Uses

indications

Mexams 10 is indicated in the following cases:

  • indicated in prevention and treatment of chronic asthma. Ltd, LTE4), which are eicosanoids that have a strong inflammatory effect, secreted from many types of cells, including mast cells and eosin blood cells. These important intermediaries are attached to cysteinyl leucotrien receptors (CYSLT1).

Cyslt type 1 receptors (CYSLT1) are found in human airfields, including smooth muscle cells and macrophages of the airway and in other pre -inflammatory cells (including eosin leukemia and some marrow cells).

CYSLT is correlated with pathological physiology of asthma and allergic rhinitis. In asthma, the intermediate effects of Leucotrien include a number of effects on the airway, such as bronchospasm, affecting the excretion of mucus, capillary permeability and the supplement of EOSIN.

In allergic rhinitis, CYSLT is secreted from the nasal mucosa after exposure to allergens in the reactions in fast and slow phases and are associated with allergic rhinitis symptoms. Montelukast is a compound associated with high affinity and selected with CYSLTI receptors. Montelukast inhibits LTD4's physiological activity at CYSLTI receptor. In humans, Montelukast inhibits cysteinyl leucotrien receptors in the airway, showing the ability to inhibit bronchospasm by Ltd4.

    Pharmacokinetics

    absorption

After drinking, Montelukast absorbed quickly and almost completely. In adults, the peak concentration in plasma is achieved after 3 hours after taking 10 mg at hunger. Born after drinking is 64%. Birth and peak concentration in plasma are not affected by the time of taking medicine.

    Distribution

The ratio of drugs linked to plasma proteins is 99%. The distribution volume (VD) in the stable state of Montelukast is 8 - 11 liters. Research on rats with Montelukast marks a minimum distribution through the bloody brain barrier.

    Metabolism

    Montelukast is quickly transformed.

    Elimination

Montelukast's clearance in plasma is 45 ml/minute in healthy adults. After taking Montelukast marking, 86% of the markers are found to be eliminated in the fertilizer of a total of 5 days and less than 2% of the urine. This shows that when taken, Montelukast and the metabolites of the drug are almost entirely through the bile.

    How to use

    oral medication.

    Dosage

    recommended dose:

Patients aged 14 years and older: 10 mg/time/day in the evening.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? It is unclear whether Montelukast can be separated through the peritoneal or blood vessel.

In an emergency, call the 115 emergency center immediately or go to the nearest local health station.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using Mexams 10, you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • Sensitive reaction: Including anaphylaxis, angioedema, itching, rash, urticaria .
  • Nervous system: anxiety, hallucinations, drowsiness, agitation including acts of aggression, hyperactivity, insomnia, abnormal/sensory reduction.

Digestive system: vomiting, nausea, indigestion, diarrhea .

  • liver: increase AST and ALT.
  • Muscle muscle: joint pain, muscle pain includes muscle contraction.
  • Hematoma: Increases the likelihood of bleeding, bleeding under the skin.

  • Other: Brushing the chest, edema.
  • Very rare, ADR

  • Hypersensitivity reactions: Eosin leukemia in the liver.
  • Nervous system: seizures.
  • liver: cholestatic hepatitis.

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Mexams 10 contraindicated in the following cases:

  • Patients who are sensitive to sodium montelukast or any ingredients of the drug.
  • Children under 14 years old.

    Be cautious when used

unknown effect when taking Montelukast in the treatment of acute asthma attacks. Therefore, Montelukast should not be used to treat acute asthma attacks. Patients need to be instructed to use the appropriate treatment available. Do not replace corticosteroids suddenly oral and inhaled with Montelukast. Although the Leukotrien receptor is not determined, it is necessary to be cautious and closely monitor clinically when reducing corticosteroid dose.

Do not use Montelukast as the only treatment for bronchospasm due to exertion. For patients with symptoms of exertion, regular treatment with inhaled beta blockers should be maintained as a backup solution. For patients with aspirin sensitive asthma should avoid using aspirin or nonsteroidal anti -inflammatory anti -inflammatory anti -inflammatory anti -inflammatorys while being treated with Montelukast.

When reducing the dose of corticosteroids using systemic sugar in patients using other anti -asthma drugs, including Leukotrien receptor antagonists, they will lead to rare cases of eosin hypernagus, rash, short breath, heart/or neurological complications.

The ability to drive and operate machinery

Some unwanted effects can be encountered when using drugs such as anxiety, hallucinations, drowsiness, excitement. Therefore, if appointed Montelukast, patients should avoid driving and operating machinery.

    Pregnancy

has not studied the use of Montelukast in pregnant women. Only use Montelukast when pregnant when really necessary.

    The period of breastfeeding

It is unclear Montelukast's excretion through breast milk. Be careful when using Montelukast during breastfeeding.

    Medicinal interaction

can be used Montelukast with other commonly used drugs in preventive and chronic treatment of asthma and allergic rhinitis treatment. In studies on drug interactions, the recommended dose in Montelukast's treatment does not significantly affect the pharmacokinetics of the following drugs: Theophyllin, Prednison, Birth control pills (Enthinyl Estradiol/Norethindron 35/1), Terfenadin, Digoxin and Warfarin.

The area under the curve (AUC) CIA Montelukast decreased by about 40% in humans with phenobarbital, no need to adjust the dosage of Montelukasi. In vivo studies show that Montelukast is CYP2C inhibitor.

However, data from medical interactive studies with each other clinically by Montelukast and Rosiglitazon (which represents drugs that are metabolized mainly by CYB2C8), replaced Montelukast without inhibiting CYB2C8 In Vivo. Therefore, Montelukast does not change the metabolism of drugs that are metabolized mainly through this enzyme (such as Paclitaxel, Rosiglitazon, Repaglinid).
