Indications

Protevir drugs are indicated in the following cases:

Treatment of chronic hepatitis B in adults with liver disease is still compensated and there is evidence of virus copying and histological evidence of active or fibrous inflammation. Fumarate has a nucleotide diester structure similar to Adenosine monophosphate, which is a precursor of Tenofovir. After being phosphoryl turned into an active diphosphate form, Tenofovir Diphosphate is resistant to HBV's polymerase and HIV -reverse code star. Tenofovir diphosphate inhibits the polymerase of the virus (reverse code star) by competing directly with the natural substrate Deoxyribonucleotide and finish the DNA chain after attaching to the DNA chain of the virus.

Antiviral activity: Tenofovir's EC50 is about 0.04 - 8.5μm. With HIV-1 strain group A, B, C, D, E, F, G and O, EC50 is about 0.5-2.2μm. With HIV-2 strains, EC50 is about 1.6-5μm.

Distribution

Dosage

Hepatitis B in adults aged 18 and older with normal kidney function

Renal failure

Tenofovir is eliminated through the excretion of the kidneys, so the Tenofovir concentration increases in patients with renal impairment. Dosage adjustment for patients with creatinine clearance

CLCR ≥ 50ml/minute: recommended dose 300mg every 24 hours.

CLCR 30 - 49ml/minute: recommended dose 300mg every 48 hours.

CLCR 10 - 29ml/minute: recommended dose 300mg every 72 - 96 hours.

Hematopathy: The recommended dose of 300mg every 7 days or after the total hemorrhage time is nearly 12 hours.

Hepatic impairment

There is no need to adjust the dose in the liver failure.

There is no remarkable interaction when using Tenofovir Disoproxil Fumarate with Entecavir, Emtricitabine, Lamivudine, Indinavir, Efavirenz, Nelfinavir, Saquinavir, Methadone, Ribavirin, Rifampicin, Tacrolimus Norgestimate/Ethinyloestradiol.

The drug must be taken at full because the food increases the bioavailability of Tenofovir.

Storage

Keep the drug in a cool dry place, temperatures below 30ºC. Avoid light.
