Uses
Indications
Negacef 500 is indicated in the following cases:
Cefuroxime has antibacterial activity due to synthesis of bacterial cell walls by attaching essential target proteins (penicillin -mounted proteins). Causes of drug resistance may be due to bacteria secretion of cephalosporinase enzymes or due to changes in penicillin -mounted proteins. Cefuroxime has an effective antibacterial activity and is very characterized by many common pathogens, including beta -lactamase/cephalosporinase strains of both gram -positive and gram -negative bacteria. Cefuroxime is especially durable with many beta enzymes - lactamase of Gram -negative bacteria.
antibacterial spectrum
Cefuroxime has an aerobic and anaerobic gram -negative and anaerobic activity, including most staphylococcus strains of penicillinase secretion and has gram -negative intestinal bacteria. Cefuroxime has high activity, so the minimum inhibitory concentration (mic) is low for streptococcus strains (group A, B, C and G), gonococcus and meningoccus strains. Initially, Cefuroxime has a low mic for gonococcus strains, Moraxella catrhalis, Haemophilus influenzae and klebsiella sp. The beta - lactamase secretion but now in Vietnam, many bacteria have been resistant to cefuroxime, so the mic of these drugs has changed. The enters, bacterioides fragilis and proteus indol have reduced sensitivity to cefuroxime.
The strains of Clostridioides Difficile, Pseudomonas SP., Campylobacter sp., Acinetobacter calecoaceticus, Legionella sp. are not sensitive to cefuroxime.
Staphylococcus aureus strains, Staphylococcus Epidermidis resistant methicillin resistant to Cefuroxime resistance. Listeria monocytogenes and most of the Enterococcus strains are also resistant to cefuroxime.
Tình hình kháng cefuroxime hiện nay của các vi khuẩn gây bệnh đã tăng như sau: shigella flexneri 11% (1998), proteus mirabilis 28,6% (1997), citrobacter freundii 46,7% (1991), s.viridans 31% (1996), s.aureus 33% (1998), e.coli 33,5% (1998), klebsiella sp. 57% (1997), Enterobacter 59% (1998).
Recent studies on drug resistance in Vietnam show that the haemophilus influenzae strains are isolated in healthy children, Cefuroxime resistance at a high rate. The level of Cefuroxime resistance of all H.influenzae strains in healthy children is 27% according to ASTS's notice No. 4 (1999). This serious situation shows that it is necessary to limit the use of broad -spectrum antibiotics and only for patients with serious infections.
pharmacokinetic
absorption
After drinking, Cefuroxime Acetil is absorbed through the digestive tract and is quickly hydrolyzed in the intestinal mucosa and in the blood to release Cefuroxime into the circulatory system. The drug is best absorbed when taken at meals. The maximum concentration in the serum is about 2-3 hours after taking the drug during a meal. Using Probenecid will also increase the area below the curve to 50%. Cefuroxime concentration is reduced during the appraisal.
Distribution
The level of cohesion with protein is different (33 - 50%) depending on the method used.
Metabolism - Elimination
Cefuroxime is not metabolized and eliminated by the filtration process in the glomerular and renal tubules. Serum disposal time is 1 - 1.5 hours.
Before taking Negacef 500 pymepharco treatment for respiratory infections, ENT infections (2 blisters x 5 tablets)
How to use
Oral drugs.
Dosage
Adults
Bronchitis: 500mg x 2 times/day.
In most cases of infection in another position: 250mg x 2 times/day.
Infolented urinary tract infections: 125mg x 2 times/day.
Inhabited gonorrhea: The only 1g dose is recommended.
Children
Normal dose: 125mg x 2 times/day or 10mg/kg x 2 times/day. The maximum dose of 250mg/day.
Children ≥ 2 years old with otitis media: can be used 250mg 2 times/day or 15mg/kg x 2 times/day. Maximum dose 500mg/day.
For cases of less than 500mg/time, Cefuroxime 250mg or Cefuroxime pills can be used to divide the dose accordingly.
Patients with renal failure
recommended dose for patients with renal failure based on creatinine clearance:
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? Protect the respiratory tract of the patient, support ventilation and infusion. If the seizure is developed, stop using the drug immediately, anti -convulsions may be used if clinical indications.
Hemorrhage can eliminate blood from the blood but most of the treatment is supporting or resolving symptoms.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Negacef 500 , you may experience unwanted effects (ADR).
Common, ADR> 1/100
Uncommon, 1/1000 Eosin leukemia, leukopenia, neutropenia. Rare, 1/10000 Anemia hematoma. polymorphic erythematosus, Stevens - Johnson syndrome, poisoned epidermal necrosis. Poisoning with temporary increase in urea, blood creatinine, interstitial nephritis. Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Negacef 500 contraindicated in the following cases:
Do not use for patients with hypersensitivity to cefuroxime or antibiotics of cephalosporin and patients hypersensitivity to other ingredients of the drug.
Precautions when used
Patients with a history of allergies to cephalosporin, penicillin or other drugs.
Should check the kidney function when treated with cefuroxime, especially in serious patients who are taking the maximum dose. Be careful when giving patients simultaneously with strong diuretic drugs because it can have adverse effects on kidney function.
Patients with renal failure
using long -term cefuroxime can make an irregular strain overdoing. Need to follow the patient carefully. If serious superinfection during treatment, medication must be stopped.
Palm colitis can occur when using broad -spectrum antibiotics. Therefore, it is necessary to pay attention to this disease and treat Metronidazole for patients with severe diarrhea due to antibiotic use. Should be cautious when using broad antibiotics for people with gastrointestinal diseases, especially colitis.
Records that kidney toxicity has simultaneously used aminoglycoside and cephalosporin antibiotics.
The ability to drive and operate machinery
No research on this object. However, the drug can cause dizziness, need to be cautious when used.
Pregnancy
only research on animals, human research is very limited. Therefore, only use drugs when really necessary.
Breastfeeding period
cefuroxime excreted through breast milk, so it is necessary to be cautious when using cefuroxime in nursing mothers.
Drug interaction
Reducing effects
Ranitidine with sodium bicarbonate reduces the bioavailability of cefuroxime acetyl. Cefuroxim acetyl should be used at least 2 hours after antacids or H2 blockers, because these drugs can increase the gastric pH.
Increase effect
High doses Probenecid reduces Cefuroxime clearance in the kidneys, making Cefuroxime concentration in plasma higher and longer.
Increased toxicity: Aminoglycoside increases the likelihood of kidney poisoning.
Storage
In a dry, cool place (under 30ºC), avoid light.
