indications

Omicap - Kit drugs are indicated in the following cases:

  • H.pylori destruction in chronic gastritis, gastric and duodenal disease.

Helicobacter pylori is considered to be the cause of gastric ulcer disease in humans. Three drug therapies include proton pump inhibitors such as omeprazole in combination with clarithromycin and tinidazole can be used as a short, simple and effective treatment time regimen.

Omeprazole is a gastric acid secretion of Benzimidazole.

Omeprazole combines with H'K 'atpase in the stomach wall, which is inactive of this enzyme system by inhibiting the secretion of hydrochloric acid by stomach wall cells in the last stage. Omeprazole also inhibits basic or stimulating gastric acid secretion.

Tinidazole is a 5 - nitroimidazole has a longer and stronger effect than Metronidazole. It has anti -single and anaerobic bacteria.

Tinidazole is an antibacterial drug that quickly kill H.pylori. Tinidazole works by damaging DNA fibers or inhibiting their synthesis.

Clarithromycin has antibacterial effects by combining with the subunit of 50s ribosom of sensitive bacteria and inhibiting protein synthesis.

Clarithromycin has a good activity in vitro (mic90 value is 0.03 g/ ml) against H. pylori so it is an appropriate substance that is put into use in the H.pylori killer.

    Dynamic pharmacokinetics

    omeprazole

    absorption:

Omeprazole is completely absorbed in the small intestine after drinking for 3 to 6 hours. Biology about 60%.

Food does not affect the absorption of drugs in the intestine. The absorption of omeprazole depends on the dose.

The drug can increase its own absorption and bioavailability due to acidic excretion.

    Distribution:

The drug is highly attached to plasma proteins (about 95%) and is distributed in tissues, especially in the border cells of the stomach.

Biological use of the first dose is about 35%, but will increase by about 60% when drinking the next day.

Although half -life is short (about 40 minutes), but the acid excretion effect is prolonged, so it can be used only once a day.

    Metabolism and elimination:

Omeprazole is almost completely metabolized in the liver, quickly excreted, mainly through urine (80%), the rest is in the fertilizer.

The metabolites are not active, but interact with many other drugs due to inhibiting the enzymes of the cytochrom P450 of liver cells.

The pharmacokinetics of the drug are not changed significantly in the elderly or the patient with renal function. In people with liver function, the bioavailability of the drug increases and the clearance of drugs decreases, but there is no accumulation of drugs and metabolites of the drug in the body.

    clarithromycin

    absorption:

Clarithromycin, when taken, is quickly absorbed through the gastrointestinal tract and is subjected to the first metabolism at a high level that reduces the biochemical of the mother medicine to about 55%.

The absorption level is almost not affected by food. The peak concentration of clarithromycin and the main metabolites 14 - hydroxy clarithromycin is about 0.6 - 0.7 micrograms/ ml, after taking a single dose 250 mg.

In the dynamic equilibrium at the same dose level for the peak concentration of about 1 microgram/ ml.

Clarithromycin pharmacokinetics are not linear and dependent on doses. Large doses can create peak concentrations without proportion due to saturated drug metabolism.

    Distribution:

Clarithromycin and main metabolites are widely distributed and the concentration in tissue exceeds the serum concentration due to a part of the drug is collected into the cell.

    Metabolism and elimination:

Metabolic drugs are high in the liver and release stool through biliary tract. A significant part is discharged through urine.

About 20 and 30% in the order of the dose of 250 mg and 500 mg are discharged in urine in the form of non -metabolized. 14 - Hydroxy Clarithromycin as well as other metabolites are also discharged through urine.

Half -life Clarithromycin is about 3-4 hours when the patient takes 250 mg Clarithromycin, 2 times/ day, and about 5-7 hours when the patient takes the dose of 500 mg, 2 times/ day. Half life is prolonged in patients with kidney failure.

    tinidazole

    absorption:

Tinidazole is absorbed most after drinking and especially the peak concentration in plasma reaches about 40 micrograms/ ml after 2 hours of using a single dose of 2 g, dropped to 10 micrograms/ ml after 24 hours and 2.5 micrograms/ ml after 48 hours.

With a daily maintenance dose of 1 g can maintain concentrations on 8 micrograms/ ml.

The equivalent dose, used by intravenous line, also gives similar plasma concentrations such as oral dose. Half lifetime elimination in plasma is 12 - 14 hours.

    Distribution:

Tinidazole is widely distributed and the concentration achieved in bile, milk, cerebrospinal fluid, saliva and other tissues in the body are similar to plasma concentrations; The drug is easy through the placenta. Only 12% attached to plasma proteins.

    Era:

The drug has not metabolized and the metabolites of the drug are excreted in the urine and a less part in the feces.

    How to use

    omicap - Kit used orally.

Need to take omeprazole pills away from meals, must swallow the whole tablet without chewing.

    Dosage

in the morning, 1 omeprazole capsule + 1 tinidazole + 1 clarithromycin tablet.

The evening also drinks the same morning.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when using overdose? In all cases of overdose, symptomatic treatment and supportive treatment. Each unrelated substance needs to be pulled from the digestive tract and need to monitor patients carefully.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Omicap - Kit contraindicated in the following cases:

    omeprazole:

  • Hypersensitivity to Omeprazole.
  • Clarithomycin:

  • People who are allergic to macrolids.
  • Absolute contraindications with terfenadin, especially in the case of heart disease such as arrhythmia, slow rhythm, prolonged q - t interval, local myocardial anemia or electrolyte imbalance.
  • tinidazole:

  • Hypersensitivity to Tinidazol,
  • Hematopoiia or a history of acute porphyrin metabolism.
  • The first three months of pregnancy; The mother is breastfeeding.
  • Patients with physical neurological disorders.
  • Be cautious when using

omeprazole: If suspected to be a stomach ulcer, it is necessary to eliminate malignant possibility before taking omeprazole, because this medication can cover malignant diseases and slow diagnosis.

Clarithromycin: If there is a serious kidney failure with or without liver failure, the dose should be reduced and prolonged the medication distance.

The ability to drive and operate machinery

    does not affect.

    Pregnancy

Pregnancy: Not for the first three -month pregnancy. Only use Omicap - Kit in pregnant people after the first three months when really necessary.

    The period of breastfeeding

It is unclear whether the ingredients of Omicap - Kit are excreted through breast milk or not and because there are many drugs through breast milk, it is best not to use omicap - kit during breastfeeding, unless it is necessary.

    Drug interaction

omeprazole has the ability to interact with the Cytochrom P450 enzyme system, then reduce or inhibit the metabolism of drugs such as Diazepam, Warfarin and Phenytoin. Because Omeprazole inhibits deep and prolonged stomach acid secretion, Omeprazole can affect the absorption of drugs that the stomach pH determines their bioavailability (such as ketoconazole, ampicillin, ester and iron salts).

Clarithromycin for patients who are using theophylllin may increase theophyllin level in serum. Clarithromycin may increase carbamazepine levels. Combining erythromycin with digoxin has seen increased digoxin content.

Cimetidine can reduce the excretion of Tinidazole from the body. Perhaps due to cimetidine inhibits the metabolism of Tinidazole in the liver, it increases both treatment and toxic effects. Rifampicin may increase Tinidazole. It may be due to increased metabolism of Tinidazole in the liver and reduces the treatment effect. Ethylic alcohol can cause "alcohol antabuse syndrome".

    Storage

Store in a cool dry place, avoiding light. To be out of reach of children.
