Uses

Indications

Micardis 80 pills are indicated in the following cases:

  • Treatment of idiopathic hypertension. Telmisartan selectively linked with AT 1. Telmisartan does not show affinity for other receptors, including AT2 and other AT receptors less characteristic. Aldosterone concentration in plasma is reduced by telmisartan.

Telmisartan does not inhibit human blood renin or prevent ion channels. Telmisartan does not inhibit the enzyme conversion of angiotensin (kininase II), the enzyme also decomposes Bradykinin. Therefore, it is said to not cause side effects through Bradykinin intermediaries.

    Pharmacokinetics

    absorption

Telmisartan's absolute average bioavailability is about 50%. When using Telmisartan along with food, the decrease in the area curve of time concentration in plasma (AUC 0-∞) of Telmisartan varies from about 6% (40 mg dose) to about 19% (160 mg dose). Until 3 hours after use, the concentration in plasma is similar whether Telmisartan is used when hungry or ate.

    Distribution

Telmisartan binds to plasma proteins greater than 99.5%, mainly albumin and Alpha-1 Glycoprotein. The indicating distribution volume in the average stable state (VDSS) is about 500 l.

    Metabolism

Telmisartan is metabolized by combining with glucuronid of the original compound into non -active substance.

    Elimination

After oral use, Telmisartan is almost except for feces, mainly in the form of unchanged compounds. Excretion accumulated through urine is less than 1% of the dose. Total clearance in plasma (about 1,000 ml/min) compared to the blood flow through the liver (about 1500 ml/min).

    How to use

Telmisartan tablets used to drink once a day and should be used with liquid, with or without food.

    Dosage

    Treatment of idiopathic hypertension

Effective doses are usually 40 mg x 1 time/day. Some patients may have benefited from daily doses of 20 mg. In the absence of target blood pressure, Telmisartan can be increased to 80 mg once a day.

In addition, Telmisartan can be used in combination with thiazide diuretic such as hydrochlorothiazide, which has been shown to have the effect of lowering blood pressure plus Telmisartan. When considering increasing the dose, it should be noted that the maximum hypotension effect is usually achieved from four to eight weeks after the beginning of treatment.

    Cardiovascular Prevention

recommended dose is 80 mg x 1 time/day. It is not known whether the dose lower than 80 mg of Telmisartan is effective in reducing the incidence of cardiovascular disease.

When starting with Telmisartan treatment to reduce the incidence of cardiovascular disease, it is advisable to closely monitor blood pressure and if possible, it is possible to adjust the medications that reduce blood pressure.

    Patients with renal failure

No dose adjustment in patients with renal failure, including patients with dialysis.

    Patients with liver failure

Do not use more than 40 mg once a day in patients with mild and medium liver failure.

    Elderly

    No dose adjustment.

    Children and teenagers

Not enough safe and effective data for drugs for children under 18 years old.

The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when overdose? Supporting treatment should be conducted if there are symptoms of hypotension. Telmisartan is not removed from the body when dialysis.

In an emergency, call the 115 emergency center immediately or go to the nearest local health station.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using micardis 80 , you may experience unwanted effects (ADR).

    Uncommon, 1/1000

  • Body: fatigue, headache, dizziness, limb edema, sweating.
  • Central nervous system: agitation. digestive: dry mouth, nausea, abdominal pain, acid reflux, indigestion, flatulence, anorexia, diarrhea.

urinary tract: Reduce kidney function, increase creatinine and nitrogen urea (bun), urinary tract infections . Respiratory: sore throat , sinusitis, upper respiratory infections (cough, congestion or ear pain, fever, nasal congestion, runny nose, sneezing, sore throat).

  • bone - joints: back pain, pain and muscle spasm.
  • Metabolism: hyperkalemia.
  • Rare, ADR

  • Systemic: Evaluation.
  • Eye: visual disorders .
  • Cardiovascular: tachycardia, reducing blood pressure or fainting (common in people with blood volume reduced or reduced salt, treatment with diuretics, especially in standing posture).

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Micardis 80 Contraindications used in the following cases:

  • Hypersensitivity to active ingredients or any excipients of the drug.
  • 3 months between and the last 3 months of pregnancy.
  • Biliary obstruction disorders.
  • Severe liver failure.
  • Contrain to use micardis with Aliskiren products in patients with diabetes or renal failure (GFR

    Be cautious when used

micardis is not used for patients with cholestasis, bile congestion disorders or severe liver failure because Telmisartan is eliminated most through bile. These patients may be reduced in the liver for Telmisartan. Micardis should only be used with mild to moderate liver failure.

Increased risk of severe hypotension and renal failure when patients with narrowed kidney stenosis on both sides or artery stenosis to a single active kidney is treated with drug products that affect the renin-angiotensin-krosterone system. When using micardis for patients with impaired renal function, periodic monitoring of potassium and serum creatinine should be monitored. There is no experience in using micardis in recent kidney transplants.

Symptomic hypotension, especially after the first dosage of micardis, can occur in patients with decreased volume and/or sodium due to strong diuretic treatment, limiting salt in diet, diarrhea or vomiting. The decline in volume and/or sodium should be adjusted before using micardis.

There is evidence that the use of enzyme inhibitors simultaneously, Angiotensin II or Aliskiren receptor blockers increases the risk of hypotension, hyperkalemia and reducing kidney function (including acute renal failure). Therefore, it is recommended not to use the drugs at the same time on the RAAS system (enzyme inhibitors, angiotensin II or Aliskiren receptor blockers).

Angiotensin II receptor inhibitors should not be used simultaneously for patients with diabetes due to diabetes.

In patients with vascular tone and renal function depends mainly on the activity of the renin-angiotensin-aldosterone system (for example, patients with severe congestive heart failure or potential kidney disease, including kidney stenosis), treated with drug products that affect this system such as Telmisartan related to acute hypotension, hyperburgery, rarity, rarity.

Patients with primary aldosteron hypernigine in general will not respond to antihypertensive drugs that inhibit the Renin-Anotensin system. Therefore, the use of Telmisartan is not recommended.

As well as other vasodilators, especially cautious in patients with aortic valve stenosis or mitral valve, or obstructive hypertrophic cardiomyopathy.

In these patients, hypoglycemia can occur when treated with Telmisartan. Therefore, in these patients, it is necessary to consider and monitor appropriate blood sugar; Insulin or anti -diabetes may be required, when indicated.

The use of drug products that affect the renin-anidensin-aldosterone system can cause hyperboly.

In the elderly, in patients with renal impairment, in patients with diabetes, in patients treated simultaneously with other drug products, which can increase the concentration of potassium and/or in patients with simultaneous events, increased potassium can be fatal.

Telmisartan and other Angiotensin II receptor antagonists seem to be less effective in lowering blood pressure in blacks than non -black people.

As with any lowering hypotension, excessive reducing blood pressure in patients with myocardial ischemia or cardiovascular disease due to ischemia can lead to myocardial infarction or stroke.

The ability to drive and operate machinery

When driving or operating machinery, it should be noted that sometimes it may occur dizzy or drowsiness when using hypotension like micardis.

    Pregnancy

The use of Angiotensin II receptor drugs is not recommended in the first three months of pregnancy.

Contraindicated use of Angiotensin II receptor antagonists in the middle and last 3 months of pregnancy.

    Breastfeeding period

Do not take medicine for breastfeeding women.

    Drug interaction

    digoxin

When micardis is used simultaneously with digoxin, it will increase the concentration of digoxin in plasma (49%) and bottom concentration (20%).

Potassium -saving diuretic or potassium supplements

Micardis medicine reduces potassium loss due to diuretic. Potassium -saving diuretic such as spirinolactone, eplerenone, triamterene, or amiloride, potassium supplements, or salt -containing salt substitutes can lead to significant increase in serum potassium. If used simultaneously indicated for hypokalemia has been recorded, they should be used carefully and regularly monitor serum potassium.

    lithium

There has been a report on the increase in lithium and toxic lithium recovery when using simultaneously lithium with transferred enzyme inhibitors and with Angiotensin II receptor antagonistic drugs, including micardis. If the use is necessary, should carefully monitor the serum lithium concentration.

NSAID can reduce the effect of lowering the blood pressure of micardis.

Pre -dosage treatment with high -dose diuretic such as Furosemide (diuretics) and hydrochlorothiazide (thiazide diuretics) can lead to decreased volume and risk of hypotension when starting with micardis treatment.

Micardis's hypotension effects can be increased when used simultaneously with other antihypertensive drugs.

corticosteroids when used with micardis reduces the effect of lowering blood pressure.
