Uses
Indications
Pariet® 10 mg pills are indicated in the following cases:
Rabeprazole sodium belongs to the acid -secreting drug group, the derivatives of benzimidazoles, there is no anti -anti -secret properties or antihistamine H2, but inhibit acid secretion in the stomach by inhibiting the enzyme enzyme H+/K+ -Aatpase (acid pump or proton pump).
Acid secretion effect
After taking a dose of 20 mg Rabeprazole sodium on the onset of acid secretion effect within an hour, with the maximum efficiency between 2 and 4 hours.
23 hours after the first Rabeprazole sodium dose, the basic acid secretion is 69% and the inhibitor of acid secretion caused by food is 82% and the inhibitor time lasts up to 48 hours.
The acid secretion effect of Rabeprazole Sodium increases slightly when the daily dose is repeated, reaching stable inhibition after three days.
When stopping the drug, normal acid secretion activity after 2 to 3 days.
influence on serum gastrin
In clinical studies, patients are treated with 10 or 20 mg Rabeprazole sodium once a day for a period of 43 months. Serum galastin levels increased gradually in the first 2 to 8 weeks that reflect the inhibitory effect on acid secretion and maintain stability when continuing treatment. Gastrin concentration returns to the level before treatment 1 to 2 weeks.
Other effects
So far has not seen the system effect of Rabeprazole Sodium on the central nervous system, the cardiovascular system and respiratory system.
pharmacokinetic
absorption
Pariet® 10 mg is a Rabeprazole sodium preparation in the form of tablets in the intestine (insoluble in the stomach). This form of preparation is necessary because Rabeprazole is not durable in the acidic environment. Therefore, the absorption of Rabeprazole only occurs after the pills leave the stomach.
Fast absorption with peaks of rabeprazole in plasma is about 3.5 hours after a dose of 20 mg.
In healthy people, the semi -cancellation time is in plasma, about an hour (from 0.7 to 1.5 hours) and the body clearance is 283 98 ml/min.
There is no clinical interaction with food. Food as well as the time of medication during the day do not affect the absorption of Rabeprazole Sodium.
Distribution
In humans, 97% Rabeprazole is connected to plasma proteins.
Metabolism and output
Rabeprazole sodium, as well as other drugs in the Proton pump inhibitor (PPI), are metabolized through the liver's liver's P450 (CYP450) cytochrome P450 (CYP450) system.
About 90% of the dose is exported in urine in two metabolic forms: the form of Mercapturic Acid (MF) and the form of carboxylic acid (MF) and two other unknown metabolites.
The rest of the dose is found in the feces.

How to use
Take oral use. For the instructions of the drug once a day, Pariet® 10 mg tablets should be taken in the morning, before eating and although the time of the day as well as the food does not affect the effects of Rabeprazole sodium, the use of drugs in such morning makes it convenient for compliance with treatment. Patients should be noted that they should not chew or crush tablets, but should swallow whole tablets.
Dosage
adults/the elderly:
Active duodenal ulcer, benign gastric ulcer and mouth ulcer: The proposed oral dose is pariet 10mg or 20mg used once a day in the morning.
Most duodenal ulcerative patients will heal for four weeks. However, some patients need to treat four more weeks to heal. Most benign stomach ulcer patients will heal for six weeks. However, as above, some patients need to treat six more weeks to heal.
Gastroesophageal reflux disease or erosion (GERD): The recommended oral dose for this condition is Pariet 10mg or 20mg oral once a day for four to eight weeks. The dose of Pariet 10mg or 20mg Take oral twice daily for another 8 weeks when the Proton (PPI) inhibitors inhibitors are not effective for patients with reflux esophagitis patients. However, the dose of Pariet 20mg Take oral twice daily should only be applied to patients with serious mucosa damage.
Long -term maintenance treatment of gastroesophageal reflux disease: In long -term treatment, the maintenance dose of pariets 10mg or 20mg can be used once a day depending on the response of the patient.
Treatment of symptoms in gastroesophageal reflux disease from average to very severe: Pariet 10mg once a day for patients without esophagitis. If symptoms do not improve after four weeks, patients need further examination. When the symptoms disappear, the symptoms can be controlled later by the use of the drug on demand with the dose of pariet 10mg once a day when necessary.
Zollinger-Eleson syndrome and other pathological increased conditions: The dosage needs to vary depending on the patient. Start with the dose of Rabeprazole Sodium 60mg a day, then may increase to 100mg once a day or 60mg twice daily. Some patients need to be divided. It is necessary to continue taking the drug until the clinical is still necessary. Some patients with Zollinger-Ellison syndrome have been treated continuously for a year.
H.pylori except: H.pylori -infected patients should be treated with an exorcic therapy. Combination treatment for 7 days is proposed as follows:
Pariet 20mg twice daily + Clarithromycin 500mg twice daily and amoxicillin 1g twice daily.
For the instructions of the medication once a day, the tablet pariet should be taken in the morning, before eating; And although the time of the day as well as the food has no effect on the effects of Rabeprazole Sodium, the use of the drug in such morning makes it convenient for compliance with treatment.
Patients should be noted that they should not be chewed or crushed to the tablet, but should swallow the whole tablet.
kidney failure and liver failure:
No dose adjustment for patients with renal failure or liver failure.
Please refer to the special and cautious item especially for the use of pariets in the treatment of patients with severe liver failure.
Children:
Safety and effectiveness of Rabeprazole Sodium 20mg in short -term treatment GERD (up to 8 weeks) in teenagers aged 12 and over is supported by:
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Symptoms
So far is very rare for intentional or random overdose. The maximum dose does not exceed 60 mg twice daily, or 160 mg once a day. These effects recorded very little and can recover without any other medical intervention.
Handling
There is no specific antidote. Rabeprazole sodium is bound to protein and therefore not easy to separate. As in other cases of overdose, symptomatic treatment should be applied and applied measures to raise the total weight.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Pariet® 10 mg drug , you may experience unwanted effects (ADR).
generally Pariet® 10 mg of tablets are well tolerated in clinical trials in adults and adolescents. The most common side effects throughout the controlled clinical trials with Rabeprazole are headache, diarrhea, abdominal pain, weakness, flatulence, rash and dry mouth. Unwanted effects are often fleeting and light or medium suitable between adults and adolescents. The following side effects have been recorded on clinical trials and reports after widely used in the market.
Common: 1/100 Mental disorders: Insomnia. Respiratory disorders, chest and mediastinum: cough, sore throat, rhinitis. Uncommon: 1/1000 Respiratory disorders, chest and mediastinum: bronchitis, sinusitis. Gastrointestinal disorders : indigestion, dry mouth, belching. Kidney and urinary disorders: Urinary infection. Rare: 1/10,000 Immune system disorders: allergic reactions. Mental disorders: depression. Very rare: ADR Not determined frequency Bloody disorders: peripheral edema. Instructions on how to handle ADR Notify the doctor with unwanted effects when using the drug.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Pariet® 10 mg drug is contraindicated in cases where patients are hypersensitively to Rabeprazole sodium, benzimizazoles derivatives or excipients in the formula.
Be cautious when using
Improving symptoms through Rabeprazole Sodium treatment does not exclude the presence of stomach or esophagus, so it is necessary to eliminate malignant possibility before starting treatment with pariet® 10 mg.
Patients with long -term treatment (special treatment for more than a year) need to be tested regularly. Patients should be noted that it should not be chewed or crushed Pariet® 10 mg of tablets, but should swallow the whole tablet.
Pariet® 10 mg is not recommended for children under 12 years old, because there is no experience in using Rabeprazole in this age group.
In a study in patients with mild to medium liver function impairment compared to the normal and equivalent group of age and gender, there is no significant evidence of safety related to Rabeprazole. However, because there is no clinical data for the use of Pariet® 10 mg in the treatment of patients with severe liver dysfunction, the doctor is carefully advised to start treatment with Pariet® 10 mg in this group.
Hypoglycemia with symptoms with millions have been reported as rare in patients treated with PPI for at least 3 months, in most cases after 1 year of treatment. Serious side effects include spasms, arrhythmia and epilepsy. In most patients, hyperglycemia treatment by supplementing magnesi and stopping using PPI.
For patients expected to be treated for a long time or using ppi with drugs such as digoxin or drugs that can cause blood magnesia (for example, pepper pills), health workers may consider monitor monitoring Magnesi levels before starting treatment with PPI and periodically. Observing research shows that Proton pump inhibitors (PPI) can be associated with an increase in the risk of osteoporosis related to hip, wrist or spine fractures. The risk of increased fractures in patients with high doses and long -term treatment with PPI (one year or longer).
Documents show that simultaneous use of ppi with methotrexate (mainly at high doses, viewing the prescription information about methotrexate) may increase and prolong its concentration of methotrexate and/or its metabolites, which can lead to toxicity of methotrexate. When using high doses of methotrexate, temporary death can be considered in some patients. Treatment with proton pump inhibitors may increase the risk of gastrointestinal infections such as Clostridium difficile.
The ability to drive and operate machinery
Based on the pharmacological properties and reports on side effects show that Pariet® 10 mg does not reduce the ability to drive or use machines. However, if sleepy reduces sensitivity, avoid driving or operating complex machinery.
Pregnancy
There is no complete or well -controlled research on pregnant women and experience after the drug is used on the market is limited. Only use Rabeprazol sodium in pregnant women when the potential benefits are more than the risk that may occur for the fetus.
Breastfeeding period
It is not clear whether Rabeprazole sodium is excreted in milk or not and no studies in nursing women. However, Rabeprazole Sodium has excreted in mouse milk. Therefore, rabeprazole sodium should not be used in breastfeeding women. If it is required to use Rabeprazole Sodium, you must stop breastfeeding.
Medicinal interaction
Rabeprazole sodium causes the acidic inhibition of strong and prolonged stomach acid. The interaction with drugs may occur depending on the pH. The simultaneous use of Rabeprazole sodium with ketoconazole or iTraconazole may significantly reduce antifungal concentration in plasma. Therefore, it is necessary to monitor each individual case to determine whether or not to adjust the dose when used simultaneously ketoconazole or iTraconazole with pariet® 10 mg or not.
In clinical trials, antacidants are simultaneously used with Pariet® 10 mg and in a special drug interactive study shows that Rabeprazole does not interact with liquid acid antacidants.
Simultaneous use of Atazanavir 300 mg/ritonavir 100 mg with omeprazole (40 mg once daily) or Atazanavir 400 mg with lansoprazole (60 mg once daily) for healthy volunteers leads to significant reduction of acanavir levels. The absorption of Atazanavir depends on the pH. Although simultaneously used with Rabeprazole has not been studied, it is expected that the result is similar to other proton inhibitors. Therefore, PPIs, including Rabeprazole, should not be used simultaneously with Atazanavir.
Published reports, pharmacokinetic research and rescue analysis show that simultaneous use of ppi and methotrexate may increase and prolong methotrexate levels and active substances metabolic substance hydroxymethotrexate. However, no official research on drug interaction of methotrexate with PPI has been conducted.
