Uses

Indications

Maxxcardio La 4 drugs are indicated for treatment of monochromatic hypertension or in combination with other antihypertensive drugs, including receptor anti-receptor drugs β-adrenergic, diuretics and enzyme inhibitors.

Pharmacokological

lacidipin is a drug belonging to the three -generation Dihydropyridin group opposite Calcium channel strong and specialized with high selective properties, mainly calcium channels in blood vessel plain muscle.

The main impact of the drug is to relax peripheral arteries, reduce peripheral blood vessels and reduce blood pressure.

After healthy volunteers drink 4 mg lacidipin, people have noted the minimum extension of QT (the average QTC increased between 3.44 to 9.6 ms in young and old volunteers).

pharmacokinetics

lacidipin is a high fat compound, it is absorbed quickly but poorly through the gastrointestinal tract after oral use. It is excessively transformed in the liver. Absolute biometical an average of about 10%.

Peak of plasma peaks achieved after 30-150 minutes after drinking> 95% lacidipine associated with plasma protein (albumin and alpha-1-glycoprotein).

lacidipin is converted into 4 main metabolites and pharmacological impacts of these metabolites if any, there are very few. The drug is excreted mainly through the liver metabolism (related to P450 CYP3A4). There is no evidence of lacidipine causing induction or inhibiting liver enzymes.

About 70% of the dose is eliminated in the form of metabolites in feces and the rest are metabolites in urine.

In a stable state, the average selling time of lacidipin is about 13 - 19 hours.

Before taking Maxxcardio La 4 Ampharco U.S.A tablets for hypertension (3 blisters x 10 tablets)

How to use

Maxxcardio La 4 medicine is taken orally. The drug should be taken in whole with a full glass of water, with or without food, should drink at the same time every day, preferably in the morning.

Dosage

Treatment of hypertension should be adjusted depending on the severity of the disease and patient response.

The starting dose is recommended for 2 mg once, daily. The dose may increase to 4 mg (then to 6 mg if necessary) after enough time for the pharmacological effects to fully indicate. In fact, this time should not be less than 3 to 4 weeks. The daily dose over 6 mg has not shown more obvious effect.

Patients with liver failure

The starting dose for patients with liver failure is 2 mg. Lacidipine is metabolized mainly in the liver and therefore in patients with liver failure, lacidipin's bioavailability can increase and increase the effect of increasing blood pressure. Should monitor these patients needing the patient, if necessary, the dose can be reduced.

Patients with kidney disease

Because lacidipine does not excrete through the kidneys, no dose adjustments in patients with renal disease.

Used in children and teenagers

Due to the lack of experience in safety and effectiveness of lacidipin in children, it is not recommended to use this drug for children and teenagers under 18 years old.

Older patients

No need to change the dose in older patients.

Treatment can continue for a long time.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose?

Symptoms of function and entity

Symptoms may include prolonged peripheral vasodilation that causes hypotension and tachycardia. Slow or prolonged Heart rhythms that can occur.

Treatment

There is no specific antidote. The common common measures should be taken to monitor heart function and appropriate support and treatment measures.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Side Effects

When using Maxxcardio La 4, you may experience unwanted effects (ADR).

lacidipin is often well tolerated. Some patients may experience mild side effects related to the pharmacological effects of peripheral vasodilation. These effects are often transient and often out when continuing to use lacidipine in the same dosage.

Common, ADR> 1/100

  • Nervous system disorders: headache, dizziness. Urinary tract: urine.
  • Cardiovascular disorders: worsen angina, fainting, hypotension.
  • Nervous system disorders: depression.
  • Nervous system disorders: Run.

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Maxxcardio medicine La 4 Contraindicated in the following cases:

  • Hypersensitivity to lacidipin or any ingredients of the drug. Stable.

    Caution when using

In specialized studies, Lacidipin has been shown not to affect the spontaneous function of the atrial sinus button (SA) as well as not causing extension of transmission in the atrial node (AV). However, in theory, it should be noted that the possibility of affecting the operation of the atrial and atrial node of calcium inhibitors, so lacidipine should be used cautiously in patients with abnormal history in the atrial and atrial node activity.

As reported with some Calci channel antagonists in the dihydropyridine group, Lacidipin should be used cautiously in patients with a congenital or congenital QT segment. Should be cautious when taking lacidipin for patients simultaneously with the drugs that are known to extend the qt segment such as anti -arrhyths group I and II, three -ring antidepressants, some anti -mental disorders, antibiotics (for example, erythromycin) and some antihistamines (for example: Terfenadine).

As well as other calcium antagonists, lacidipine should be used cautiously in patients with poor heart supply reserves.

As well as other Calci dihydropyridin antagonists, lacidipine should be used in patients with unstable angina pain as well as in patients with unstable angina during treatment.

There is no evidence that lacidipine is useful in secondary disease prevention.

Lacidipin's efficiency and safety in the treatment of malignant hypertension has not been determined.

lacidipine should be used carefully in patients with liver failure due to the effect of hypertension may increase.

There is no evidence that lacidipine reduces glucose tolerance or changes in diabetes control.

The drug contains lactose. For patients with galactose intolerance, lactase deficiency or glucose-galactose absorption should not be taken.

The ability to drive and operate machinery

lacidipin can cause dizziness. Patients should be warned that they should not drive or operate machines if they have dizziness or related symptoms.

    Pregnancy

although some dihydropyridin compounds have been found to be teratogenic in animals, mice and rabbits do not give evidence of lacidipin's teratogenic effects.

Excessive doses of treatment compared to the treatment range, lacidipine in animals shows evidence of toxicity on the mother due to increased miscarriage before and after nesting and may delay bone chemistry. Evidence from animal experiments has shown that lacidipin extends pregnancy and prolonged time and causes difficulty as a result of uterine muscle relaxation.

There is no data of lacidipin in pregnancy in humans.

lacidipin should only be used during pregnancy when the benefit of the mother is greater than the possibility of adverse effects on the fetus or babies.

The ability that lacidipine can cause uterine muscle relaxation: The possibility of lacidipine can cause uterine muscle relaxation at the end of pregnancy also needs to be considered.

    Breastfeeding period

lacidipin or its metabolites may be excreted in milk.

Should only use lacidipin during breastfeeding when the benefit for mothers is greater than the ability to have harmful effects on babies.

    Medicinal interaction

simultaneous use of lacidipine with other drugs that have other antihypertensive effects including hypertension medications (for example, diuretics, β-adrenergic receptor or transferred inhibitors) may have a plus effect of lowering blood pressure. However, there are no specific pharmacological interaction problems confirmed in studies with common hypertension medications (for example: beta and diuretics) or with digoxin, tolbutamid or warfarin.

Lacidipine concentration in plasma may increase when used simultaneously with cimetidine.

As well as for all drugs to treat hypertension causing vasodilation, it is necessary to be cautious when used with alcohol because this can increase the effect of the drug.

As other dihydropyridin, lacidipin should not be taken with grapefruit juice because biological uses can change.

In a clinical study in kidney transplant patients treated with ciclosporin, lacidipin has shown to prevent the decline of plasma flow through the kidneys and glomerular filtration.

lacidipin is metabolized by cytochrom CYP3A4, so be careful when taking lacidipin simultaneously with enzyme inhibitors CYP3A4, such as ketoconazol, iTraconazole or with CYP3A4 induction drugs, such as phenytoin, carbamazepin, phenobarbital, rifampicin and should be adjusted dose If necessary.

Simultaneous use of lacidipine with corticosteroids or tetracosactide drugs may reduce the effect of lowering blood pressure.
