Uses
Indications
Mobic® 15 mg/1.5 ml is indicated in the following cases:
In Vivo (In Vivo), Meloxicam inhibits prostaglandin biosynthesis in a stronger inflammatory position in the stomach or kidney mucosa. The above differences are related to selective inhibition on COX - 2 compared to COX - 1 and it is believed that the inhibition of COX - 2 provides the therapeutic effects of NSAIDs.
pharmacokinetic
absorption
Meloxicam is completely absorbed after intramuscular injection. Relative bioavailability compared to oral form is approximately 100%.
Therefore there is no need to adjust the dose when transforming from intramuscular treatment to oral form.
The peak concentration of plasma is achieved after the injection of 1-1.6 hours at a dose of 15 mg is 1.6-1.8 μg/ml.
After the intramuscular injection, linearly the dose is shown within the scope of treatment from 7.5 mg to 15 mg.
Distribution
Meloxicam is strongly linked to plasma proteins, mainly albumin (99%). Meloxicam penetrates well into joint fluid and achieves a concentration of approximately half of plasma concentrations.
Low distribution volume, approximately 1L after intramuscular or intravenous, and shows vary depending on the individual from 7 - 20%.
Metabolism
Meloxicam metabolizes strongly through the liver. The metabolic deformation of Meloxicam through the urine has been determined to have no biological activity. In vitro research shows that CYP 2C9 plays an important role in this transformation line with a small contribution from Isoenzyme CYP 3A4.
Antioxidant activity in patients is probably responsible for two other metabolites, with the number of 16% and 4% corresponding to the dose.
Era:
Meloxicam is excreted mainly through urine and stools at the same level as a metabolic.
less than 5% of the dose is excreted in the form of intact, while spending a very small amount is discharged through the urine in the form of integer.
The average selling time ranges from 13 and 25 hours after drinking, intramuscularly and intravenously.

How to use
Mobic® medicine is used in intramuscularly, not used intravenously.
Dosage
only used for adults in the first few days of treatment, injection 7.5 - 15 mg/ time/ day. Do not overdose 15mg/ day. Then maintain oral treatment.
For patients with kidney failure dialysis, the maximum dose is 7.5mg/day.
Because the dosage for children and teenagers has not been determined, limit the use of injection solutions for adults.
Because it is likely that the adverse reactions increased with the dose and time of use, so the drug should be used in the shortest possible time and with the lowest dose to be effective.
Mobic's daily daily dose should be used as single dose. The maximum daily dose is recommended for 15 mg for any form of preparation.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when using overdose? A clinical trial shows that Cholestyramine increases the speed of Meloxicam elimination.
What to do when forgetting a dose?

Instructions on how to handle ADR
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Mobic® 15 mg/1.5 ml contraindicated in the following cases:
People under 16 years old.
History of hemorrhage or gastrointestinal perforation, related to the previous nsaids .
Gastric ulcer - duodenum progression or history of stomach -duodenal ulcer.
Active intestinal inflammation (Crohn's disease, ulcerative colitis).
Serious liver function impairment .
Severe kidney failure.
Gastrointestinal bleeding, blood vessel bleeding or other bleeding disorders.
Severe heart failure.
Pain after the northern coronary surgery (CABG).
Hypersensitivity to Meloxicam or with one of the excipients or hypersensitivity to the same effects, such as NSAIDs, Aspirin.
Disease asthma , nasal polyps, nerve edema or urticaria after taking aspirin or other nsaids.
Pregnant and lactating women.
Precautions for use
Caution for patients with a history of gastrointestinal diseases and patients being treated with anticoagulant drugs.
Must regularly monitor the symptoms of the gastrointestinal tract, if the ulcerative or duodenal ulcer or gastrointestinal bleeding appears to stop taking the drug.
These symptoms may be fatal at any time without a warning sign or a history of severe disasters on the gastrointestinal tract.
Serious skin reactions, some may cause death, including skin peeling, Stevens - Johnson syndrome, epidermal poisoning, very rare reports related to the use of nonsteroidal anti -inflammatory drugs. Patients who have the highest risk of these adverse adverse reactions are usually in the early stages when they start treatment, in most cases, the reaction onset in the first month of treatment. MoBic should be stopped as soon as the skin appears on the skin, lesions in the mucosa or any signs of sensitivity.
The drug increases severe cardiovascular thrombosis, myocardial infarction , stroke, which can lead to death, the highest risk subjects include:
Elderly patients.
Patients with tears, congestive heart failure, cirrhosis, nephrotic syndrome and the kidney disease are noticeable.
Patients are being treated simultaneously with diuretics, enzyme inhibitors or angiotensin II receptor inhibitors.
Patients who are undergoing surgery can lead to reduced blood volume.
In the aforementioned patients, it is necessary to closely check the volume of urine and kidney function at the beginning of treatment.
More rare, nonsteroidal anti -inflammatory drugs can cause interstitial nephritis, glomerulonephritis, kidney necrosis or nephrotic syndrome.
Mobic's dose in patients with end -stage renal impairment is dialysis must not exceed 7.5 mg. There is no need to reduce the dose in patients with mild or medium renal failure (as in patients with creatinine clearance on 25 mi/minute).
Mobic increases the serum transaminase or other parameters of the liver function. In most cases, a slight increase is on normal and transient limits. If the abnormalities are significant or prolonged, it is necessary to stop using mobic and conduct monitoring tests.
No dose reduction in clinical stable cirrhosis patients.
Need to monitor carefully in weak or weak organs that endure the side effects of the drug.
Be careful when taking drugs in elderly patients because they are more likely to have impaired renal, liver or heart function.
NSAIDs can cause sodium, potassium and water salts as well as prevent the effect of stimulating sodium secretion in urine of diuretics. Heart failure or hypertension may appear or worsen in sensitive patients. But patients are at risk of clinically closely monitored.
Meloxicam, as well as other NSAIDs can cover the symptoms of the main infection.
Use meloxicam as well as cyclooxygenase inhibitors/other prostaglandin synthesis may cause harmful effects on reproduction and are recommended not to be used for women who want to get pregnant.
Therefore, women are difficult to get pregnant or women who are conducting a poor reproductive function to consider stopping treatment with Meloxicam.
The ability to drive and operate machinery
The drug with pharmaceutical ingredients is Meloxicam dizzy, confused , loss of orientation, blurred vision should not be used for drivers and operating machinery.
Pregnancy
Experimental research does not see Meloxicam's teratogenic evidence. But Meloxicam is still recommended not to be used for women in the last 3 months of pregnancy.
breastfeeding period
Meloxicam is recommended not to be used for breastfeeding women.
Drug interaction
Pharmacological interaction
Other nonsteroidal anti -inflammatory drugs (NSAIDs) and acetylsalicylic acid from 3 g/day or more.
corticosteroid
Increases the risk of bleeding or gastrointestinal ulcer.
anticoagulant or heparin is used in geriatrics or at healing dose
significantly increases the risk of bleeding.
Medications that soluble thrombosis and anti -platelets
significantly increases the risk of bleeding.
Selective Serotonin reabsorption inhibitors (SSRI)
Increased risk of gastrointestinal bleeding.
Diuretics, enzyme inhibitors and Angiotensin-II antagonistic drugs
Reduce the effects of diuretics and other antihypertensive drugs.
other antihypertensive drugs (such as beta blockers).
Calcineurin inhibitors (such as cyclosporin, tacrolimus)
The toxicity on the kidneys of calcineurin inhibitors can be enhanced.
uterine tools
Reduce the effectiveness of uterine tools.
pharmacokinetic interaction (the effect of meloxicam on the pharmacokinetics of other drugs)
lithium
Increases blood lithium concentration (due to reduced lithium excretion in the kidneys), can reach toxic value.
methotrexate
NSAIDs can reduce methotrexate excretion through the renal tubules, thus increasing the concentration of methotrexate in plasma. Thus, patients need to notify the doctor accurately about the drugs being used so as not to cause adverse interactions.
Interaction with alcohol
Do not use alcohol during treatment with meloxicam due to increased risk of gastrointestinal bleeding.
taboo
No mobic mixed with other drugs.
Storage
Store in a dry place, avoid direct sunlight and temperature from 15 - 30 ° C.
