    Pharmacy

Pharmacological group: Coordination of penicillin group, including beta-lactamase inhibitors.

    ATC code: J01CR02.

Amoxicillin is a semi -synthetic antibiotic with wide antibacterial spectrum against many Gram -positive and gram -negative bacteria. However, amoxicillin is easily decomposed by beta-lactamase enzymes, so the effect of Amoxicillin is used alone does not include these bacteria.

Clavulanic acid is a beta-lactam, structurally related to penicillin, capable of inactiveness of many types of beta-lactamase yeast commonly seen in resistance to penicillin and cephalosporin. In particular, clavulanic acid works well against beta-lactamase specified by plasmids that often cause propagation resistance. Clavulanic acid often affects the Beta-lactamase type 1 specified by chromosomes.

The presence of clavulanic acid protects amoxicillin from decomposition by beta-lactamase enzymes, thus expanding amoxicillin antibacterial spectrum includes many common bacteria resistant to amoxicillin and other cephalosporins. Therefore, combination of amoxicillin and clavulanic acid has a special feature of a broad-spectrum antibiotic and a beta-lactamase inhibitor.

    Bacterial spectrum:

    common sensitive bacteria

  • Aerobic gram -positive bacteria: Bacillus anthracis, Enterococcus Faecalis, Listeria monocytogenes, Nocardia Asteroides, Streptococcus Pyogenes, Streptococcus Agalactiae, Streptococcus SP With methicillin), Staphylococcus saprophyticus (methicillin sensitive), bacteria without coagulase (methicilin sensitive).
  • Aerobic gram -negative bacteria: Bordetella Pertussis, Haemophilus influenzae, Haemophilus parainfluenzae, Helicobacter pylori, moraxella catarrhalis, neisseria gonorrhoeae, pasturella multocida, vibrio cholaee.
  • Aerobic grams: Clostridium spp.
  • Gram of anaerobic sound: Bacteroides fragilis, bacteroides spp., Capnocytophaga spp.

  • Other: Borrelia Burgdorferi, Leptospira ITERTOHAemorrhagiae, Treponema Pallidum.
  • Bacteria whose antibod antites can be a problem

  • Aerobic Gram -negative bacteria: Escherichia Coli, Klebsiella Oxytoca, Klebsiella Pneumoniae, Klebsiella

Aerobic gram -positive bacteria: corynebacterium spp.

  • Bacteria that have been resistant to drugs

  • Aerobic gramy bacteria: Acinetobacter spp., Citrobacter Freundii, Entobacter spp. Yersinia Enterolitica.
  • Other: Chlamydia Pneumoniae, Chlamydia Psittaci, Chlamydia spp., Coxiella Burnetii, Mycoplasma spp.
  • Mechanism of resistance:

    Two MAIN BETA-LACTAM antibiotic antibiotics:

The drug is inactivated by the bacterial beta-lactamase enzyme secreted and not inhibited by clavulanic acid, including groups B, C and D.

Change PBP reduces antibiotic affection for the goal.

In addition, the change of bacterial permeability or the mechanism of pumps to push out the pump also contributes to the resistance of bacteria.

    pharmacokinetic

    absorption

Amoxicillin and clavulanic acid are both absorbed quickly and well through oral. The drug is optimally absorbed when used before meals. Oral bioavailability of amoxicillin and clavulanic acid is about 70%. Amoxicillin and clavulanic acid achieve maximum concentration in serum after 1 hour.

Clavulanic acid does not affect amoxicillin pharmacokinetics, but amoxicillin can increase the absorption through the gastrointestinal tract and eliminate the kidneys of clavulanic acid compared to when using lone Clavulanic acid.

    Distribution

Clavulanic acid is connected to plasma proteins about 25% and amoxicillin associated with protein about 18%. The distribution of amoxicillin is about 0.3 - 0.4 liters/kg and about 0.2 liters/kg for clavulanic acid.

Amoxicillin and clavulanic acid are all distributed into the lungs, pleural fluid and peritoneal fluid, passing through the placenta. A small amount of pills found in sputum, saliva as well as in breast milk. When the meninges are not inflamed, the concentration of the medication in the cerebrospinal fluid is very low, but the drug concentration is higher when the meninges are inflamed.

    Metabolism

Amoxicillin is partially converted into penicilloic acid in the form of inactiveness in urine with an equivalent of 10-25%) of the starting dose. Clavulanic acid is strongly metabolized in the human body, then eliminated through urine, feces and in the form of CO2 in exhaled gas.

    Elimination

Amoxicillin is excreted mainly through the kidneys. Clavulanic acid excreted through the kidney and outside the kidneys.

The average selling time of amoxicillin and clavulanic acid is about 1 hour and the total clearance coefficient is about 25 liters/hour at a healthy person. Different studies indicate that urine excretion is 50 - 85% for amoxicillin and about 27-60% for clavulanic acid for 24 hours. Clavulanic acid excreted most in the first 2 hours after taking the drug.

both amoxicillin and clavulanic acid are removed when hemolysis. When abdominal distances, clavulanic acid is removed while only a very small amount of amoxicillin is removed.

Probenecid extends the excretion time of amoxicillin but does not affect the elimination of clavulanic acid at the kidney.

    Premature babies

There is no recommended dose for premature babies.

    Elderly

No dose adjustment; Use the dose like adults. If there are signs of renal failure, the dose should be adjusted by patients with renal impairment.

    Renal failure

Adjust the dose according to the level of renal failure:

    Renal failure

    dose adjustment

    No dose adjustment

Do not use Claminat 500mg/62.5mg due to the ratio of amoxicillin, clavulanic acid 8: 1 without recommendations on the dose adjustment

Claminat 500mg/62.5mg powdered powder for oral fluid should only be used for patients with creatinine clearance higher than 30 ml/min.

    People with liver failure

Be cautious when prescribing, should periodically check liver function.

Not enough data to give recommended dose.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

The treatment time is determined based on the patient's reaction. Some infections (such as osteomyelitis) need longer treatment time. Do not extend the treatment time more than 14 days without re -assessing the patient's condition.

What to do when overdose? Some patients have rash, agitation or drowsiness.

Convulsions can occur in patients with impaired renal function or high doses.

Primary crystal causes kidney failure that occurs in some patients using overdose amoxicillin .

    How to handle:

Stop using drugs, symptomatic treatment and supportive measures if needed.

Pay attention to water and electrolyte balance to maintain the urinary and reduce the risk of crystalline urination.

It is possible to remove amoxicillin and clavulanic acid from circulation by blood decomposition method.

    Children

Overdose below 250mg/kg does not come with significant clinical symptoms and no need to clean the stomach.

What to do when forgetting a dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

If you forget to use the medication on time, do not use the dosage at a late day, should continue to use the normal dose the next day in the course.

    Side Effects

When using Claminat 500mg/62.5mg, you may experience unwanted effects (ADR).

    Very popular (ADR ≥ 1/10)

  • Digestive system: diarrhea.
  • Popular (1/100 ≤ ADR Other: Candida fungus on the skin and mucosa.

  • The digestive system: nausea, vomiting
  • Nervous system: headache , dizziness.
  • Digestive system: indigestion. ADR
  • Blood and lymphatic system: Restoration of leukemia (including neutropenia) and thrombocytopenia.
  • Other: superinfection. Extend bleeding time and prothrombin time. Convulsions may appear in patients with impaired renal function or high doses. Good dental hygiene can prevent tooth color changes because this symptom can be removed by brushing. With Eosinophilia and Systemic Symptoms - Dress).

If allergic reactions occur such as erythema, edema, anaphylactic shock, Stevens -Johnson syndrome, must be stopped, immediately treated emergency treatment with Adrenalin, oxygen breathing, intravenous corticoid therapy and ventilation, including internal intubation and never being treated with drugs containing antibiotic components of penicilin or cephalosporin.

    Handling when fake colitis:

    Mild: Stop drugs.

Heavy (possibilities due to Clostridium Difficile): Water and electrolyte, anti -Clostridium antibiotics (Metronidazol, Vancomycin).

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

  • Warnings

Before using Claminat 500mg/62.5mg, you need to read the user manual carefully and refer to the information below.

    Contraindicated

Claminat medicine 500mg/62.5mg contraindicated in the following cases:

  • Patients with amoxicillin sensitivity, clavulanic acid, penicillin antibiotics or any ingredients of the drug.
  • Patients with a history of serious hypersensitivity (eg anaphylaxis) with other drugs belonging to the beta-lactam group (such as cephalosporin, carbapenem or monobactam).

Patients with a history of jaundice or liver dysfunction related to amoxicillin and clavulanic acid.

    Precautions when using

Before starting treatment with Claminat 500mg/

62.5mg, need to learn about the patient's allergic history with penicillin, cephalosporin or other beta-lactam drugs.

Patients treated with penicillin may have severe hypersensitivity reactions and sometimes fatal (anaphylaxis). These reactions are likely to occur in patients with a history of penicillin sensitivity and patients with allergies. If allergic reactions occur, medication must be discontinued and have appropriate alternative treatments.

In the case of Beta-lactamase enzymes secreted by bacteria, which is not inhibited by clavulanic acid, which leads to resistance to bacteria with beta-lactam antibiotics, does not use Claminat 500mg/62.5mg for treatment. Do not use this medication to treat S.pneumoniae resistant to penicillin.

Convulsions can occur in patients with impaired renal function or patients using high doses.

It is necessary to avoid Claminat 500mg/62.5mg for patients with mononoculatoculus hyperlemen due to the high risk of rash in these patients if they use amoxicillin. Using allopurinol while being treated with amoxicillin may increase the risk of skin allergies.

Take the drug that lasts sometimes causes excessive growth of non -sensitive bacteria.

The appearance of all -body typhus with pustules at the beginning of the treatment may be a symptom of the acute all -body pustules syndrome (AGEP). When there are signs, patients need to stop taking the drug and do not use amoxicillin -containing drugs to treat the disease in later cases.

Be cautious when using Claminat 500mg/62.5mg for patients with impaired liver function. Cases of liver occur mainly in male patients, elderly patients and prolonged and rare patients in children. Typically, signs and symptoms usually occur during the process or after a short time of treatment, but in some cases, it may not be clear until several weeks after stopping treatment. These symptoms can often recover from themselves, but there are still cases of severe, even death, but very rare, often occurs in patients with serious illness or use in combination with drugs that affect the liver.

Colitis caused by antibiotics occurs with most antibiotics, including amoxicillin, the level of mild to life -threatening fluctuations. Therefore, this diagnosis should be noted in patients with diarrhea during or after any antibiotic use. If antibiotic colitis occurs, stop taking the drug immediately and have appropriate treatment. Contraindicated anti -spasms in this case.

periodically evaluate the function of organ systems, including liver, kidney function and hematopoietic system during prolonged treatment.

Increasing blood clotting time may occur and rarely in patients using amoxicillin and clavulanic acid. Patient status should be monitored when used simultaneously with anticoagulant drugs and adjust the dose of appropriate anticoagulants.

Little urine patients may experience crystal but rare cases, mainly occur when taking injection drugs. When using high doses of amoxicillin, it is necessary to balance the amount of drinking water and release to reduce the ability to form amoxicillin crystals. The bladder pipes must be checked regularly to avoid precipitation causing clogging when there is a high concentration of preparations in the urine.

Patients with urinary phenylceton and patients need to control phenylalanin to be cautious when taking Claminat 500mg/62.5mg because the drug contains aspartame will be converted in the digestive tract into phenylalanin.

Claminat 500mg/62.5mg contains manitol that can cause light laxative.

While treating amoxicillin, the glucose oxidase enzyme should be used when it is necessary to test glucose in the urine because non -enzyme methods can give false positive results.

Clavulanic acid causes nonsense cohesion of IgG and albumin on the red blood cell membrane, resulting in a false positive result of the Coombs test.

The drug can cause false positive testing Bio-Rad Laboratories Platelia Aspergillus EIA due to cross-reactions with polysaccharide and polyfuranose are not Aspergillus. Therefore, for patients who are taking amoxicillin and clavulanic acid, should be cautious when reading the results if there is a positive result when tested by this method and should be redefined by other diagnostic methods.

The ability to drive and operate machinery

There is no adequate research to prove that the drug affects the ability to drive and operate machinery. However, unwanted effects such as allergies, dizziness, convulsions, ... can reduce the ability to focus and react of the patient, therefore, should be careful when driving and operating machinery if using the drug.

    Pregnancy

    Use drugs for pregnant women:

Claminat medicine 500mg/62.5mg does not cause congenital deformities. In premature women, early rupture of amniotic fluid, the drug increases the risk of necrotizing intestinal inflammation in newborns. Therefore, avoiding drugs during pregnancy, unless the doctor is indicated.

    Breastfeeding period

amoxicillin and clavulanic acid are both breast milk. Therefore, diarrhea and fungal infections on mucous membranes can occur in infant breastfeeding, need to stop breastfeeding when babies have the above manifestations. Only use the drug during breastfeeding after evaluating benefits and risks from a doctor.

    Medicinal interaction

Oral anticoagulant drugs: combination of oral anticoagulants and penicillin antibiotics are widely used in practice without reporting on interaction.

However, there have been cases of increased international normalization index in patients using acenocoumarol or warfarin in combination with amoxicillin. In case of simultaneous use, prothrombin should be monitored or international chemical index and adjust the anticoagulant dose when needed.

methotrexate: Amoxicillin reduces methotrexate secretion, increases the risk of toxicity.

Probenecid : reduces amoxicillin secretion in the renal tubules and can increase, prolong the concentration of amoxicillin in the blood but do not increase clavulanic acid. Therefore, do not use the drug simultaneously with Probenecid.

    nifedipin : Increases amoxicillin absorption.

Oral contraceptive pills: like antibiotics with broad -effects, the drug can reduce the effectiveness of oral contraceptives, so it is necessary to notify the patient in advance.

Allopurinol: Patients with hyperuricemia when using simultaneously Allopurinol along with amoxicillin will increase the ability of amoxicillin.

Bacterial substances: There may be antagonists between amoxicilin and bactericidal substances such as fusidic acid, chloramphenicol , tetracyclin.

    affect clinical tests:

  • While treating amoxicillin, the glucose oxidase enzyme should be used when the glucose test is required in the urine because the methods of non -enzyme can produce false positive results.
  • Clavulanic acid causes an nonsense cohesion of IgG and albumin on the membrane of red blood cells, resulting in a false positive result of the Coombs test.
  • The drug can cause false positive testing Bio-Rad Laboratories Platelia Aspergillus EIA due to cross-reactions with polysaccharide and polyfuranose are not Aspergillus. Therefore, for patients who are taking amoxicillin and clavulanic acid, should be cautious when reading the results if there is a positive result when tested by this method and should be redefined by other diagnostic methods.
