indications

Momate ointment - S are indicated in the following cases:

  • Treatment of psoriasis psoriasis. Corticosteroids have a very diverse mechanism of action including anti -inflammatory effects, immunosuppressive, anti -regeneration. Anti -inflammatory effects are the result of reducing the formation, secretion and activity of inflammatory intermediaries (such as kinin, histamine, liposom enzymes, prostaglandin, leukotrien), thus reducing the initial manifestations of the inflammatory process. Corticosteroid inhibits the movement of cells to the damaged area, restores the dilated vessels and increases the permeability in the area, reducing the access to cells into the damaged area. Vascular contractions reduce serum, swelling and discomfort.

The immunosuppressive effect reduces response to instant and slow sensitivity reactions (Type and II II III reactions). The result is due to the inhibition of toxic effects from antigen and antibodies formed in the vascular walls that cause vasculitis subcutaneously due to allergies, and the inhibition of the effects of lympholin, target cells and macrophages, the same agents that cause contact dermatitis reactions. In addition, the approach of sensitive le leukocytes and macrophages to target cells can also be prevented by corticosteroids. The anti -regeneration effect of the drug reduces the increase in tissue of psoriasis.

Salicylic acid peels over excessive horny epithelium due to dissolving substances between the horn cells, causing the tissue to bulge, soft, wet and peel off. Salicylic acid at a concentration of 3 to 6% has the effect of eliminating horns, it has the effect of destroying tissue at a concentration of over 6%, the concentration from 6 to 60% is used to remove bottles or warts, in the treatment of psoriasis and other over -horn diseases.

    pharmacokinetics

The starting time has the effect of combination of Salicylic Acid and Mometasone.

Psoriasis skin treatment: will begin to progress well after 4 days of using a combination of salicylic acid/minetasone acid twice a day. This is the first time to evaluate the effect, it may not show the true improvement effect of therapy.

    Best effect

Psoriasis: Applying combined treatment for salicylic acid and mometasone twice a day, in 21 days, obtaining the maximum results at the end of the treatment. Studies with longer treatment time have not been published and the best time is not known.

    absorption

Dynamic data on use of skin -to -skin use combined with salicylic acid and mometasone are not available. It is unclear whether the absorption or other pharmacokinetic parameters are affected when used in combination.

When using Mometasone Furoate 0.1%, body absorption is 0.7%. Closed bandages or for inflamed skin can increase the absorption through the skin. There is a great difference in the levels of corticosteroid absorption over the skin between individuals and different positions: for example, 1%forearm; Scalp 4%, forehead 7%, scrotum 36%, the maximum permeability level is seen in the groin, armpits and face. Skin absorption may increase when the skin is inflamed or other disorders.

There is no observation of any relationship between the volume of topical and absorption. The rate of penetration through the skin or through the hair follicles is limited by the skin's transportation.

When using Salicylic Acid Eintment 5%, 25.1% of Salicylic Acid is absorbed.

    Metabolism

Mometasone Furoate is very strong in the liver after being applied on the skin and is absorbed. Salicylic acid is converted into salicylate glucuronides and salicyluric acid.

    Elimination

After absorbing through the skin, corticosteroids are eliminated mainly through the kidneys and a small part of the bile. The half -life of Mometasone is 5.8 hours.

Salicylic acid exhaust level is 95 % and the half -life of serum cancel is about 2 to 3 hours with low doses of salicylic acid, if high doses are used, the half -life period is up to 12 hours (with Salicylic Acid that is absorbed in the body).

    What to do when overdose?

    Symptoms

Local corticosteroids for too long time can inhibit the pituitary-renal function, which causes secondary adrenal implants. Symptoms of salicylic acid poisoning include tinnitus, nausea and dizziness.

    Treatment

In case of overdose must apply appropriate symptomatic treatment. Symptoms of acute corticosteroid overdose can recover. Need to treat electrolyte imbalance. In case of chronic poisoning, corticosteroids should be stopped gradually.

In an emergency, call the 115 emergency center immediately or go to the nearest local health station.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Secondary adrenal implants and other undesirable effects (see caution).

The most common adverse reactions in patients with scales should be used for 0.1%mometasone/Salicylic acid 5%are topical reactions such as burning, irritation, itching and skin atrophy (the total adverse effect is 10 - 20%). Most of them are from light to medium. Some patients have to stop treatment due to local reaction.

The most serious complication is that skin atrophy is seen in 4% of patients taking 3 -week drugs in 1 study. This ratio is larger than using Mometasone alone (0.7%) so it should not be used for a long combination.
