Domperidone is homogeneous with Dopamine receptor receptor with the effect directly on the receptor activation area. In addition, Domperidone also has a serotonine receptor antagonist (5 - HT3) and is used as an anti -vomiting drug in short -term treatment of nausea, vomiting due to different origins.

Simethicone is more useful and abdominal discomfort due to stagnant stasis in the gastrointestinal tract as in indigestion and gastroesophageal reflux disease.

    Patients with liver failure

mutecium - M contraindicated with patients with medium and severe liver failure. No need to adjust the dose for patients with mild liver failure.

    Patients with renal failure

Due to the prolonged domeridone's sale time in patients with severe renal impairment, if used to repeat, the number of times the taken of mutecium - m should be reduced to 1-2 times/day and adjust the dose depending on the level of renal failure.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? Should monitor the electrocardiogram due to the ability to cause extension of QT.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using mutecium - m , you may experience unwanted effects (ADR).

    contraindicated

Mutecium - M contraindicated in the following cases:

  • Sensitive to any ingredients of the drug.
  • Vomiting after surgery, gastrointestinal bleeding, mechanical bowel obstruction.

  • Use domperidone regularly or long -term.
  • Patient Hepatic failure average and severe.
  • Patients with prolonged heart impulse transmission time, especially about QT.
  • Patients with obvious electrolytes or patients with cardiovascular disease such as congestive heart failure.
  • Simultaneously used with drugs that extend QT (see "Drug interaction").
  • Simultaneously used with strong CYP3A4 inhibitors.
  • Be cautious when using

can only use mutecium - m without more than 12 weeks for people Parkinson's disease .

Be cautious when used for people with liver dysfunction.

Use domperidone with the lowest doses effectively in adults and children.

    kidney failure

Domperidone's waste time is prolonged in patients with severe renal impairment. In the case of repeated use, the frequency of using domperidone should be reduced to 1-2 times/day depending on the level of renal failure. Can adjust the dose if needed.

    Heart effect

Domperidone extends the QT distance on the electrocardiogram. In the process of post -commerce monitoring, there are very few reports on the QT interval and the top torsion related to the use of domperidone. These reports have risk factors such as electrolyte disorders or simultaneous medications.

Epidemic studies show that domperidone may increase the risk of serious ventricular arrhythmia or sudden cardiovascular disease. This risk is higher for patients over 60 years old, patients taking daily doses greater than 30 mg, patients use simultaneously the drug extends the QT or CYP3A4 inhibitors.

Need to stop treating with domperidone and exchange with health workers if there are any symptoms or signs related to arrhythmia.

Advise patients to quickly report symptoms on the heart.

The ability to drive and operate machinery

domperidone does not affect or negatively affect the ability to drive and operate machinery.

    Pregnancy

Do not use domperidone for pregnant women.

    Lactation period

domperidone excreted through breast milk and breastfed babies receive less than 0.1% dose according to the mother's weight. The adverse effects, especially the effect on the heart can still occur after breastfeeding.

Consider the benefits of breastfeeding and the benefits of treatment for mothers to decide to stop breastfeeding or stop/avoid domeridone treatment. Caution should be careful in case of risk factors that extend the QT range in breastfeeding.

    Interactive drug

anti -cholinergic drugs that can inhibit the effect of mutecium - M. If forced to use this combination of drugs, Atropine can be used after giving mutecium - m.

If using mutecium - m along with antacids or acid secretions, mutecium - m must be taken before meals and must take antacids or acid secretion inhibitors after meals.

Increase the risk of extending the QT distance due to pharmacokinetics or pharmacokinetic energy.

    Contraindicated with the following drugs:

Medications that extend the QT range:

  • Anti -arrhythmic drug IA (for example: disopyramide , hydroquinidine, quinidine). (Example: Haloperidol, Pimozide, Serttindole). Pentamidine). mizolastin).

Strong CYP3A4 inhibitor (regardless of the effect of extending QT) such as protease inhibitors, Azole body antifungal drugs, some macrolide groups (erythromycin, clarithromycin and telithromycin).

Do not recommend simultaneous use with medium CYP3A4 inhibitors (for example: diltiazem, verapamil).

Use carefully when used simultaneously with slow -hearted drugs, medications that reduce blood potassium and some following macrolide drugs contribute to extending QT interval: azithromycin, Roxithromycin (contraindicated clarithromycin due to strong CYP3A4 inhibitors).

The list of substances above is representative and incomplete drugs.

To avoid interactions between drugs, notify the doctor or pharmacist about the drugs being used.
