Uses

indications

Losartan 50 mg is indicated in the following cases:

  • Treatment of hypertension: Can be used alone or in combination with other anti -hypertension drugs.

Angiotensin II, made up of angiotensin I in the reaction of the enzyme switching angiotensin (ACE), is a strong vasoconstriction, which is the main vascular activation hormone of the Renin - Angiotensin system, and is an important component in the pathological physiology of hypertension. Angiotensin II also stimulates the adrenal glands of Aldosteron.

Losartan and the main metabolites have a clogging and aldosteron secretion of angiotensin II by preventing Angiotensin II selectively, not close to the AT1 receptor is found in many tissues (for example, blood vessel muscles, adrenal glands). In many tissues, there is also an AT2 receptor but it is unclear whether this receptor has nothing to do with the cardiovascular air conditioner. Both Losartan and the main active metabolites do not show the local dominant effects in the AT1 receptor and have a much larger AT1 receptor affinity (about 1,000 times) than the AT2 receptor.

Losartan is a competitive, reversible inhibitor of the AT1 receptor. The metabolic substance has the activity of drugs 10 to 40 times stronger than Losartan, calculated by weight and is a non -competitive, reversible inhibitor of the AT1 receptor. Angiotensin II antagonists also have hemodynamic effects such as ACE inhibitors, but there is no unwanted effect of the common ACE inhibitors are dry cough.

    Dynamic pharmacokinetics

After drinking, Losartan absorbs well and initially metabolized through the liver thanks to the cytochrome p450 enzymes. The bioavailability of iOSartan is approximately 33%. About 14% of losartan dose taken into active metabolites, this substance is mostly opposing the Angiotensin II receptor resistance. Losartan waste sale time is about 2 hours, and of metabolites about 6-9 hours. The average peak concentration of Losartan is within 1 hour, and of the metabolites with activity within 3-4 hours.

both Losartan and active metabolites are highly linked to plasma proteins, mainly albumin and they do not pass the blood -brain barrier. Losartan's distribution form is about 34 liters and of metabolites with an activity of about 12 liters. The full clearance of Losartan's plasma is about 600 ml/minute and of the active metabolites of 50 ml/minute; Their clearance in the kidney corresponds to about 75 ml/min and 25 ml/min. After drinking Losartan marks 14C, collecting about 35% of the radioactive in the urine and about 60% in the feces.

Patients with mild to medium cirrhosis, the area under the curve (AUC) of Losartan and of the metabolites have a higher activity, corresponding, 5 times and 2 times higher than in patients with normal liver.

    How to use

    oral medication.

    Dosage

Dosage depending on the patient and must be adjusted according to the blood pressure response.

    Starting dose: 1 dose of 50 mg/day.

Patients are likely to lose epidemic in the heart (users of diuretic drugs), liver failure: The starting dose is 25 mg/day.

Maintenance dose: 25 - 100 mg/day. Depending on the blood pressure response of the patient.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when using overdose? The most common manifestation of overdose is probably hypotension and tachycardia, which may also experience slow heart rate due to sympathetic nervous stimulation (vagus nerve).

If symptomic hypotension occurs far, supportive treatment must be treated. Both Losartan and active metabolites cannot be removed by hemolysis.

What to do when you forget 1 dose? If it is nearly time to take the next dose, skip the forgotten dose and take medicine at the next recommended dose. Do not take double dose to compensate for the forgotten dose.

    Side Effects

When using Losartan 50 mg, you may experience unwanted effects (ADR).

Most of the unwanted effects are light and lost over time.

  • Cardiovascular: lower blood pressure.
  • Central nervousness: insomnia, dizziness.
  • Endocrine - Metabolism: hyperkalemia.
  • digestive: diarrhea, indigestion.
  • Hemorrhage: Light hemoglobin and hematocrit.
  • Neuropathy - bone: back pain, leg pain, muscle pain.
  • kidney: lower blood uric acid (when high doses).
  • Respiratory: cough (less when using ACE inhibitors), nasal congestion, sinusitis.
  • Uncommon:

  • Cardiovascular: Hypotenary pressure, chest pain, block A - V degree II, chest drum, sinus slow pace, tachycardia, face edema, blush.
  • Central nerve: anxiety, loss of air conditioning, confusion, depression, migraine, headache, sleep disorders, fever, dizziness. skin: hair loss, dermatitis, dry skin, erythema, light sensitivity, itching, urticaria, bruises, foreign rash.

  • ears: tinnitus.
  • kidney: urinary tract infections, slight increase in creatinine or urea.
  • Respiratory: Difficulty breathing, bronchitis, nosebleeds, rhinitis, airbodery, discomfort in the throat.
  • Other effects: sweating.
  • Instructions on how to handle ADR

Notify the doctor with unwanted effects when using the drug.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Losartan drug is contraindicated in the following case:

  • Hypersensitivity to any ingredients of the drug.
  • Be cautious when using

Special monitoring and/or decreased dose in patients dehydrated, diuretic treatment and patients have other factors that can easily lead to hypotension. Patients with kidney stenosis on both sides or one side, only one kidney has a high risk of unwanted effects (increased creatinine and blood urea) and should be closely monitored in treatment.

Patients with lower doses of liver failure.

The ability to drive and operate machinery

Losartan can be stunned, dizzy so it is not used for drivers and machinery operation.

    Pregnancy

Using drugs directly on the Renin - Angiotensin system in the middle and last three months of pregnancy can cause less amniotic fluid, hypotension, anuria, urinary, face deformation and death in infants. Although, the use of drugs in the first three months of pregnancy has not seen the risk of the fetus, but after detecting pregnancy, it is necessary to stop iosartan as soon as possible.

    The period of breastfeeding

It is not known whether Losartan will be secreted into breast milk or not, but there are significant amounts of Losartan and metabolites whether the drug's activity in mouse milk. Due to harmful potential for breastfeeding, deciding to stop breastfeeding or stopping the drug, taking into account the importance of the drug for the mother.

    Drug interaction

Losartan does not affect the pharmacokinetics of Digoxin oral or intravenously.

Take iosartan along with cimetidine increases the area under the curve (AUC) of Losartan by about 18%, but does not affect the pharmacokinetics of the active metabolites of Losartan.

Take Losartan along with Phenobarbital reduces about 20% of Losartan's AUC and of active metabolites.

There is no pharmacokinetic interaction between losartan and hydrochlorothiazide.

    Storage

Store in a dry place, not more than 300C, avoid light.
