Uses
Indications
MIBEPLEN 5 mg Hasan 3 x 10 is indicated in the following cases:
Felodipine does not work on venous smooth muscles or on sympathetic vascular control, so it does not cause hypotension.
Felodipine has anti-angina and anti-anemia effects due to improving the supply-supply-oxygen balance of the heart muscle. Felodipine increases exertion and reduces the number of angina pain in patients with stable angina.
pharmacokinetics
After drinking, Felodipine is absorbed almost completely through the digestive tract and is initially metabolized in the liver. Birth is about 15%. When taking Felodipin at meals, the drug absorption rate increases and the maximum concentration of the drug increases to 60% when the meal has high carbohydrates and fat.
About 99% of the blood felodipine is linked to protein, mainly albumin. Felodipin's waste sale time is 11-25 hours.
The drug is strongly metabolized through the liver and the output is mainly in urine in the form of inactive metabolites, the rest is excreted through feces.

Adjust the dose in elderly patients or severe liver failure: The normal starting dose is 2.5 mg x 1 time/day, up to 5 mg/day.

What to do when overdose?
Symptoms
Excessive peripheral vessels causes significant hypotension and sometimes causes slow heart rate.
Handling
When severe hypotension appears, symptomatic treatment is required.
Patients need to lie on their backs, high legs. If the heart rate is slow, it is necessary to inject intravenous atropine 0.5 - 1.0 mg. If not effective, it is necessary to increase the volume of plasma with a transmission solution such as glucose, physiological saline or dextran. The sympathetic neurological medications have a stronger effect on the receptor α-1 (isoprenalin, dopamine or noradrenalin) that can be used if all of the above measures are effective.
In an emergency, call the 115 emergency center immediately or go to the nearest local health station.
What to do when forgetting a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

Yeast inhibitors (such as alcohol, cimetidin, erythromycin, otraconazole, ketoconazole) can increase felodipine levels in plasma.
Combining felodipine with intravenous dantrolen may be very dangerous because Verapamil (another calcium choice) when combined with intravenous dancrolen leads to ventricular vibration and death in experimental animals.
enamel induction substances (such as phenytoin, carbamazepin, rifampicin, barbiturat, primidon, oxacarbazepine ...) reduce felodipine levels in plasma.
baclofen enhances felodipine hypotension effect, so the dose adjustments should be adjusted accordingly.
iTraconazole increases the risk of edema because of reducing metabolism in the liver of calcium antagonists. Need to monitor and adjust the felodipine dose during and after combined with iTraconazole.
Use Felodipine combination with sympathetic beta blockers that can cause excessive hypotension, aggravating heart failure in patients with potential or uncontrolled heart failure.
corticosteroids: reduce anti -hypertension effect of felodipine.
sedative, anti -depression drugs Imipramine enhances the hypotension as well as the risk of hypotension.
