Uses

indications

OPECERIN 50MG OPV 3x10 is indicated in the following cases:

Treatment of symptoms of hip or knee degeneration, with slow effect. It is not recommended for the treatment of hip degeneration that has progressed rapidly due to being weaker with diacerein.

Pharmacokinus

Medicine group treatment: Anti -inflammatory and anti -rheumatism, non -steroids.

Diacerein is an anthraquinon derivative with medium anti -inflammatory activity. It is an anti -inflammatory drug used high doses without causing irritation to the stomach. The drug has a slow impact and appears around the 30th day and significantly impacts around the 45th day.

in vitro, diacerein have the following properties:

  • Practicing and moving of macrophages. This substance is a sulpho complex.

After absorbing a single dose of 50 mg of diacerein, the peak of plasma peaks reaches an average after 2.5 hours and the peak concentration of the drug is 3 mg/l. Using diacerein during meals increases bioavailability (the area below the curve increases by nearly 25%) and the absorption is delayed.

For dose from 50 to 200 mg of diacerein at a single dose, all pharmacological parameters do not depend on the dose.

Very high protein cohesion (99%). Mainly linking high affinity with albumin.

Rhein's waste time is about 4.5 hours. The total amount of excretion in urine is about 30%. Rhein is eliminated in urine up to 80% in the form of sulfo and glucuronid and 20% in constant form.

At the repeated dose (50 mg x 2 times daily), diacerein has low accumulation.

In patients with severe renal impairment (creatinine clearance below 30 ml/minute), the area under the curve and the half -life is double and the excretion through the urinary tract decreases by half.

In elderly patients, due to the good tolerance of the drug, it is not necessary to change the dose, although the elimination is slower.

No dose adjustments in patients with medium renal failure and the elderly.

Renal failure: In case of severe renal failure (Creatinine clearance below 30 ml/minute), daily dose should reduce mineral minerals 50% recommended for adults.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

How to handle: It is necessary to treat symptoms with water-electric balance correction. No specific antidote.

  • Disorders of the digestive system: abdominal pain, diarrhea.
  • Common (1/10> 1/100)

  • Disorders of the digestive system: Distant to many times, increasing flatulence peristalsis. These effects will decrease or disappear by themselves after a period of treatment. Some cases of severe diarrhea can lead to dehydration and electrolyte balance disorders.
  • Skin disorders and soft tissue: itching, rash, eczema.

    Uncommon, 1/1000

  • Liver -bile disorders: Serum enzyme increased.
  • Data from post -commerce monitoring

Bile liver disorders: Cases of acute liver damage, including serum liver enzymes and cases of hepatitis related to diacerein were reported during post -commerce. Most of these cases occur in the first months at the beginning of treatment. Need to closely monitor the signs and symptoms of liver damage in patients.

Instructions on how to handle ADR

Stop using the drug. With minor adverse reactions, usually just stop the drug. In case of severe sensitivity or allergic reactions, supportive treatment (airy keeping and using epinephrin, oxygen breathing, antihistamine, corticoid ...).

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindications:

OPECERIN 50MG OPV 3X10 Contraindicated in the following cases:

  • Hypersensitivity to diacerein or any excipients.
  • Colitis (ulcerative colitis, Crohn disease ...). Intestinal obstruction or semi -bowel syndrome. Abdominal pain syndrome of unknown causes.
  • Too hypersensitivity to Rhein and substances that are almost similar to it.

Patients who are liver diseases or a history of liver disease.

  • Patients with a history of severe diarrhea, dehydration, hypokalemia to be hospitalized.
  • Be cautious when using

diarrhea: regular Diacerein can cause diarrhea, thereby leading to tears and reducing blood potassium. Patients should stop using when diarrhea and exchange with the doctor for treatment of alternative treatments.

Be cautious when using diacerein for patients who use diuretics due to increased risk of dehydration and hypokalemia. Especially cautious to prevent hypotension in patients using the cardiac glycosides (Digitoxin, Digoxin). Avoid simultaneous use of diacerein with laxatives.

Liver toxicity: Increased serum liver enzyme level and acute liver damage symptoms were recorded during the circulation of diacerein in the market.

Before starting treatment with diacerein, the patient should be asked for attached and history diseases, especially liver diseases and screening the main causes of progressive liver disease. If diagnosis of liver disease is identified, contraindicated using diacerein.

Need to closely monitor the signs of liver damage and caution when using diacerein simultaneously with drugs that can cause liver damage. Recommend patients to reduce alcohol while being treated with diacerein.

Stop using diacerein if liver enzyme detectors are detected or suspected of signs and symptoms of liver toxicity and requires patients to immediately contact the treating doctor in case of detection of signs of liver damage. Renal failure changes the pharmacokinetics of diacerein and is needed to reduce the dose when the clearinine clearance is

Not for children under 15 years old.

This drug contains lactose. Patients with rare genetic problems galactose intolerance, Lapp Laclase deficiency or Glucose-Galactose should not use this drug.

    Pregnancy

Experimental research in animals shows that the bone delay in the fetus is due to the toxicity of the mother's individual toxicity in high doses. There is currently no adequate clinical data to assess the effects of deformities or monsters possible by diacerein when used during pregnancy. Therefore, it is not recommended to use this drug during pregnancy.

    The period of breastfeeding

should not use this medication for breastfeeding women. Because Anthraquinon derivatives go into breast milk in small amounts that have been proven in the literature documents.

should not be used simultaneously diacerein with drugs that change the time to move food through the intestine. Do not use simultaneously with aluminum hydroxide and/or magnesi medications to maximize the bioavailability of diacerein.

Diacrein wastes can lead to diarrhea and hypokalemia. Caution should be carefully used with diuretics and/or cardiac glycosides (digitoxin, digoxin), due to increased risk of arrhythmia.
