Medoclav 1g medicine is indicated for treatment in a short time of the following infections:

  • Upper respiratory infections: tonsillitis, sinusitis, otitis media have been treated with conventional antibiotics but does not decrease.

  • Lower respiratory infections by H. Influenzae and Branhamella CatVrbalis producing beta - Lactamase: acute and chronic bronchitis, pneumonia - bronchial.
  • Severe urinary - genital infections by E. coli, Klebsiella and Enerobacter produce beta - lactamase: cystitis, urethritis, pyelonephritis (female genital tract infection).
  • Skin infections and soft tissue: pimples, insects, teeth, cell inflammation, wound infections.
  • Bone and joint infections: Bone inflammation.

Other types of infections: infections caused by miscarriage, bacterial infection, abdominal infections.

    Pharmacy

Amoxicillin is a semi-synthetic antibiotic belonging to β-lactam group with wide bactericidal spectrum for many gram-positive and gram-negative bacteria due to synthetic inhibition of bacterial cell walls. But because Amoxicillin is easily destroyed by β-lactamase, therefore does not work for the strains of bacteria that produce these enzymes (multiple strains of Entobacteriaceae and Haemophilus influenza).

Clavulanic acid due to the fermentation of Streptomyces Clavuligerus, with β-lactam structure is similar to penicillin, is capable of inhibiting β-lactamase due to the majority of Gram-negative bacteria and Staphylococcus. Especially, it has a strong inhibition effect of β-lactamase transmitted via plasmid that resists penicillin and cephalosporin.

Pseudomonas Aeruginosa, Proteus Morganii and Rettgeri, some Entobacter and providentia anti -drugs, both anti -methicilin staphylococcus also. Clavulanic acid itself has very weak antibacterial effects.

Clavulanic acid helps amoxicillin not be destroyed by β-lactamase, and expand the antibacterial spectrum of amoxicillin effectively for many common bacteria that have resisted amoxicillin, resistant penicillins and other cephalosporins.

The bactericidal spectrum of the drug includes:

    bacteria

    strain

    Aerobic:

Streptococcus Faecalis, Streptococcus Pneumonia, Streptococcus Pyogenes, Streptococcus Viridians, Staphylococcus aureus, Corynebacterium, Bacillus anthracis, Listeria monocytogenes.

    anaerobic :

    Clostridium, peptococcus, peptostreptoccus species.

    Aerobic:

Haemophilus Influenza, Escherichia Coli, Proteus Mirabilis, Proteus vulgaris, Klebsiella, Samonella, Shigella, Bordetella, Neisseria Gonorrhoeae, Neisseria Meningitides, Vibrio Cholerae, Pasteurella Multocida.

    anaerobic:

    Bacteroides including B. Fragilis.

Amoxicillin and clavulanic acid are easily absorbed by oral. The concentration of these 2 substances in the serum is maximum after 1-2 hours of taking the medicine. The absorption of the drug is not affected by food and it is best to drink right before meals.

    Distribution

both clavulanate and amoxicillin have a ratio of low serum protein (about 25% for clavulanic acid and 18% for amoxicillin). Like other penicillins, amoxicillin can be excreted in breast milk. A very small amount of Clavulanat is also present in breast milk.

    Metabolism

Amoxicillin is partially eliminated in the urine in the form of peniciloic acid that is not active with an amount equivalent to 10-25% of the first dose.

Clavulanic acid is converted into 2,3-dihydro-4- acid (2-hydroxyethyl) -5-oxo-1h-1h-pyrroole-3-carboxylic and 1-amino-4-hygry-butan-2-one and eliminated in urine and in the form of carbon dioxide.

    Elimination

Similar to other penicillins, the main excretion path of amoxicillin is through the kidneys, while clavulanate eliminates by both paths that are through the kidneys and through the mechanism of the kidneys.

About 60 - 70% amoxicillin and 40 - 65% Clavulanic acid are discharged through urine in the form of unchanged in the first 6 hours after taking a single -dose 250/125mg or 500/125mg.

Concentrated with probenecid extends to eliminate amoxicillin but does not affect the excretion of the kidneys of clavulanic acid.

Crystal pisses in some cases leading to kidney failure have been reported after the overdose of amoxicillin in adults and children. Kidney damage has recovered. Hypertension can occur in people with kidney function damaged by reducing the elimination of both amoxicillin and clavulanic acid.

    Handling

In case you use more than the recommended dose, immediately notify your doctor or go to the nearest medical center for instructions.

What to do when forgetting a dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Contraindicated

Medoclav 1g medicine contraindicated in the following cases:

  • Sensitive to the beta - lactam group (penicillin, cephalosporin).
  • People with a history of jaundice or liver disorders due to the use of amoxicillin, clavulanate or penicillin.

    Be cautious when using

If you are old or have a history of jaundice, liver dysfunction due to amoxicilin and potassium clavulanate, be careful when taking this medication because clavulanic acid causes increased cholestatic risk in the liver.

It is necessary to be especially cautious when taking the drug if you have liver failure or show signs of liver dysfunction. Signs and symptoms of cholestasis jaundice are rarely occurring when taking the drug but may be severe. However, those symptoms are often recovered and will end after 6 weeks of stopping treatment.

If you have kidney failure, notify your doctor to adjust the dose accordingly.

Do not use Medoclav tablets if you have average or severe renal failure.

There has been a report on severe hypersensitivity reactions in patients with a history of penicillin allergy, so before starting treatment with amoxicillin, the doctor needs to notify the doctor if you have a history of allergies to penicillin, cephalosporin and other allergens.

Be cautious for patients with amoxicillin with redness and fever.

Do not take the drug for a long time because it can develop drug -resistant bacteria.
