Uses
indications
NIrDICIN 500mg drug is indicated for use in cases of infections caused by bacteria sensitive to levofloxacin.
Pharmacology
levofloxacin is a synthetic antibacterial antibacterial antibacterial antibacterial of the fluoroquinolon group, is the L-Ioseom isomer of Ofloxacin. Levofloxacin has bactericidal effects due to inhibiting Topoisomerase II enzyme (and-gyrase) and/or Topoisomerase IV are essential enzymes of bacteria involved in the catalyst of copying, transcription and DNA repair of bacteria.
levofloxacin has high bactericidal in vitro and cross -resistant between levofloxacin and other fluoroquinolones. Due to the mechanism of action, there is often no cross -resistance between levofloxacin and other antibacterial families.
Effect spectrum
In vitro sensitive bacteria and clinical infections
levofloxacin is absorbed quickly and completely; The peak concentration is achieved in 1 hour. Absolute bioavailability of about 100%. Eat less affects the absorption of levofloxaxin.
Distribution
about 30-40% levofloxacin binds to plasma proteins. Stable state is achieved within 3 days. The drug penetrates well into bone tissue, burns, and lung tissue but poorly in the cerebrospinal fluid.
Metabolism
levofloxacin is metabolized at a very low level, two metabolites are desmethyl-levofloxacin and levofloxacin n-oxide account for Elimination
levofloxacin is eliminated from relatively slow plasma (t1/2: 6 - 8 hours). Excretion mainly through the kidney (> 85% dose). There is no big difference in pharmacokinetic parameters after oral or after intravenous injection, suggesting that oral and intravenous sugar can be used instead of each other.

How to use
oral medication. Nirdicin needs to be swallowed, not crushed, with a sufficient amount of water. The drug can be broken by the lines to break the dose. The drug can be taken during meals or between two meals.
NIrDICIN should be taken at least 2 hours before using iron, antacid and sucralfat preparations because it can reduce the absorption of the drug.
Dosage
Dosage used depending on the type and weight of infection and sensitivity of pathogens.

Because most levofloxacin is excreted in the urine in a constant form, it is not necessary to adjust the dose in case of liver failure.
Older people
No dose adjustment in elderly patients. However, special attention should be paid to the kidney function in the elderly patients, and only need to adjust the dose appropriately.
Children
levofloxacin is contraindicated for children under 18 years old.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Expression
According to animal toxicity studies, the most important signs can be seen after an acute levofloxacin overdose are symptoms of the central nervous system such as confusion, tinnitus, perceived disorders and seizures, increased QT interval and symptoms of digestive disorders such as vomiting and mucosal corrosion.
Handling
If overdose occurs, symptomatic treatment should be treated. Monitor electrocardiograms because the QT interval may last. Anti -acid drugs are used to protect the stomach lining. Hematoparoa, including peritoneal and Capd, does not eliminate levofloxacin from the body. No specific detoxification.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not use double the prescribed dose.
Side Effects
When using NIrDICIN 500 mg, you may experience unwanted effects (ADR).
CommonLess
Urinary, genital: Vaginitis, genital candidiasis. Rare musculoskeletal: joint pain, muscle weakness, muscle pain, bone marrow, tendonitis. Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
NIRDICIN 500 mg is contraindicated in the following cases:
Be cautious when using
tendonitis and tendon rupture: tendonitis can rarely occur, sometimes can lead to tendons, especially heel tendons (Achilles tendons). Older patients are more susceptible to tendonitis. The risk of tendons can be increased when used with corticosteroids. If suspected of tendonitis, it is necessary to immediately stop treating Levofloxacin and must let the tendon hurt to rest.
diarrhea, especially severe, persistent diarrhea, and/or bloody, during and after treatment of levofloxacin, may be symptoms of fake colitis caused by Clostridium difficile. If a fake colitis is suspected, levofloxacin should be suspected and patients need support for support (such as taking Metronidazol or Vancomycin).
Be careful when using drugs for patients with central neurological diseases such as epilepsy, brain vasoconstriction, ... because it may increase the risk of convulsions. Patients with an active enzyme active glucose - 6 - potential dehydrogenase phosphate or really easy to have blood -soluble reactions when treated with quinolon antibacterial drugs. Consider this ability when using levofloxacin.
In patients with renal failure, the dose adjusted levofloxacin because levofloxacin is excreted mainly through the kidneys.
Hypersensitivity reactions with many different clinical manifestations, even anaphylaxis shock when using levofloxacin, including quinolon. It is necessary to stop the drug if there are signs of sensitive reactions and apply appropriate management measures.
As with quinolons, hypoglycemia can occur especially in oral diabetes at the same time with hypoglycemic drugs (such as glibenclamide) or with insulin. Therefore, blood sugar is needed when using levofloxacin.
Although sensitive to light is very rare with levofloxacin, patients should not expose unnecessarily with the sunlight or artificial sunlight. Be cautious when taking patients with a long -term QT patient: patients with hypokalemia, patients are using anti -arrhythmia groups of IA or group III, and patients with acute myocardial ischemia.
Serious reactions are likely to not recover and cause disabilities, including tendonitis, tendon, peripheral neuropathy and adverse effects on the central nervous system. Fluoroquinolon antibiotics are associated with serious harmful reactions that can cause disabilities and non -recovery on different organs of the body. These reactions may appear simultaneously on the same patient. Harmful reactions are often recorded including tendonitis, tendon, joint pain, muscle pain, peripheral neuropathy and adverse effects on the central nervous system (hallucinations, anxiety, depression, insomnia, severe headaches and confusion). These reactions may occur within hours to a few weeks after using the drug. Patients at any age or no risk factors that exist before may have these harmful reactions.
Stop using the drug as soon as there are signs or first symptoms of any serious harmful reactions. In addition, avoid using Fluoroquinolon antibiotics for patients who have experienced serious reactions related to fluoroquinolon.
The ability to drive and operate machinery
levofloxacin can cause unwanted effects such as tinnitus, dizziness, drowsiness and visual disorders, can become a risk in cases such as driving or operating machinery.
During pregnancy and lactation
Do not use levofloxacin for pregnant women and nursing women.
Drug interaction
Two hours before or after taking Levofloxacin, do not take preparations that contain two chemotherapy cations or triple chemotherapy like iron or antacids containing magnesi or aluminum because it can reduce absorption.
Levofloxacin's bioavailability reduces the meaning when used with sucralfat, so only drink sucralfat for 2 hours after taking Levofloxacin.
In a clinical study, there is no pharmacokinetic interaction of levofloxacin with theophylin. However, the strategy of seizures in the brain can be significantly reduced when shared with quinolon with theophylin, fenbufen or similar nonsteroidal anti -inflammatory drugs or other drugs that lower the seizure threshold.
Be cautious when using levofloxacin with drugs that affect the excretion of the kidneys such as probenecid and cimetidine, especially in patients with renal impairment.
cyclosporin, digoxin: interactions are not clinically significant, so there is no need to adjust these drugs when used simultaneously with levofloxacin.
Increased blood clotting time (PT/INR) and/or bleeding, may be seriously seen in patients treated with levofloxacin with vitamin K antagonistic drugs (such as warfarin). Therefore, it is necessary to monitor blood clotting tests in patients treated with antagonistic drugs.
