Uses
indications
Plendil 5mg drug is indicated in the following cases:
Felodipine works by reducing the impact of peripheral vessels, especially the resistance in the arteries. Electrical and spasms of vascular smooth muscle cells are inhibited through effects on calcium channels in cell membranes.
Due to the selective effect on the smooth muscle in the artery area, Felodipine at the dose of treatment does not have a negative muscle contraction effect on the heart, and also has no clinical symbolic physiological effect.
Felodipine relaxes the respiratory muscle, has little effect on the gastric movement function, does not significantly affect clinical effects on blood lipids when treated long -term and does not affect metabolic control in patients with diabetes 2 in 6 months of treatment.
Felodipine can be used for patients with left ventricular failure and treated with regular therapy, or with bronchial asthma, diabetes, gout, or hyperlipidemia.
Anti -hypertension effect: Felodipine reduces arterial blood pressure by reducing the barrier in the peripheral vessels. When treated, the plendin will reduce blood pressure in a lying position, sit and even when resting during exercise. Felodipine will not cause hypotension because there is no effect on venous muscles or control of the sympathetic system. Lower blood pressure can cause temporary reflexes to increase heart rate and blood ratio, felodipine treatment is associated with the regression of left ventricular hypertrophy.
Anti -angina effect: Felodipine has the effect of dilating coronary artery, thereby improving perfusion and providing oxygen for the heart. Reducing the load in the heart by reducing peripheral artery resistance (post -load reduction), resulting in reducing myocardial oxygen demand, which stops coronary spasms. Felodipine has the effect of improving exertion and reduces the frequency of angina in patients with angina.
Dynamic pharmacokinetics
The active ingredient in prolonged release tablets is felodipine attached to a polymer that forms a gel layer when exposed to water, from which felodipine is continuously released leading to slow effects.
Absorption: Birth is about 15%, regardless of the absorption of food at the same time.
Distribution: The level of cohesion to plasma protein is about 99%, the distribution volume in a stable state is 10 l/kg.
Metabolic: Metabolized in the liver by cytochorome P450 3A4 and no identification metabolites have a vasodilation effect.
Elimination: The average clearance is 1200 ml/min. 70% of the dose is eliminated in the form of metabolites in urine, partly through stool.
Before taking Plendil 5mg Astrazeneca tablets treat hypertension, angina (3 blisters x 10 tablets)
How to use
prolonged release tablets should be used once a day in the morning and swallow with water. Time works within 24 hours. To maintain prolonged release characteristics, do not split, grind or chew the pill. Prolonged release tablets can be taken when hungry or with low -fat snacks and carbohydrates.
Dosage
Hypertension
The dose is adjusted to each patient. Treatment should be started by 5 mg once daily. The usual maintenance dose is 5-10 mg once daily. If necessary, the dose can be increased or added another medication to treat hypertension with felodipine. Normally, no doses are higher than 10 mg per day.
Stable angina
Should adjust the dose for each patient. The treatment should start by 5 mg x 1 time/day, and if necessary, it may increase to 10 mg x 1 time/day. Can coordinate felodipine with beta receptor inhibitors.
Elderly
Should consider the beginning of treatment with 2.5 mg once a day.
impaired renal function.
No dose adjustment in patients with renal function impairment. However, be careful when using felodipine in patients with severe renal impairment.
Liver function impairment
Patients with impaired liver function may increase felodipine levels in plasma and respond to lower doses.
Children
Due to limited clinical experience, it is advisable to avoid using felodipine in patients with hypertension children
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Toxicity:
Overdose can cause excessive peripheral vessels, causing significant hypotension and sometimes causing slow heart rate.
Management:
Activated carbon, if you need to wash your stomach.
If serious hypotension occurs, symptomatic treatment should be treated. Place patients in lying position, high foot. In case of being accompanied by slow heart rate, using Atropin 0.5-1 mg intravenously. If not effective, plasma volume should be increased by infusion (for example, glucose solution, physiological salts or dextran). The sympathetic drugs that have the most dominant effect on the Alpha-1 receptor can be used if the mentioned measures are not effective enough.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.
Side Effects
When using Plendil, you may experience unwanted effects (ADR).
Common, ADR> 1/100
less common, 1/1000 Most of the red reactions, headache, chest drums, dizziness and fatigue are depending on the dose and occur at the beginning of treatment or after increasing the dose. When it occurs, these reactions often fleeting and decreasing over time. Experience from clinical studies shows that 2% of patients have to stop treatment due to ankle swelling. Cases of gum obesity, can avoid gums or fix it by careful oral hygiene. When experiencing side effects of serious drugs, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
Felodipine drugs contraindicated in the following cases:
Be cautious when used
The efficiency and safety of felodipine in the treatment of unpretated emergency hypertension. Felodipine can hyp up significantly with tachycardia later. This can lead to myocardial anemia in sensitive patients.
Note the simultaneous use of stimulants or strong inhibits CYP3A4 enzymes leads to a significant reduction in plasma felodipine levels.
Plendil contains lactose. Patients with rare genetic problems about galactose tolerance or malposure-galactose absorption should not be taken. Plendil contains castor oil, so it can cause stomach pain and diarrhea.
The ability to drive and operate machinery
because dizziness and fatigue may occur related to treatment with felodipine, this should be considered when there is a high concentration, for example, when driving or operating machinery
Pregnancy
Contraindicated use of felodipine during pregnancy.
Breastfeeding period
Felodipine is excreted through breast milk. If women who are breastfeeding have felodipine treatments, only a very small dose is transmitted through breast milk to the baby. There is no enough experience in treating felodipine during breastfeeding to assess risks for children. Therefore, Felodipine should not be used during breastfeeding. In the case of medical treatment benefits is considered to be more important, safety measures should be stopped breastfeeding.
Interactive drug
simultaneously used with inhibitors or CYP3A4 inhibitors or induction may affect felodipine levels in plasma.
CYP3A4 induction drugs
The drug increases the metabolism of felodipine by touching P450, such as: Carbamazepine, Phenytoin, Barbiturate drugs such as Phenobarbital, Rifampicin, as well as St. John (Hypericum Perforatum). When used with carbamazepine, phenytoin and phenobarbital, the concentration below the AUC curve of Felodipine has decreased by 93% and CMAX decreased by 82%. Avoid coordinating with CYP 3A4 induction drugs.
CYP3A4 inhibitors
Strong CYP 3A4 inhibitors such as Azole antifungal drugs ( ITraconazole , ketoconazole ) and Macrolid antibiotics (erythromycin) and enzyme inhibitors of HIV virus. To avoid coordination with strong CYP 3A4 inhibitors.
Grapefruit juice inhibits CYP 3A4. Should avoid taking medicine with grapefruit juice.
cimetidine : simultaneous use of felodipine with cimetidine increases cmax and auc of felodipine about 55%.
Other interactions
Felodipine may increase the concentration of Tacrolimus. When used simultaneously, the tacrolimus concentration should be monitored and may need to adjust the dose.
ciclosporin : simultaneously use felodipine with ciclosporin, making felodipine cmax increased by 150% and AUC increased by about 60%. However, the effect of felodipine on the pharmacokinetics of ciclosporin is limited.
Other drugs are strongly linked to protein: The strong level of felodipine with plasma proteins does not affect the non -connected parts of other drugs that are also capable of strongly connected with protein such as warfarin.
Storage
Do not store over 300C. Avoid light and avoid moisture.
To be out of reach of children.
