Indications
Biseptol 480 drugs are indicated in case of infections caused by bacteria sensitive to co-trimoxazol:
biseptol is a mixture of trimethoprim (1 part) and sulfamethoxazol (5 parts). Trimethoprim is a derivative of pyrimidine that inhibits the specific enzyme dihydrofolat reductase. Co-trimoxazol is a mixture of sulfamethoxazol: trimethoprim in the ratio 5: 1. Sulfamethoxazol is a sulfonamide, inhibiting the competition of bacterial folic acid synthesis. Combining trimethoprim and sulfamethoxazol so that inhibits two consecutive stages of folic acid metabolism, thus effectively inhibits the synthesis of purin, thymin and final DNA of bacteria. This serial inhibition has bactericidal effects. This co -mechanism also resists the growth of anti -drug bacteria and makes the drug work even when bacteria resist each ingredient of the drug.
The optimal effect against most microorganisms is seen in a ratio of 1 part of trimethoprim with 20 parts of sulfamethoxazol. Co-Trimoxazol preparations are combined with the ratio of 5 parts of sulfamethoxazol and 1 trimethoprim part; Due to the difference in pharmacokinetics of 2 drugs, the body's peak concentration ratio reaches approximately 1:20. However, it is unclear whether the drug can achieve the optimal rate in all positions, and if both drugs achieve the concentration of treatment, the contribution of synergies to the effect of co-trimoxazol Invivo is still unknown. The following microorganisms are often sensitive to drugs: E. Coli, Klebsiella Sp. H. Influenzae (including ampicilin strains), S. Pneumoniea, Shigella Flexneri and Shigella Sonnei, Pneumocystis Carinii. Co-Trimoxazol has a few effects on Plasmodium Falciparum and Toxoplasma Gondi. The anti -drug microorganisms are: Enterococcus, Pseudomonas, Campylobacter, anaerobic bacteria, meningococcal brain, gonorrhea, Mycoplasma.
pharmacokinetics
After drinking, both trimethoprim and sulfamethoxazol are quickly absorbed and highly available. After 2 hours of taking at a dose of 800 mg of sulfamethoxazol and 160 mg of trimethoprim, the average serum concentration of trimethoprim is 2.5 mg/liter and of sulfamethoxazol is 40 - 50 mg/liter. The stable concentration of trimethoprim is 4 - 5 mg/liter, of sulfamethoxazol is 100 mg/liter after 2-3 days of treatment with 2 doses daily. The half -life of Trimethoprim is 9 - 10 hours, of sulfamethoxazol is 11 hours.
So for 12 hours apart is appropriate. The 1: 5 ratio between trimethoprim and sulfamethoxazol is the most suitable for tablets. However, in serum, this ratio is 1: 20 because Trimethoprim diffuses better out of blood vessels, entering the tissues. Trimethoprim goes into tissues and fluids better than sulfamethoxazol. The concentration of drugs in urine is 150 times higher than the serum concentration.
Before taking Pharbaco biseptol drugs treat infections caused by sensitive bacteria (1 blister x 20 tablets)
How to use
Take medication during or immediately after meals. Drink plenty of water when taking the drug.
Dosage
Urinary tract infections, gastrointestinal tract caused by shigella Bacilli and the acute chronic bronchitis in adults:
The usual dose is 960 mg of co-trimoxazol (2 biseptol 480), 2 times a day. Take drugs for 10-14 days for gastrointestinal infections, 14 days for the acute and chronic bronchitis and 5 days for the gastrointestinal tract infection caused by Shigella Bacilli.Urinary tract infections, gastrointestinal tract caused by Shigella Bacilli and acute otitis media in children:
usually use 48 mg of co-trimoxazol/kg body weight/day divided into 2 times. Do not overdose specified for adults. Take 10 -day drug for gastrointestinal infections and acute otitis media, 5 days for bacterial infections, caused by Shigella Bacilli.
pneumocystis carinii pneumonia in adults and children:
The proposed dose is 90 - 120 mg of co -trimoxazol/kg weight/day divided into 4 times in 14 - 21 days.Prevention of infections caused by pneumocystis carinii:
Adults: 960 mg co-trimoxazol (2 biseptol 480), 1 time/day for 7 days.
Children: 24 mg co-trimoxazol/kg body weight, divided into 2 times in 3 consecutive days. The maximum dose of 1920 mg (4 biseptol 480).
Diarrhea in adults is caused by E. coli: The proposed dose is 960 mg (2 biseptol 480) every 12 hours.
For patients with renal failure. Patients with clearance> 30 ml/min, use regular doses. Half-dose reduction for patients with renal impairment with creatinine clearance 15-30 ml/min. Do not use drugs for patients with creatinine clearance below 15 ml/min.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Symptoms
Symptoms may occur during an overdose including anorexia, nausea, vomiting, dizziness, headache, fever, sleep, loss of consciousness, bleeding or crystal. The later stage may appear inhibiting the activity of bone marrow and jaundice.
Handling
When an overdose includes gastric lavage, vomiting, drinking plenty of water if urinary minorities and normal kidney function. Excretion trimethoprim increases when the patient's urine is acidicated. For patients to use drugs to alkaline urine and increase the secretion of the drug will help the output of sulfamethoxazol. Overdose treatment is generally proposed to drink lots of water and monitor blood and electrolytes. If the proposed overdose is used, immediately notice to the doctor or pharmacist.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Common, ADR> 1/100
Rare, ADR
Instructions on how to handle ADR
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Biseptol 480 drugs in the following cases:
Precautions when using
impaired renal function.
People are prone to folic acid deficiency such as elderly patients, patients using high doses of co-trimoxazol.
Pregnant women, dehydration, malnutrition.
Co-trimoxazol can cause hemolytic anemia in people with a deficiency G-6PD.
The ability to drive and operate machinery
The drug does not affect the driver and operating machinery. However, it is necessary to be cautious when unwanted effects can occur such as headache, dizziness, stress, fatigue.
Pregnancy
The drug is only used during pregnancy if the doctor has considered the benefit between the use of the drug for the mother and the risk of the fetus.
Breastfeeding period
Both trimethoprim and sulfamethoxazol are absorbed into the breast milk, so do not use medication while breastfeeding.
Drug interaction
thiazid diuretics: simultaneous use of co-trimoxazol with diuretics, especially thiazid in older patients may increase the risk of platelet reduction and bleeding.
Phenytoin: Co-trimoxazol increases the effect of phenytoin.
methotrexate: sulfonamid increases the effect of methotrexate.
Sulfonylurea derivatives: Co-trimoxazol increases the effects of diabetes with sulfonylurea output, thus lowering strong blood sugar.
Digoxin: Co-trimoxazol increases blood digoxin levels in elderly patients.
Triple-round antidepressants: Co-trimoxazol reduces the effects of three-round antidepressants.
pyrimethamine: Co-trimoxazol simultaneously used with pyrimethamine 25mg/week increases the risk of huge red blood cell anemia.
Cyclosporin: In patients with kidney transplantation when treated with co-trimoxazol and cyclosporin, there have been records of cases of temporary disorders of the implanted renal function leading to increased serum creatinine levels, possibly due to the effects of trimethoprim.
Due to the chemical structure, sulfonamid may cause an allergic reaction to patients with hypersensitivity to anti -thyroid drugs, diuretics (acetazolamid and thiazid) and some other oral diabetes.
Co-trimoxazol can extend the time of prothrombin in patients who are using warfarin.
