Uses

indications

Piromax 10 mg is indicated in the following cases:

  • Treatment of rheumatoid arthritis and osteoarthritis, osteoarthritis. Oxicam, the drug has anti -inflammatory, analgesic and antipyretic effects. The mechanism of action of the drug is not clear. However, the general mechanism for the above effects may be due to inhibition of prostaglandin synthetase, thus preventing the formation of prostaglandin, thromboxane, and other products of cyclooxygenase enzymes. Piroxicam can also inhibit the activity of neutral polygon leukemia, even when there are products of cyclooxygenase, so anti -inflammatory effects also include proteoglycanase and collagenase inhibitors in cartilage. Piroxicam does not work by stimulating the pituitary -kidney axis. Piroxicam also inhibits platelets.

Because Piroxicam inhibits the synthesis of prostaglandin in the kidneys, it causes reduced blood flow to the kidneys, this dose is especially important for patients with kidney failure, heart failure and liver failure, and important for patients with changes in plasma volume. After that, decreased to form prostaglandin in the kidneys can lead to acute renal failure, water retention and acute heart failure.

    Pharmacokinetics

    absorption

Piroxicam is well absorbed through the gastrointestinal tract. The peak concentration in plasma appears 3-5 hours after taking the drug. Food and antacids do not change the speed and level of drug absorption. Because Piroxicam has a liver -intestinal cycle and has a lot of difference in absorption among patients, the sale time of the drug in plasma changes 20 - 70 hours, which can explain why the effects of the drug are so different from patients, when using the same dose, this also means that the stable state of the drug is achieved after very different treatment time, from 4 to 13 days.

    Distribution

The drug is very strong with plasma proteins (about 99%). The distribution volume is approximately 120 ml/kg. The concentration of drugs in plasma and in the epidemic is approximately equal when the drug state is stable (ie after 7-12 days).

    Metabolism - Elimination

less than 5% of urine excreted drugs in the form of unchanged. The main metabolism of the drug is hydroxylation of pyridine, followed by glucuronic acid, then this complex is excreted in urine.

    How to use

    oral medication.

    Dosage

    Adults

20 mg x 1 time/day (some people may respond to a dose of 10 mg daily, others may have to use 30 mg daily, drink once or divided into 2 times a day).

Because the sale time of the drug is prolonged, the drug concentration has not reached a stability within 7-10 days, so the response to the drug increases slowly over a few weeks.

    Treatment of acute goutt

    40 mg/day x 5 - 7 days.

    Children

The drug should not be used for children. However, Piroxicam can also be used for children 6 years of age and older with rheumatoid arthritis. Usual oral dose: 5 mg/day for children weighing less than 15 kg, 10 mg/day for children weighing 16-25 kg, 15 mg/day for children to weight 26 - 45 kg, and 20 mg/day for children to weigh 46 kg or more.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

    Symptomatic treatment and support.

Currently, due to the lack of experience in handling overdose, it is impossible to know the effectiveness and recommendations of specific detoxification effectiveness. However, gastric and supportive treatment measures should be applied. Drinking activated carbon can reduce the absorption and reabsorption of Piroxicam effectively, and thus reduces the remaining drug.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Instructions on how to handle ADR

Notify the doctor the unwanted side effects when using the drug.

Doctors should regularly monitor patients with signs of ulcerative syndrome and gastrointestinal bleeding, notifying the patient closely monitor those signs. If it happens, it is necessary to stop the drug.

Patients who are taking drugs that have vision disorders need to be tested and evaluated for their eyes.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Piromax 10 mg is contraindicated in the following cases:

  • Hypersensitivity to piroxicam.
  • Stomach ulcer, acute duodenum ulcer.

History of bronchospasm, asthma, nasal polyps and quincke edema or jute caused by aspirin or another nonsteroidal anti -inflammatory drug.

    cirrhosis.

    Severe heart failure.

People at high risk of bleeding.

Renal failure people with glomerular filtration level below 30 ml/min.

    Be cautious when using

Use cautiously in the following cases:

  • Elderly people.

The ability to drive and operate machinery

Caution when using piroxicam for train drivers and operating machinery because the drug can be stunned.

    Pregnancy

Like other Prostaglandin inhibitors and release, Piroxicam causes a risk of lung pressure in newborns due to artery closing before birth if these drugs are used in the last 3 months of pregnancy. Piroxicam also inhibits labor, prolongs pregnancy, and has toxicity for gastrointestinal tract in pregnant women. Do not use piroxicam for pregnant women in the last 3 months or near labor.

    Breastfeeding period

Piroxicam excreted into breast milk. Not determined the safety of the drug used for mothers during breastfeeding.

    Drug interaction

When taking piroxicam simultaneously with anticoagulant drugs and drugs with high protein bonding, it is necessary to monitor patients closely to adjust the dose of the drugs accordingly. Because of the high protein bonding ability, Piroxicam can push other drugs from plasma proteins.

Do not treat simultaneously with aspirin because this will lower the plasma concentration of piroxicam (about 80% when treated with 3.9 g of aspirin), which is not more effective than when only treated with aspirin, which increases unwanted effects.

When treating the drug simultaneously with lithium, it will increase lithium toxicity due to increasing lithium concentration in plasma, so it is necessary to closely monitor the concentration of lithium in plasma.

Use piroxicam simultaneously with antacidants that do not affect the concentration of piroxicam in plasma.

  • Storage

Store in a dry place, avoid light, temperature not exceeding 30 ° C.
