Uses
Indications
Repoz - 20 drugs are indicated in the following cases:
Rabeprazole is a benzimidazole derivative, which inhibits proton pump. Rabeprazole is a new generation of proton pump inhibitors (PPI) in the benzimidazole nucleus, inhibiting stomach acid secretions due to the inactivated enzyme H+/K+–atpase in the stomach cells.
Rabeprazole has the effect of inhibiting basic gastric secretion and in a state of stimulation, no acetylcholine resistant or histamine H2 receptor antagonist, by inhibiting H+/K+ -–atpase enzyme in the cell wall of the gastric mucosa. This enzyme is considered an acid, hydrogen or proton pump in the cell so Rabeprazole is considered a proton pump inhibitor.
rabeprazole attaches to the enzyme H+/K+–atpase, blocking the final stage of gastric secretion. In the stomach wall cell, Rabeprazole is protonized and converted into activated sulfenamide, then attached to the cysteine of the proton pump as this enzyme inactivation.
pharmacokinetic
absorption
Rabeprazole is absorbed very quickly and the peak concentration in plasma reaches 3.5 hours after taking the drug.
Oral bioavailability is about 52% for tablets soluble in the intestine due to metabolized through the first circulatory and unchanged when the dose is one -time or the dose is repeated.
Distribution
Rabeprazole attaches nearly 97% to plasma proteins.
Metabolism
In vitro, Rabeprazole metabolized in the liver by the ISOENZE Cytochrome P450 (CYP2C19 and CYP3A4) into thioeters, thioeters of carboxylic acid, sulfon and desmethylioeter.
In test plasma concentrations, Rabeprazole does not touch or inhibit CYP3A4.
Elimination
Metabolic derivatives are excreted mainly through urine (nearly 90%), the rest is deducted through feces.
Rabeprazole's semi -waste time in plasma is about 1 hour, an increase of 2-3 times in patients with hepatic impairment, 1.6 times in CYP219 enzymes that are slowly metabolized and increased by 30% in the elderly.
