Uses

Indications

Pizar 6 drugs are indicated for the following infection treatment:

Gastrointestinal worm infection

  • Ivermectin is indicated for the treatment of gastrointestinal eelworm infection (uncommon) due to Strongyloides Stercoralis.
  • Ivermectin is indicated for treatment of worm infections only due to onchocerca Volvulus roundworms. Comparative research using Diethylcarbamazin Citrate (Dec-C). The mature biological period resides in subcutaneous notes that are not discovered. This nodulectomy removal surgery can be considered in the treatment of patients infected with worms, because they will eliminate mature parasites producing larvae.

    Pharmacokology

Ivermectin is the semi -synthetic substance of one of Avermectin, a group of lacton structures, isolated from the streptomyces Avermitilis fermentation.

Ivermectin has a wide activity street on roundworms and secretions of cattle, thus being used in veterinary. The drug was first used for humans in 1981. Ivermectin was effective on a variety of round worms such as races, hair worms, needle worms, roundworms, hookworms and worms Wuchereria bancrofti. However, the drug does not work on liver fluke and tapeworms.

Currently, Ivermectin is a chosen drug to treat worms only onchocerca Volvulus and is a worm larvae that is only very strong, but has little effect on mature parasites. After drinking for 2-3 days, the worm larvae only go away quickly, while the cornea and eye room are slower.

The effect of the drug may last up to 12 months. One month after use, the larvae at the uterus only mature unable to escape, and then degenerated and eliminated. The effect of prolonged larvae is very helpful in preventing the spread of the disease.

already know quite well the mechanism of action of Ivermectin on worms only onchocerca. The drug causes direct toxic effects, immobile and eliminating larvae through lymphatic lymphatic. Ivermectin stimulates the secretion of neurotransmitters, gamma-amino butyric acid (GABA). In sensitive worms, the drug acts by enhancing the release of gaba after the neuromuscular joint, causing the worm to be paralyzed.

Ivermectin does not easily join the central nervous system of mammals. Therefore, it does not affect the nerve conduction dependent of these species.

    pharmacokinetic

    absorption

Ivermectin is absorbed after oral, is still unknown to the absolute use of the drug after taken. In general, there is no difference in the peak concentration in the plasma between the tablet form and the capsule form (46 compared to 50 micrograms/l).

However, when using iVeMectin in a alcohol solution, the peak concentration doubled and the relative bioavailability of the tablet, so it is only 60% of the bioavailability of the solution. Time reaches a plasma concentration of about 4 hours and is not different between the form of preparation.

    Distribution

Ivermectin has an integral distribution of 3 - 3.5 l/kg, not through the brain barrier.

About 93% of the drug is associated with plasma proteins.

    Metabolism

Metabolic drugs in the liver, mainly through Cytochrom P450 Isoenzym CYP3A4.

    Elimination

The sale time of Ivermectin is about 18 hours. The drug is eliminated mostly in the form of metabolism for about 2 weeks, mainly through feces, less than 1% of the dose is discharged through urine and less than 2% in milk.

    How to use

Pizar 6 drugs are taken orally. The drug is taken with water in the early morning when not eaten or may be elsewhere, but avoid eating within 2 hours before and after taking the drug.

    Dosage

    Grew worm infection

Ivermectin's recommended dose in the treatment of eelworm infection is a single dose of oral design to provide about 200 µg/kg body weight Ivermectin.

See Table 1 for a dose instructions. In general, there is no need for additional dose. However, stool tests should be tested after taking the drug to confirm the parasite has been destroyed.

Table 1. Instructions for Ivermectin dose to treat eelworm infection:

    weight (kg)
Oral single dose 50 9MG Only

Ivermectin's recommended dose in the treatment of worm infection is only a single oral dose equivalent to about 150 µg/ kg body weight Ivermectin. See table 2 for instructions. In the mass distribution campaign in the international treatment program, the most common time to use the drug is 12 months. When treating for each patient, it is possible to consider re -treatment in short periods of about 3 months.

Table 2. Ivermectin dose instructions to treat worm infection only:

Body weight (kg) Oral single dose 64 9mg Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose? Other undesirable effects include seizures, loss of air conditioning, shortness of breath, abdominal pain, abdominal pain and urticaria.

When infected, infusion and electrolytes, respiratory aids (oxygen and artificial respiration if needed), use hypertension if hypotension. Causing vomiting or gastric wash as soon as possible. After that, use bleach and other anti -toxic measures if needed to prevent the absorption of more drugs into the body.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using Pizar 6, you may experience unwanted effects (ADR).

Ivermectin is a safe drug, very suitable for broad treatment programs. Most of the unwanted effects of the drug are due to immune reactions to dead larvae. Therefore, the severity of this effect is related to the density of larvae in the skin. Unwanted effects have been notified including fever, itching, dizziness, edema, sensitive skin pain in lymph nodes, sweating, shivering, muscle pain, joint swelling, face swelling (Mazzotti reaction). Hypoglycemic hypotension has been released with sweating, tachycardia and confusion.

Unwanted effects often occur in the first 3 days after treatment and depending on the dose. The ratio of unwanted effects has been notified very differently. In a report including 50,929 patients using iVemectin, the unwanted effect is about 9%. In areas with high circulation, the ratio of unwanted effects can be much higher.

Here is ADR: In the treatment of worms only onchocerca dose 0.1 - 0.2 mg/kg.

    Common, ADR> 1/100

  • Osteoarthritis: joint pain/epidemicitis (9.3%).
  • Skin: itching (27.5%), skin reactions such as edema, nodules, pustules, skin rash and urticaria (22.7%).
  • Systemic: Fever (22.6%).
  • Central nervous system: headache (0.2%).

Notify the doctor the unwanted effects when using the drug.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Pizar 6 contraindications in the following cases:

  • Hypersensitivity to Ivermectin or any ingredients of the drug.

    Precautions when using

still do not have much experience using drugs for children under 5 years old, so it is not used for this age.

Due to gaba increased drugs, there is a notion that the drug has the effect on the central nervous system in humans, the barrier of the brain is damaged (such as in meningitis, trypanosoma disease).

When treated with Ivermectin for people with worm -caused dermatitis onchocerca, the reaction can be increased, more harmful reactions may occur, especially edema and make the condition worse.

To be out of reach of children.

The ability to drive and operate machinery

There is no data showing that the drug has an influence on the ability to drive and operate machinery. However, the unwanted effect of the drug is headache, dizziness, hypotension that may occur. Therefore, it is necessary to be cautious when driving and operating dangerous machines.

    Pregnancy

The monster effect has been monitored on research animals. Because there is no study controlled on pregnant women, Ivermectin is not recommended for pregnant women.

    Breastfeeding period

Ivermectin secretes breast milk with low concentrations (less than 2% of the drug used). Safety for infants has not been determined, so only this medication is for the mother when it is more beneficial to the mother.

    Medicinal interaction

Theoretically, the drug may increase the effects of anti -dramatic drugs of GABA receptors (such as benzodiazepine and sodium valproat).

Avoid simultaneous use of Ivermectin with BCG. Ivermectin reduces the effects of BCG, typhoid vaccine Ivermectin may increase the effects of anti -vitamin K drugs.

The effect of Ivermectin may increase when used simultaneously with azithromycin.

    P-Glycoprotein/ABCB1 inhibitors.

Take it with alcohol to increase plasma concentrations of Ivermectin.

  • Storage

In a dry place, avoid light, the temperature does not exceed 30ºC.
