Pharmacology

methylprednisolon is a glucocorticoid, 6-alpha-methyl derivative of prednisolon, anti-inflammatory, anti-allergic and immunosuppressive effects.

Anti -inflammatory effects of methylprednisolon increases 20% compared to the effect of prednisolon.

Glucocorticoid uses systemic sugar to increase the number of neutrophils and reduce the number of lymphocytes, eosin leukocytes, multi -core leukocytes in peripheral blood.

Glucocorticoid also inhibits the function of lymphocytes and tissue macrophages. Their response ability with antigens and split substances decreases.

Glucocorticoid also affects inflammatory reaction by reducing prostaglandin synthesis.

Glucocorticoid reduces capillary permeability due to inhibition of the activity of kinin, bacterial internal toxins and reduces the amount of histamine released by base leukocytes.

Dynamic pharmacokinetics

absorption: Methylprednisolon quickly absorbs and plasma concentrations reach a maximum of 1.5 - 2.3 hours after taking the drug. Absolutely highly available about 82 - 89%.

Distribution: Methylprednisolon is widely distributed throughout the tissue, through the brain blood barrier, and excreted into breast milk. The indicator distribution is approximately 1.4 liters/kg. Plasma protein attachment is about 77%.

Metabolic: Metabolic mainly in the liver, partly in the kidneys and then excreted through urine. Methylprednisolon is metabolized in the liver into non-active metabolites mainly 20α-hydroxymethylprednisolon and 20β-hydroxymethylprednisolon. The metabolism in the liver occurs mainly by CYP3A4 enzyme. Like the substrates of CYP3A4, methylprednisolon may be the substrate of p-glycoprotein, which can affect the distribution of tissue and interact with other drugs.

Elimination: The average selling time is about 1.8 -5.2 hours. The total clearance is about 5 - 6 ml/min/kg. No need to adjust the dose in patients with renal failure. Methylprednisolon can be eliminated through hemorrhage.

How to use

Dosage

Starting dose 4 - 48 mg/day. Dosage should be maintained and adjusted until the response is satisfied. If the treatment for a long time should determine the lowest dose that can achieve the need for gradually reducing the dose until the signs or symptoms increase.

Treatment of acute asthma attacks: 32 - 48 mg/day, used for 5 days, then additional treatment can be added at a lower dosage in a week. When from acute attacks, methylprednisolon is quickly reduced.

Severe diseases: The starting dose is 0.8 mg/kg/day divided into a small dose, then reinforced treatment for a single daily dose, followed by gradually decreasing to the minimum dosage.

Rheumatoid arthritis: The starting dose is 4 - 6 mg/day. During the acute phase, higher doses are 16 - 32 mg/day, then gradually decreases.

Chronic arthritis in children with life-threatening complications: Sometimes methylprednisolon is sometimes used in attacking therapy, at a dose of 10-30 mg/kg/batch (usually used 3 times).

Chronic ulcerative ulcer: severe exacerbations, taking 8 - 24 mg/day.

Nephrotic syndrome: The starting dose is 0.8 -1.6 mg/kg/day for 6 weeks, then gradually reduces the dose for 6-8 weeks.

Immune -soluble hemolysis: Take methylprednisolon 64 mg/day, for 3 days. Methylprednisolon must be treated at least 6 - 8 weeks.

Sarcoid disease: Take methylprednisolon 0.8 mg/kg/day. Using maintenance dose is low 8 mg/day.

Exacentality of scattered sclerosis: 160 mg/day, used for 1 week, then reducing the dose to 64 mg/day, used for 1 month.

Do not stop the drug suddenly after a long time of high doses, slowly reduced dose.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose?

When using too high doses for a long time, increase adrenal gland energy and inhibit the adrenal glands may occur. In these cases, it is necessary to consider to be able to decide properly to suspend or stop using Glucocorticoid.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
