Uses
indications
Omez drug are indicated in the following cases:
Mechanism of impact
omeprazol is absorbed in the intestine and to the stomach wall cells. In the acidic environment of the stomach wall cells become concentrated and activated (protonization) into a sulphenamid activity. The restraint of the dose dependence. Omeprazol's daily oral dose is 20mg and higher proves to control acid stable and effective.
Dynamic pharmacokinetics
sensitive omeprazol and decomposition in the acidic environment of the stomach. Therefore, omeprazol must make nuggets in the intestine and close.
Food does not affect the bioavailability of the drug. Birth has been reported at about 40% at a dose of 20 to 40mg. The time to reach the peak plasma concentration is from 0.5 to 3.5 hours. 95% Omerazol binds to plasma proteins. Omeprazol capsules are completely and quickly metabolized, because there is no initial drug in the feces and urine. The main enzyme participating in the metabolism of omeprazol is a diverse form expressed in the form of Cytochrom P450 (CYP), S-Mephenytoin Hydroxylase isomers, CYP2C19. Half -life elimination in plasma does not change significantly while repeating.
Despite the fast elimination (the average half -life is 0.5 to 1 hour), the anti -excretory effect of omeprazol is prolonged due to its strong affinity for acidic pump in the gastric wall cells.
Before taking Omez 20mg hard capsules Dr. Reddy's treat and prevent recurrence of duodenal ulcer (2 blisters x 10 tablets)
How to use
Take oral use.
Dosage
for adults
Take for short -term treatment of duodenal ulcer: 20mg/day/4 weeks. Sometimes it is necessary to treat additional 4 more weeks.
Take for short -term treatment of stomach ulcers: 40mg/day/4 weeks.
Take for short -term treatment of reflux esophagitis accompanied by symptoms of gastroesophageal reflux disease (GERD) and no esophageal bubble: 20mg/day/4 weeks. For patients with esophagitis accompanied by GERD syndrome: 20mg/day/4 - 8 weeks.
The starting dose to control Zollinger - Ellison syndrome: 60mg. The dose may vary depending on the patient, may be up to 120mg/day. May be 80mg/day divided into 2 times.
For children
Safety and effectiveness of Omeprazole capsules are not set for children.
For older patients
No dose adjustment.
For patients with liver failure
may need to reduce the dose for patients with liver function, because Omeprazole metabolizes strongly in the liver and the elimination rate lasts longer than normal patients.
For patients with renal failure
No dose adjustment.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
There is no specific anti -toxic substance to overdose Omeprazol. Omeprazol is closely attached to protein so it is not ready to separate. Handling overdose due to drugs is usually symptomatic treatment and supportive treatment. Rarely reports on Omeprazol overdose from 320 to 900mg. The usual manifestations in this high doses are confused, dizzy, blurred vision, tachycardia, nausea, sweating, blushing, headache, dry mouth.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Omez drug , you may experience unwanted effects (ADR).
omeprazol is well tolerated. The unwanted effects occur ≥ 1%without assessing the causal relationship may be: abdominal pain (5.2%), weakness (1.3%), constipation (1.5%), diarrhea (3.7%), flatulence (2.7%), nausea (4.0%), vomiting (1.9%), heartburn (1.9%), headache (2.9%).
Other unwanted effects
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
Omez drug contraindicated in the following cases:
Patients with hypersensitivity to omeprazol or other excipients in the formula.
Pregnant and lactating women.
Be cautious when using
eliminate malignant tumors before starting treatment with Omeprazol capsules when diagnosing stomach ulcers, as omeprazol mitigates symptoms that make the diagnosis of the disease late. There has been a notice of muscle atrophy in patients with long -term stomach treatment with Omeprazol capsules.
The ability to drive and operate machinery
has reported on unwanted effects such as drowsiness and dizziness. Patients who experience these unwanted effects, must be cautious when driving and operating machinery.
Pregnancy
Do not use omeprazol for pregnant women because of safety and effectiveness not established.
Demonstrate period
Do not use omeprazol for women who are breastfeeding because of safety and effectiveness.
Drug interaction
The potential of metabolic interaction between omeprazol and other drugs is very limited. However, because Omeprazol inhibits the metabolism of microscopic drugs in the liver (the cytochrom P450 enzyme system), the elimination of other drugs is reduced because they have to metabolize in the liver through the cytochrom P450 enzyme system or those drugs are strongly extracted through the liver and dispute the enzyme system with omeprazol. That effect leads to slow elimination and increased blood concentration of diazepam, phenytoin and anticoagulants such as warfarin, need to monitor blood levels or blood coagulation time (prothrombin time) to dose dose, as the dose adjustment may be needed during treatment with omeprazol.
Omeprazol increases the pH of the stomach, so it is likely to affect the bioavailability of any drug where the absorption depends on the pH and omeprazol can also prevent the decomposition of the drug that is not durable with acidic. Therefore, when used simultaneously otraconazole, ketoconazole with omeprazole can lead to the absorption of these drugs decreased.
Omeprazol does not affect any related isomers of CYP (Cytochrom P450 system) such as no metabolic interaction with substrates for CYP1A2 (Cafein, Phenacetin, Theophylin), CYP2C9 (S-Warfarin, Piroxicam, Diclofenac and Naproxen) (Metoprolol, Propanolol), CYP2E1 (Ethanol), and CYP3A4 (Cyclosporin, Lidocaine, Quinidine, Estradiol, Erythromycin, Budesonid).
