Mecosol medicine is indicated in the following cases:

absorption
Esomeprazol absorbs rapidly through the gastrointestinal tract, the peak concentration in plasma is achieved after 1-2 hours.
Increased bioavailability depends on the dose and repeated use, about 68% - 89% for the dose of 20mg and 40mg respectively. Food slows down and reduces the absorption of the drug, although this does not significantly affect the effects of Esomeprazol on gastric acid secretion.
Distribution
Esomeprazol binds 97% to plasma proteins.
Metabolism and excretion
Esomeprazol is completely metabolized through the liver thanks to the Cytochrome P450 (CYP) system.
Most of the metabolism of Esomeprazol depends on the CYP2C19 enzyme, forming hydroxy and desimethyl metabolites of Esomeprazol - substances that do not affect gastric acid secretion. The rest of the metabolic process depends on the CYP3A4 enzyme, forming Esomeprazol Sulphone.
The process of metabolism through the liver for the first time and the body clearance decreases when the dose is repeated, the cause may be due to the inhibition of CYP2C19. However, Esomeprazol completely eliminated from plasma between doses without accumulation when used 1 time/day.
Selling time for plasma is about 1.3 hours. About 80% of Esomeprazol is excreted in urine in the form of metabolites, the rest is eliminated in feces.

How to use
oral Esomeprazol for at least 1 hour before eating, swallowing pills, not chewing or grinding pellet.
Dosage
Adults and children> 18 years old:
Esophagitis inflammation due to reflux: 1 tablet/day for 4 weeks. Should treat 4 more weeks for patients with untreated esophagitis or persistent symptoms.
Stomach ulcer - duodenum: 1 tablet/day, drink for 4 - 8 weeks.
Prevention and treatment of stomach - duodenal ulcer due to Helicobacter pylori infection: Combined with appropriate antibacterial regimen to eradicate Helicobacter pylori: 1 tablet/day for 7 - 10 days.
Treatment of Zollinger - Ellison syndrome: Start 1 tablet/day, then adjust to response. Most patients are controlled at a dose of 2-4 tablets/day, divided 2 times.
Elderly, kidney failure, mild to moderate liver failure: No dose adjustment.

What to do when overdose?
Symptoms
The oral dose of 160mg/time is still well tolerated. When using a high doses up to 2400mg, the symptoms occur very diverse, including confusion, drowsiness, blurred vision, fast heart, nausea, sweating, redness, headache and dry mouth.
Handling
There is no specific antidote. In case of overdose, symptomatic treatment and use of supportive measures should be used.

Drug interaction

Esomeprazol significantly reduces the concentration and AUC of Atazanavir. Increasing the dose of Atazanavir to 400mg has not compensated for the effects of ecomeprazol on the concentration and AUC of Atazanavir. Do not combine two drugs together.
Metabolic drugs by CYP2C19 enzyme
Concomitance Esomeprazol with metabolic drugs by CYP2C19 such as Diazepam, CitalPram, Imipramine, Clomipramine, Phenytoin ... as a concentration of these drugs in plasma can increase, need to decrease the dose.
Clinically, when using 40mg of Esomeprazol for patients who have used warfarin shows that blood clotting time is still within the allowed limit. However, some Inr increase cases have been reported when used in combination with warfarin with esomeprazol.
Esomeprazol has been shown to have no clinical effects on pharmacokinetics of amoxicillin or quinidine.
