Pantoprazol is a proton pump inhibitor. The drug in the acid secretary tubes of the stomach walls to be converted into activated sulfenamid, this form is associated with the HK-ATPASE enzyme (also known as the proton pump) on the surface of the cell into the stomach, inhibiting this enzyme, preventing the final step of the acid secretion into the stomach.

So Pantoprazol has the effect of inhibiting the basic acid secretion and even when the stomach is irritated by any stimulating agent. The effect of pantoprazol depends on the dose, the time to inhibit the excretion of gastric acids lasts more than 24 hours, although the half -life of excretion of Pantoprazol is much shorter (0.7 - 1.9 hours).

After the oral dose starts 40mg of pantoprazol, the excretion of gastric acid is inhibited an average of 51% after 2.5 hours. Take Pantoprazol once a day 40mg in 7 days reduces up to 85% of gastric acid excretion. Excreting stomach acid back to normal within 1 week after stopping pantoprazol and there is no phenomenon of hygiene (rebound). In addition, Pantoprazol can eliminate Helicobacter pylori in the stomach with a duodenal ulcer and/or reflux esophagitis contaminated with such bacteria. In vitro pantoprazole reduces the number of H. pylori more than 4 times in PH4.

    pharmacokinetic

    absorption

Pantoprazole absorbs fast, reaching the highest concentration in the blood after drinking about 2 - 2.5 hours. Pantoprazol is absorbed well, less initially metabolized in the liver, oral use about 77%.

    Distribution

Pantoprazol is strongly attached to plasma proteins (98%), the volume of distribution in adults is 0.1 liters/kg. The prolonged time to prevent gastric acid secretion when intravenously pantoprazol is 24 hours. After an intravenous injection of a single dose of 20 to 120mg, the drug effect starts within 15-30 minutes and the effect within 24 hours depends on the dose of 20 - 80mg. Within 2 hours after the injection of 80 ng, the acid excretion flow is completely removed. The dose of 120mg also does not increase the effect.

    Metabolism

Pantoprazole metabolizes mainly in the liver thanks to the cytochrom P450 enzyme system, isoenzyme CYP2C19 to convert it into Desmethylpantoprazol. A small part is metabolized by CYP3A4, CYP2D6, CYP29. In some people deficienced, the genetic CYP2C19 enzyme system (Asians are 17 - 23% of slow metabolism), which slowly metabolize pantoprazol, leading to a plasma concentration in plasma may increase 5 times higher than people with enough enzymes.

    Elimination

The half -life of pantoprazol elimination is 0.7 - 1.9 hours, prolonged in people with liver failure, cirrhosis (3 - 6 hours) or slow -metabolic metabolic people (3.5 - 10 hours). The metabolites excreted mainly through the kidneys (about 80%), 18% through the bile into the feces.
