Uses

indications

Klacid drugs 125mg/5ml are indicated in the following cases:

  • Treatment of upper respiratory tract infections (such as streptococcal sore throat). Mycobacterium or spread by M.Avium or M.intracellulare; Local infections caused by M. Chaelonae, M.Fortuilum, or M.Kansaii. Clarithromycin often has a bactericidal effect on many gram -positive bacteria and some Gram -negative bacteria. The drug can have a high doses of bactericidal or very sensitive strains. Clarithromycin inhibits the synthesis of protein in sensitive bacteria by attaching the subunit of 50S ribosom, thus preventing the movement of aminocyl transfer-rna and inhibiting polypeptide synthesis. The acting position of Clarithromycin is also the effect of erythromycin, clindamycin, lincycin and chloramphenicol.

In vitro, Clarithromycin have similar or stronger effect than erythromycin for bacteria sensitive to erythromycin and also have anti -micro -microbiological activity (such as the non -typical Mycobacteria, toxoplasma).

    Pharmacokinetics

    absorption

Clarithromycin after oral is absorbed quickly through the gastrointestinal tract and subject to the first high level of metabolism, absolute bioavailability of Clarithromycin about 55%. The absorption level is almost not affected by food. The peak concentration of serum reaches about 2-3 hours after drinking. Stable concentration is reached in about 3-4 days; The peak concentration of clarithromycin and the main metabolites 14-hydroxy clarithromycin in the order of about 1 and 0.6 micrograms/ ml, after taking 250 mg tablets, every 12 hours. The same dose in the form of an emotional epidemic creates stable plasma concentrations in about 2 micrograms/ml for clarithromycin and about 0.7 micrograms/ml for 14-hydroxyclarithromycin.

    Distribution

about 42 - 72% of the drug concentration is attached to plasma proteins.

    Metabolism

Clarithromycin and main metabolites 14-Hydroxy Clarithromycin are widely distributed and the concentration in tissue exceeds the serum concentration due to part of the drug is collected into the cell.

    Elimination

Metabolic drugs are high in the liver and release stool through biliary tract (5 - 10% of the mother medicine). A significant part is discharged through urine.

    Warnings

Before using KLacid 125mg, you need to read the user manual carefully and refer to the information below.

    Contraindicated

Klacid drugs 125mg/5ml contraindicated in the following cases:

Hypersensitivity to antibiotics family macrolid or any excipients of the drug.

Concentrated with the following medications: Astemizole, Cisapride, Pimozide, Terfenadine because it can cause QT interval and arrhythmia including ventricular tachycardia, ventricular vibration and torsion.

Use clarithromycin with ergotamine or dihydroergotamine because it can cause fungus poisoning.

Use Clarithromycin and HMG-CAA Reductase inhibitors (Statin drugs) that are metabolized largely by CYP3A4 (lovastatin or simvastatin), due to increased risk of muscle diseases, including muscle prize.

Patients with a history of extended QT interval on ECG or ventricular arrhythmia, including vertices.

Using Clarithromycin with Colchicin in patients with liver failure, renal failure is using p-glycoprotein inhibitors or CYP3A4 inhibitors.

    Be cautious when using

If the clarithromycin chaos for children is used for children after puberty, physicians should consider beneficial when they have suspected or identify pregnant children.

Using Clarithromycin for a long time, as with other antibiotics, can cause excessive bacteria that are not sensitive and fungus. If superinfection occurs, appropriate treatment should be taken.

Be careful when used for patients with severe renal impairment.

There have been reports on liver dysfunction, including liver enzyme, liver cell inflammation and/or cholestatic hepatitis, with or without jaundice when using Clarithromycin. Liver dysfunction may be serious and often recovered. Stop using clarithromycin if signs and symptoms of hepatitis, such as anorexia, jaundice, dark urine, itching, or abdominal pain.

Use clarithromycin to treat H.pylori infection can produce anti -drug bacteria.

Fake colitis and diarrhea related to Clostridium difficile (CDAD) have been reported when used with Clarithromycin, and the degree of mild diarrhea to death.

Symptoms of myasthenia gravis are more serious in patients treated with clarithromycin.

Be careful when using clarithromycin for patients with liver function damage and patients with mild to severe kidney failure.

There has been a report on colchicine poisoning when used simultaneously with clarithromycin, especially in the elderly, some occur in patients with renal impairment. Died in some cases.

Be careful when using Clarithromycin along with triazolobenzodiazepine drugs, like Triazolam and Midazolam.

Due to the risk of extension of QT, Clarithromycin should be used cautiously in patients with pathological conditions related to the trend of increasing the extension of QT and twisting.

Due to the increasing streptococcus pneumonia's macrolid drug resistance, the implementation of antibiotics is important when prescribing Clarithromycin for patients with pneumonia with community. Clarithromycin should be used in combination with other appropriate antibiotics in the treatment of pneumonia.

Currently, macrolide drugs are only considered to be indicated for the treatment of skin and soft tissue infections, such as bacterial infections caused by corynebacterium minutissimum (recurrent chronic dermatitis), acne, skin infections that cause high -class fever and cases of penicillin. In the case of acute and severe hypersensitivity reactions, such as Stevens-Jonhnson syndrome, anaphylaxis, poisoned epidermal necrosis, dress and Henoch-Schonlein bleeding committee, immediately stop using Clarithromycin and urgently treat appropriate treatment.

Be cautious when using Clarithromycin along with drugs irritating the enzyme system CYP3A4.

Note the cross -resistance between Clarithromycin and other macrolids as well as with Lincomycin and Clindamycin.

Simultaneous use Clarithromycin and oral hypoglycemic drugs and/or insulin may significantly reduce blood sugar. Clarithromycin inhibits the CYP3A4 enzyme, which can cause hypoglycemia when used in combination with blood sugar drugs such as Nateganide, Pioglitazone, Repaglinide and Rosiglitazone. Need strict control of blood sugar.

Simultaneous use Clarithromycin and warfarin are at risk of serious bleeding, increasing the INR index and prothrombin time.

Do not use clarithromycin simultaneously with lovastatin or simvastatin. In cases where clarithromycin must be used and statin drugs, the lowest dose recommendations of statin drugs, consider using statin drugs regardless of CYP3A4 metabolism.

The ability to drive and operate machinery

There has been no report on the impact of Klacid 125mg while driving and operating machinery. It should be recommended that patients with the risk of dizziness, confusion, and disorientation may occur when using the drug.

    Pregnancy

Clarithromycin's safety during pregnancy has not been set. Therefore, Clarithromycin should not be used during pregnancy unless the benefits are greater than the risk.

    Lactation period

The safety of clarithromycin during breastfeeding has not been set. Clarithromycin is determined in breast milk.

    Drug interaction

Increased concentration of drugs after use simultaneously with clarithromycin: Mostly metabolic drugs through CYP3A4 (Alprazolam, Astemizole, Carbamazepine, Cilostazol, Cisapride, Cyclosporine, Disopyramide, Alkaloids Methylprednisolone, Midazolam, Omeprazole, oral anticoagulant, Pimozide, Quinidine, Rifabutin, Sildenafil, Simvastatin, Taccrolimus, Terfenadine, Triazolam and Vinblastine), Tadalafil, Varianenafil, Theophyllline, CarbamerePinem, Carbamepine, Tolterodine, Colchicine, Digoxin, Valproate, Verapamil, ...

The drug irritants of the CYP3A4 enzyme system (Rifampicin, Phenytoin, Carbamazepine, Phenobarbital, St John ‘S World) increase the metabolism of clarithromycin. Use Clarithromycin and rifabutin at the same time increase rifabutin levels and reduce serum clarithromycin levels, along with increased risk of grape veins.

Simultaneously use Clarithromycin with ergotamine/dihydroergotamine causing acute toxicity of chicken mushrooms.

The following drugs affect the circulatory concentration of Clarithromycin: Efavirenz, Nevirapine, Rifampicin, Rifabutin, Rifapentin, Eravirine, Fluconazole, Ritonavir. Using Clarithromycin with the following drugs leads to 2 -way drug interaction: Atazanavir, Itraconazole, Saquinavir.
