Not recommended for children under 12 years old.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?

    paracetamol:

Symptoms of nausea, vomiting, and abdominal pain usually occur within 2-3 hours after taking the poison of the drug. Blood methemoglobin, which leads to blue-purple, mucous and nail blue, is a specific sign of acute poisoning P-aminophenol; A small amount of sulfhemoglobin can also be produced. Children tend to create methemoglobin easier than adults after taking paracetamol.

Management: Early diagnosis is important in the treatment of paracetamol overdose. There are methods to quickly determine the concentration of drugs in plasma. However, do not postpone treatment while waiting for the test results if the history is suggested to overdose.

When severe poisoning, it is important to treat positive support. Gastric lavage is needed in any case, preferably within 4 hours after drinking. The main detoxification is the use of sulfhydryl compounds, perhaps partly due to the addition of glutathion reserves in the liver.

n-acetylcysteine works when taken or intravenously. It must be used to detoxify immediately, as soon as possible if less than 36 hours after taking paracetamol. Treatment with n-acetylcysteine is more effective when giving the drug for less than 10 hours after taking paracetamol.

When drinking, dilute N-acetylcystein solution with water or drink without alcohol to achieve a 5% solution and must be taken within 1 hour after mixing. Give N-acetylcystein with the first dose of 140 mg/kg, then give 17 more doses, each dose of 70 mg/kg 4 hours apart.

Termination of treatment if the paracetamol test in plasma shows a low risk of liver toxicity. N-acetylcysteine can also be used by intravenous line: The initial dose is 150 mg/kg, mixed in 200 ml of 5%glucose, intravenous injection for 15 minutes; Then intravenously dose of 50 mg/kg in 500 ml of 5% glucose for 4 hours; The next is 100 mg/kg in 1 liter of solution within the next 16 hours. Without 5% glucose solution, 0.9% sodium chloride solution may be used.

ADR of n-acetylcystein includes skin rash (including urticaria, no need to stop the drug), nausea, vomiting, diarrhea, and anaphylactic reaction.

Without n-acetylcystein, methionine can be used (see specialized methionine). If you have used activated carbon before using methionin, you must remove activated carbon from the stomach first. Also can use activated carbon and/or salt bleach because they have the ability to reduce the absorption of paracetamol.

    caffein:

Symptoms of epigastric pain, vomiting, diuretic, tachycardia, arrhythmia, central nervous stimulation.

Management: There is no specific antidote, can use activated carbon for 1-4 hours after overdose.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Pasepan medicine is contraindicated in the following cases:

  • Hypersensitivity to any of the ingredients of the drug.
  • People with liver failure, severe kidney failure.

    Be cautious when using

    paracetamol:

Risk of cardiovascular thrombosis: Doctors need to periodically evaluate the appearance of cardiovascular events, even if the patient has no previous cardiovascular symptoms.

Paracetamol is not toxic at the dose of treatment.

Sometimes skin reactions include itching and urticaria; Other sensitive reactions include larynx edema, angioedema, and anaphylactic reactions that may rarely occur. Platelets, leukopenia, and all bloody hematuria have occurred with the use of p-aminophenol derivatives, especially when used for large doses. Neutral leukopenia and thrombocytopenic hemorrhage occur when using paracetamol. Rarely loss of granulocytes in patients using paracetamol.

Be cautious when using paracetamol for people with liver failure, kidney failure, alcoholic, chronic malnutrition or dehydration. Avoid high doses, prolonged and intravenous use for people with liver failure.

Must use paracetamol carefully in patients with anemia before, because purple blue may not manifest clearly, although high concentrations at a dangerous level of methemoglobin in the blood.

Doctors need to warn the patient about signs of serious skin reactions such as Steven -Johnson syndrome (SJS), toxic skin necrosis syndrome (Ten - Toxic Epidermal Necrolysis) or Lyell syndrome, acute all -body pustular syndrome (AGEP - Acute Generalized Examthematous Pustuosis).

Serious side effects on the skin, although the incidence is not high but serious, even life-threatening including Steven-Johnson syndrome (SJS), Toxic Epidermal Necrolysis (Ten) or Lyell's syndrome, acute pustular syndrome (AGEP).

When detecting signs of rash on the first skin or any other hypersensitive reactions, patients need to stop using the drug. People who have had severe skin reactions caused by Paracetamol must not use the drug again and when they come to medical examination and treatment, they need to notify the medical staff on this issue.

    caffeine:

While taking the drug should not use a lot of coffee, tea and some caffeine beverages.

    Milk shell:

In cats, dogs and experimental monkeys, echitamin in the milk shell causes hypotension. No report in humans.

In the ingredient with nipagin, nipasol, erythrosin, and Brilliant Blue can cause allergic reactions.

The ability to drive and operate machinery

The drug does not cause drowsiness, does not affect the operation of the person who is driving or operating machinery.

    pregnancy

    does not recommend use.

    breastfeeding period

paracetamol and caffeine excreted through breast milk. Cafeine stimulates breastfeeding. Should be cautious when breastfeeding, consideration between risks and benefits.

    Medicinal interaction

long -term and high -dose paracetamol oral doses increases the anticoagulant effects of COMAARIN and indoors. This effect is less or not clinically important, so paracetamol is more popular than salicylate when it is necessary to relieve mild pain or reduce fever for patients who are using cooumarin or indandion derivatives.

It is necessary to pay attention to the likelihood of serious antipyretics in patients to use simultaneously phenothiazine and cooling therapy.

Out of alcohol too much and long may increase the risk of liver toxicity. Anti -convulsions (including Phenytoin, Barbiturat, Carbamazepin) that causes enzyme induction in the liver microsom, can increase the toxicity of paracetamol due to increased drug conversion into toxic substances to the liver.

In addition, simultaneous use of isoniazid with paracetamol can also lead to an increased risk of toxicity to the liver but have not determined the exact mechanism of this interaction.

cholestyramin: reduces the absorption of paracetamol.

caffeine: Mainly metabolized through CYP1A2 should have pharmacokinetic interactions with metabolic drugs through CYP1A2 or CYP1A2 induction/ inhibitors such as anti -convulsions, beta, cimetidine, disulfiram, ketoconazol, ketoprofen ...

    Milk cover: No report.
