Pharmacokology

Esomeprazol is the isomorphic form S-Omeprazol and reduces the secretion of stomach acid with a specialized mechanism at a special destination. The drug is a specific inhibitor of acid pumping in the wall of the wall. Both types of isomers R-and S- of Omeprazol have similar pharmacological effects.

Esomeprazol is a weak, concentrated and transformed basic into a high acidic environment in the sub-control tube of the cell's secretion, where H+K+K+-atpase inhibitors (Acid pump) and inhibit both basic fluid and stimulation secretion.

pharmacokinetics

absorption:

Esomeprazol is easily destroyed in acid environments and is taken in the form of granules in the intestine. Esomeprazol is quickly absorbed with peak plasma concentrations reaching about 1-2 hours after drinking

Distribution:

Expected distribution integral in a healthy state is about 0.22 l/kg of body weight. Esomeprazol binds 97% to plasma protein.

Metabolism:

  • Esomeprazol is completely transformed through the cytochrom P450 (CYP). Other, CYP3A4, forming Esomeprazol Sulfon, the main metabolite in plasma.

About 80% of esomeprazol oral doses are excreted in urine in the form of metabolites, the rest through feces. Less than 1% of constant drugs found in urine.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Nexium 10mg drug is contraindicated in the following cases:

    History of hypersensitivity to esomeprazol, benzimidazol group or other ingredients in the formula;

Do not use esomeprazol simultaneously with nelfinavir.

    Caution when using

when there is any alarm symptom (such as significant weight loss, recurrent vomiting, difficulty swallowing, vomiting blood or black defecation) and when suspected or gastric ulcer should eliminate malignant diseases because Nexium 10mg treatment can reduce symptoms and delay diagnostic.

Patients with long -term treatment (especially those who have been treated for more than 1 year) should be monitored regularly. Long -term treatment is indicated for adults and adolescents (12 years or older).

Patients treated according to the regime when necessary should contact the doctor if there are symptoms that change in characteristics. Treatment according to the regime when necessary has not been studied in children and therefore is not recommended in this group of patients.

This drug contains sucrose and glucose. Patients with rare genetic problems such as fructose intolerance, glucose-galactose malabsorption or deficiency of sucrase-isomaltase should not be used.

Increased risk of gastrointestinal infections such as Salmonella and Campylobacter.

It is not recommended to simultaneously use Esomeprazol with Atazanavir (see interactive section). If there is no alternative, clinically closely monitored when increasing the dose of Atazanavir to 400 mg combined with 100 mg of ritonavir; Do not use more than 20 mg Esomeprazol.

Esomeprazol, as well as all other antacids, can reduce the absorption of vitamin B12 (cyanocobalamin) due to reduced gastric acid. This should be considered in patients with vitamin B12 deficiency or have the risk of reducing vitamin B12 absorption in long -term treatment.

Esomeprazol is CYP2C19 inhibitor. When starting or ending treatment with Esomeprazol, it is necessary to consider the risk of drug interaction with metabolic drugs through CYP2C19 (Clopidogrel) .. To be cautious, should not be used simultaneously Esomeprazol and Clopidogrel.

The risk of severe maggesi blood in patients treated with proton pump inhibitors (PPI) such as Esomeprazol for at least 3 months, and in most cases of using PPI for 1 year.

Serious symptoms of blood -blooded lower blood such as fatigue, muscle spasms, delirium, convulsions, dizziness and ventricular arrhyths may occur, but may be silently and not concerned. The majority of patients who have a magnesi hypoglycemia will progress well after the supplementation of Magnesi and stop ppi.

For patients expected to be treated for a long time with PPI or patients who have to use PPI along with digoxin or other drugs that can cause blood magam (such as diuretics), medical experts should consider checking the magnesi levels in the blood before starting treatment with PPI and periodic tests during treatment. Proton pump inhibitors, especially when used in high doses (more than 1 time) and for a long time (> 1 year), may increase the risk of hip fractures, wrist bones and spine, especially in elderly patients or when there is a presence of known risk factors.

Patients with osteoporosis should be cared for under the current clinical instructions and should be used enough vitamin D and calcium.

Interacts with tests: an increase in Chromographin A (CGA) level can interfere with the detection of endocrine nerve tumors. In order to avoid this intervention, it is advisable to stop treating with Esomeprazol for at least 5 days before quantifying CGA (see Pharmacoketic Resource).

The ability to drive and operate machinery

    does not record any impact.

    Pregnancy

There is no enough clinical data on the use of Nexium 10mg in pregnant women. Should be cautious when prescribing pregnant women.

    The period of breastfeeding

There are no studies on breastfeeding women. Therefore, Nexium 10mg should not be used during breastfeeding.

    Interactive drug

Studies on interactions are only done in adults.

Esomeprazol's impact on pharmacokinetics of other drugs

    For drug absorption drugs dependent

The reduction of stomach acid when treated with ecomeprazol and other ppi may increase or decrease the absorption of drugs where the absorption depends on the stomach pH. Like other acid -reducing drugs, the absorption of drugs such as ketoconazole, otraconazole and erlotinib may decrease and the absorption of digoxin may increase during treatment with ecomeprazol. There are rare reports on Digoxin toxicity. However, it is necessary to be cautious when using high doses of Esomeprazol in elderly patients. Need to increase monitoring treatment with digoxin.

Omeprazole interacts with some protease inhibitors. It is unclear clinical importance and the mechanism of impact of the recorded interactions. Increasing stomach pH during treatment with omeprazole can lead to changes in the absorption of protease inhibitors. Another possible interactive mechanism occurs through inhibition of CYP2C19. For Atazanavir and Nelfinavir, the serum concentration has been recorded when used with omeprazol, so it is not recommended to simultaneously use these drugs.

    Metabolic drugs through CYP2C19

Esomeprazol inhibits CYP2C19, the main enzyme metabolizes Esomeprazol. Therefore, when Esomeprazol is used with metabolic drugs with CYP2C19 such as Diazepam, Citalopram, Imipramin, Clomipramin, Phenytoin ..., the concentration of these drugs in plasma can increase and decrease the dose. This should be paid special attention when prescribing Esomeprazol according to the treatment regime when necessary.

Do not use simultaneously Esomeprazol and Clopidogrel.

    Unknown mechanism

There have been reports on increased tacrolimus concentration in serum when used simultaneously to Esomeprazol.

Used with methotrexate increases methotrexate levels in some patients. Patients with high doses of methotrexate should consider stopping esomeprazole.

The effect of other drugs on pharmacokinetics of Esomeprazol

Esomeprazol is metabolized by CYP2C19 and CYP3A4. When using Esomeprazol simultaneously with a CYP3A4 inhibitor, Clarithromycin (500 mg, 2 times/day) increases the concentration of esomeprazol in serum. No need to adjust the dose of Esomeprazol regularly in these situations. However, the dose adjustment should be noted in patients with severe hepatic or long -term treatment. Long -term treatment is indicated in adults and adolescents (aged 12 years and older, see the specified section).

CYP2C19 or CYP3A4 induction drugs or both (such as rifampicin and st. John’s grass) can cause reduced serum ecomeprazol levels due to Esomeprazol metabolism.
