Support treatment for antibiotics when symptoms or signs of infection.
Pharmacokic
Mometason Furoat is a synthetic glucocorticoid, with local anti -inflammatory properties at the dose levels without causing systemic effect. Many mechanisms of anti -allergic and anti -inflammatory effects of Mometason Furoat are because it produces inhibits the release of intermediate substances that cause allergic reactions. Mometason Furoat significantly inhibits Leukotrienes release from leukemia of an allergy patient.
In cell culture, Mometason Furoat has proven highly valid in inhibiting the synthesis and release of IL - 1, 1 - 5, IL - 6 and TNFα substances, it is also a strong inhibitor for the production of Leukotriene. In addition, Mometason Furoat is also a strong inhibitor to the floor of Cytokine TH2, IL - 4 and IL5 from T -CD4 cells.
pharmacokinetic
Mometason Furoat is poorly absorbed after steaming, nasal spray and on -site use. Mometason Furoat, used in a liquid form for nasal spray, has a whole body to be less than 1% in plasma, when using a test with sensitivity to the quantitative limit lower than 0.25 PG/mL. Studies show that the absorbed Mometason Furoat will be metabolized through the liver for the first time into many metabolites.
In the liver, the drug undergo a process of metabolism mainly by Cytocrom P450 3A4 (CYP3A4). Do not detect main metabolites in plasma. In Vitro's study, one of the secondary metabolites is 6β - hydroxyletason furoat. The sale time of the drug is about 5 hours, the metabolites are excreted mainly in feces and a smaller part through the urine.

When symptoms are controlled, the dose should be reduced to 1 spray for each nostril 1 time/day (total dose of 100 μg). If the symptoms have not been fully controlled, it may increase the daily dose daily is 4 sprays for each nostril 1 time/day (total dose of 400 μg). Advice to reduce the dose after the symptoms have been controlled. Patients should continue to use regularly to achieve maximum treatment effect.
Children from 2 - 11 years old: recommended dose for a day is 1 spray for each nostril 1 time/day (total dose of 100 μg).
Backup dose
Adults (including elderly patients) and children of 12 or more: The recommended dose is 2 sprays for each nostril 1 time/day (total dose of 200 μg). In patients who know the antigen -causing antigen in the seasonal allergic rhinitis, it is recommended for preventing Monitazone nasal sprays at a dose of 200 μg a day from 2 to 4 weeks before the expected time to start the pollen season.
nose polyps
Normal recommendations are 2 sprays for each nostril 1 time/day (total dose of 200 μg). If the symptoms are still not controlled after 5 to 6 weeks, the maximum dose can be increased by 2 sprays for each nostril twice a day (total dose of 400 μg). Recommendation of reducing dose after control of symptoms. If the symptoms are still not fully controlled, the replacement of other treatment therapies should be considered. There is no case of using Monitazone nasal spray in the treatment of nasal polyps lasting more than 4 months.

Precautions when using
Precautions when using
Monitazone nasal spray must be used for patients with non -functional or active respiratory tuberculosis infections, or systemic, bacterial, fungal infections that have not been treated or herpes simplex in the eye.General warning
After 12 months of treatment with Monitazone nasal spray, there is no evidence of nasal atrophy, as well as, Mometason Furoat tends to restore the nasal mucosa close to normal model phenomenon. As with any long -term treatment, the patient uses Monitazone nasal spray for a few months or longer needs to be checked periodically on possible changes in the nasal mucosa. If the fungal infection appears in the nose or throat, it is necessary to stop the treatment of Monitazone nasal spray or may require appropriate treatment. Persistent nasopharyngeal irritation may be an indication to stop using Monitazone nasal spray.
Although Monitazone nasal spray will control the nasal symptoms in most patients, simultaneously use appropriate therapies that can reduce other symptoms, especially eye symptoms.
There is no evidence of hypothalamus - pituitary - adrenal glands (HPA) after prolonged treatment with Monitazone nasal spray. However, patients shifting from long -term use of corticosteroids to the body to Monitazone nasal spray should be carefully monitored.
Corticosteroid cessation of systemic corticosteroids in these patients can lead to adrenal insufficiency for several months until the function of the HPA shaft is restored. If these patients show signs and symptoms of adrenal insufficiency, the use of generalized corticosteroids must be repeated and appropriate assessments must be conducted.
While changing from the use of whole corticosteroids to Monitazone nasal sprays, some patients may have symptoms of stopping the use of systemic corticosteroids (such as joint pain and/or muscle pain, fatigue and beginning to weaken nasal symptoms and will need encouragement to continue therapy with Monitazone nose spray. Such medication can also reveal allergic condition that exists in advance such as allergic conjunctivitis and Eczema, which has been inhibited earlier due to systemic therapy.
Patients who use corticosteroids that are likely to be inhibited immunosuppressant must be warned about the risk of exposure to infections (such as chickenpox, measles) and the importance of asking for a physician if there are such contacts.
The ability to drive and operate machinery
The drug only works on the spot so it does not affect the ability to drive and operate machinery.
Pregnancy
There are no complete and strict studies on the use of Monitazone nasal spray in pregnant women. After the nasal spray the maximum clinical recommendations for patients, the concentration of mometason in plasma is not measured. Therefore, the exposure of the fetus with the drug is thought to be negligible and the ability to be toxicity with reproduction is very low.
As well as other corticosteroids using nasal sugars, Monitazone nasal spray should only be used for pregnant women when the benefits may be greater than the potential risk for mothers and fetuses. Children with mothers use corticosteroids during pregnancy must be carefully monitored about adrenal insufficiency.
Breastfeeding period
as well as other corticosteroid products, Monitazone nasal spray should only be used for breastfeeding women when benefits may be greater than the potential risk for mothers and young children.
Drug interaction
There is no official research on drug interaction with Monitazone nasal spray.
Cytochrom P450 3A4 inhibitors: Studies show that Mometason Furoat is mostly metabolized as the liver and through other complex metabolic mechanisms. In In vitro studies confirm the main role of Cytochrom P450 3A4 inhibitors into this complex trip. When combining Mometason Fiiroat with Ketoconazole, a powerful CYP 3A4 inhibitor can increase the serum concentration of Mometason Furoat.
