Uses

indications

Omefort drugs are indicated in the following cases:

  • Indigenous indigestion due to increased acid secretion.
  • Treatment of gastroesophageal reflux disease

Omeprazol is a benzimidazole that has attached groups, structured and effectively effectively as Pantoprazol, Lansoprazol, Esomeprazol. Omeprazol is an inhibitor of the acid secretion of the stomach due to inhibiting the enzyme system hydrogen/potassium adenosin triphosphatase (H+/K+ - ATPASE) also known as the proton pump in the wall of the stomach wall. Take a single dose of 20 mg of omeprazole to create a strong and effective gastric secretion inhibitor. The maximum effect is achieved after 4 days of treatment. In patients with duodenal ulcer, it is possible to maintain a 80% reduction in gastric acid in 24 hours.

Omeprazol can restrain Helicobacter pylori bacteria in patients with duodenal ulcer and/or reflux esophagitis infected with this bacteria. Combining omeprazol with some antibacterial drugs (for example, Clarithromycin, amoxicillin) can be exposed to H. pylori accompanied by an ulcer and remission for a long time.

    Dynamic pharmacokinetics

omeprazol is destroyed in an acidic environment. The drug is prepared in the form of particles dissolved in the intestine and then closed into the capsule or stamping the tablet to avoid the destruction of the acid pH of the stomach. Omeprazole is usually absorbed in the small intestine after drinking for 3 to 6 hours. Birth is about 60%. The drug attaches about 95% to plasma proteins. Although Omeprazol has half a lifetime in a short plasma, the drug has a long time (due to the prolonged attachment of the drug to H+/K+ ATPase). So that can only be used once a day.

After absorption, omeprazol is almost completely metabolized in the liver, mainly thanks to the cytochrom P450 isoenzyme isoenzyme to become hydroxy omeprazol, and a small part transformed through CYP3A4 to form omeprazol sulfon. These metabolites are not active and are excreted mainly through urine and partially in the feces.

    How to use

Used: Omeprazol must drink when hungry (before eating 1 hour). Must swallow the intact pill is not opened, chewed or grinded.

For children under 6 years old, fear of difficulty swallowing, can open Omeprazol follicles and mix with a slightly acidic food (pH

    Dosage

To reduce indigestion related to acid, omeprazol is taken daily at a dose of 10 or 20 mg from 2 to 4 weeks.

    Treatment of gastroesophageal reflux disease

The common dose is 20 mg of omeprazol oral once a day for 4 weeks, then another 4 to 8 weeks if not healed. In case of untreated esophagitis, daily dose may be 40 mg. Treatment for maintenance of esophagitis after healing is 20 mg once a day, and with acid reflux is 10 mg daily.

Treatment of stomach ulcers - duodenum

Take a dose of 20 mg or 40 mg daily in severe cases. With duodenal ulcer, treatment continues for 4 weeks, and with stomach ulcers is 8 weeks.

To eradicate Helicobacter pylori in gastric and duodenal ulcer disease, there is a combination of omeprazol with antibacterial drugs in the 3 or 4 regimen. Three drug therapy regimens include Omeprazol 20 mg, taken twice a day or 40 mg once a day, combined with amoxicillin 1 g and Clarithromycin 500 mg, both oral medications twice a day. When the regimen 3 drugs have no results, add bismuth products (4 drugs). These ways drink for 2 weeks. Particularly Omeprazol can continue another 4-8 weeks.

ulcerative treatment related to nonsteroidal anti -inflammatory drugs

can use 20 mg of omeprazol oral daily; The daily dose of 20 mg is also used to prevent patients with a history of stomach -duodenal lesions and still have to continue treating nonsteroidal anti -inflammatory drugs.

Patients with Zollinger - Ellison syndrome

The starting dose is 60 mg of omeprazol orally once a day, then adjust it if necessary. The majority of patients are effectively controlled by doses of about 20 to 120 mg daily, but the doses up to 120 mg, three times a day have also been used. Daily doses of over 80 mg must be divided (usually 2 times).

Omeprazol is also used to prevent acid choking during anesthesia, at a dose of 40 mg at night before surgery and a dose of 40 mg more at about 2-6 hours before surgery.

    Patients with liver failure

Omeprazol's dose needs to be reduced.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? In literature, there are notices about 2 cases of overdose Omeprazol.

Clinical manifestations are mainly drowsiness, headache (perhaps due to metabolic) and fast heartbeat. Both patients recovered, had no events nor had any special treatment.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using omefort drugs, you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • Headache, drowsiness, dizziness.
  • Dowers, nausea, vomiting, abdominal pain, constipation, abdominal distention.
  • Uncommon, 1/1000

  • Insomnia, sensory disorders, fatigue.
  • urticaria, itching. Floating.
  • Transient transaminase increase.
  • Rare, ADR

  • Sweating, peripheral edema, hypersensitivity including angels, anaphylaxis.
  • leukopenia, thrombocytopenia, reducing all blood cell lines, granulocytes, autoimmune hemolytic anemia.

Confusion has recovered, agitated, depressed, hallucinating in the elderly and especially in serious patients, hearing disorders.

    Big breasts in men.

    Gastritis, candidiasis, dry mouth.

Hepatitis with jaundice or no jaundice, cerebral disease - liver in liver failure.

    Bronchospasm.

  • joint pain, muscle pain.
  • Interstitial nephritis.

Proton pump inhibitors can increase the risk of gastrointestinal tract infections due to the effect of inhibiting gastric acid secretion.

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Omefort drugs contraindicated in the following cases:

  • Patients with hypersensitivity to the drug, Esomeprazol, or other benzimidazol derivatives (such as Lansoprazol, Pantoprazol, Rabeprazol) or any ingredients in the formula.
  • Be cautious when used

Before giving stomach ulcer using omeprazol, the possibility of malignant tumor (the drug can cover the symptoms, so it is late to diagnose).

The ability to drive and operate machinery

omeprazole hardly affects the ability to drive or operate machinery. There may be unwanted effects such as dizziness and visual disorders. At that time, patients should not drive or operate machinery.

    Pregnancy

on animals does not see omeprazol is likely to cause deformities and toxic to the fetus. Clinically, so far has not seen any toxic effects on the fetus. However, the tracking time is not enough to eliminate all risks. Therefore, the use of omeprazol during pregnancy is only considered when really necessary.

    Demonstrate period

Because the drug is distributed in breast milk, should consider stopping the drug or stop breastfeeding.

    Drug interaction

omeprazole does not have clinically important interactions when used with food, alcohol, amoxycilin, bacampicilin, caffeine, lidocaine, quinidine or theophylin. The drug is also not affected by simultaneous use of Maalox or Metoclopramid.

omeprazol may increase the concentration of ciclosporin in the blood.

omeprazol increases the effect of antibiotic eradication H. pylori.

Omeprazole inhibits the metabolic of drugs metabolized by the enzyme system in the liver's cytocrom P450 and can increase the concentration of diazepam, phenytoin and warfarin in the blood. The metabolism of diazepam makes the effect of the drug longer. With a dose of 40 mg/day Omeprazole inhibits phenytoin metabolism and increases the concentration of phenytoin in the blood, but the dose of Omeprazol 20 mg/day has much weaker interaction. Omeprazol inhibits the metabolism of warfarin, but rarely changes bleeding time.

    omeprazol increases Dicoumarol's anticoagulant effect.

omeprazol reduces nifedipine metabolism at least 20% and may increase the effect of nifedipine.

Clarithromycin inhibits omeprazol metabolism and makes omeprazol levels double.

    Storage
