Uses

Indications

Losartan 25mg Stada is indicated in the following cases:

  • Hypertension (especially in patients with coughs when taking enzyme inhibitors Angiotensin) and reducing the risk of stroke in patients with left ventricular hypertrophy.
  • Kidney disease in patients with diabetes (serum creatinine in the range of 1.3 - 3.0 mg/dl in patients 60 kg with proteinuria).
  • Heart failure and myocardial infarction. Its properties prevents the effects of angiotensin II (causing vascular contractions and causing aldosteron secretion) by selecting the cohesion between Angiotensin II and AT1 receptor in many tissues (vascular smooth muscle, adrenal glands).

Metabolic substances are more active than Losartan from 10 - 40 times by weight and is an non -competitive, reversible inhibitor of the AT1 receptor.

Both Losartan and its active metabolites do not inhibit ACE (Kininase II, Angiotensin I into Angiotensin II and Bradykinin decomposition), they also do not bond or prevent receptors of other hormones or ion channels that play an important role in regulating the cardiovascular system.

    pharmacokinetics

Losartan is easily absorbed through the gastrointestinal tract when taken but significantly metabolized through the liver, resulting in the bioavailability of the whole body only about 33%. It is converted into metabolites with active properties of carboxylic acid derivatives E - 3174 (exp - 3174), which has a stronger pharmacological activity than Losartan; Some non -active metabolites are also formed.

The metabolism is mainly due to the isoenzyme of Cytochrome P450, CYP2C8 and CYP3A4. After 1 dose of oral, the peak concentration of Losartan's plasma is achieved within 1 hour and of E - 3174 achieved in 3-4 hours.

Both Losartan and E - 3174 have plasma proteins more than 98%. Losartan is excreted in urine and in the stool through the bile duct in a constant form and the form of metabolites. After drinking, about 4% of the dose is excreted in the form of constant and about 6% excreted in the form of metabolites that are active in urine. Losartan waste sale time is about 1.5 - 2.5 hours and of E - 3174 is about 3 - 9 hours.

    How to use

    Oral drugs.

    Dosage

    Hypertension:

    Adults

  • The common dose is 50 mg/time/day. If necessary, the dose can be increased to 100 mg x 1-2 times/day. The maximum hypotension effect is achieved after the beginning of treatment is about 3-6 weeks.
  • Initial dose 25 mg/time/day used for patients to reduce intravascular fluid volume. This dose is also suitable for patients with liver failure or renal failure.
  • Children aged 6 and older

Starting dose 0.7 mg/kg x 1 time/day, up to 50 mg, adjust the dose depending on the level of response. The dose is higher than 1.4 mg/kg or 100 mg/day has not been studied.

Kidney disease in patients with diabetes Type 2:

Initial dose of 50 mg/day/day, increasing dose up to 100 mg/day/day depending on the response on blood pressure.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose? The most common manifestation of overdose is hypotension and tachycardia. Slow heart rate can also occur due to sympathetic nerve stimulation (vagus nerves).

If symptoms of hypotension occur, supportive treatments should be taken. Can not remove Losartan or metabolites that are active by hemolysis.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using the drug, you may experience unwanted effects (ADR).

The unwanted effect of Losartan is usually mild and transient, including dizziness, headache, and hypotension related to the dose. Losartan causes less cough than other angiotensin transferring enzymes.

Cardiovascular: Hypotension can occur, especially in patients with reduced internal fluid volume (high -dose diuretic users).

    Respiratory: Respiratory disorders.

    musculoskeletal: back pain.

    Digestive: digestive disorders.

    Body: tired.

    Blood: neutrophils.

    Rare, ADR

    Urinary trend: renal function.

    Skin: rash, urticaria, itching, angioedema.

    Liver: Increase liver enzymes.

    Electrolyte: Hyarass hyperka.

musculoskeletal joints: muscle pain, joint pain, muscular solution syndrome.

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Losartan Stada 25 mg is contraindicated in cases of hypersensitivity to Losartan Kali or any ingredient of the drug.

    Precautions when using

Caution to use Losartan in patients with renal artery stenosis.

Losartan is excreted in urine and bile ducts, so the dose should be reduced in kidney failure and should consider for patients with liver failure.

Patients with reduced internal fluid volume (high -dose dosage users) may occur hypotension; Should adjust the situation of reducing the volume of intravascular fluid before taking the drug, or the starting dose is low.

Due to hypokalemia, serum levels should be monitored, especially in the elderly, patients with renal impairment and should be avoided simultaneously with potassium diuretics.

The ability to drive and operate machinery

There has been no research on reactions on the ability to drive and operate machinery. However, when driving or operating machinery must pay attention to dizziness and drowsiness can sometimes occur when treating hypertension, especially when starting treatment or when increasing the dose.

    Pregnancy

When used in the second quarter and third quarter of pregnancy, drugs that work directly on the Renin - Angiotensin system can cause damage, even death for the growing fetus. Stop using Losartan as soon as possible when detecting pregnancy.

    Breastfeeding period

Due to the harmful effects for breastfeeding babies, it is necessary to decide whether to stop breastfeeding or stopping the drug, depending on the importance of the drug for the mother.

    Drug interaction

In studies on drug interaction, there is no significant pharmacokinetic interaction between losartan and hydrochlorothiazid, digoxin, warfarin, cimetidine and phenobarbital.

rifampin (drug metabolic agents) reduce the level of losartan and its active metabolites.

Fluconazole (P450 2C9 inhibitor) reduces the concentration of active metabolites and increases the level of Losartan.

Like angiotensin II blockers or have the same effect, if used simultaneously with potassium diuretics (spironolacton, triamteren, amilorid), potassium supplements, salt -containing potassium replacements can lead to increased serum potassium levels.

As well as other hypertension drugs, the effect of anti -hypertension of losartan may be reduced when used with indomethacin (nonsteroidal anti -inflammatory drugs).

    Storage

Store in closed packaging, dry place, avoid light. The temperature does not exceed 30 ° C.
