attached to plasma proteins, so the amlodipine fertilizer is not effective.

What to do when forgetting the dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Lordivas 5mg Contraindicated in the following cases:

  • allergies to amlodipine, derivative dihydropyridine or any ingredients of the drug.

    Be cautious when using

Safety and effectiveness of amlodipine in the hypertension has not been studied.

    Patients with heart failure:

  • Need to be cautious when treated. Controlled research with Placebo for a long time shows that the rate of pulmonary edema in the group using amlodipine is higher than the placebo group.

    Patients with liver failure:

  • Extended waste time and increased AUC value, no recommended dose in patients with liver failure.

    Patients with renal failure:

  • can use amlodipine at the usual doses.

Elderly: Be cautious when increasing the dose.

The effect of the drug on driving and operating machinery

Amlodipine may have a slight or medium impact on the ability to drive or operate machinery.

When taking amlodipin, patients may get dizzy, headache, fatigue or nausea, and alertness. Particularly cautious in the first stage of the tri.

used for pregnant and lactating women

    Pregnant women

Safety in pregnant women has not been established. In animals, reproductive toxicity is recorded in high doses. Only use amlodipine in pregnant women when there is no safer replacement and the condition that is likely to endanger the mother and the fetus.

    breastfeeding women

Amlodipin is discovered in infants when the mother is treated. It is unclear the impact of amlodipine on babies. The decision to continue or stop breastfeeding or use amlodipine based on the benefit of breastfeeding on babies and the benefits of amlodipin on mothers
