Uses

Indications

Remoclar 500 pills are indicated in the following cases:

  • Remoclar is indicated to treat infections caused by sensitive strains. Remoclar is also indicated to kill H. pylori in patients with peptic ulcer.
  • Pharmacokological study

Clarithromycin is the semi -synthetic substance of erythromycin A. Anti -bacteria by attaching to the ribosom 50s of sensitive bacteria and inhibiting protein synthesis. A strong antibiotic antibiotic against many G-- GR+ aerobic and anaerobic bacteria. The minimum inhibitory concentration (MICS) of Clarithromycin is generally 2 times lower than that of Erythromycin's microphone. Clarithromycin's 14-hydroxy metabolism also has antibacterial activity. The microphone of this type of metabolism is equivalent to or 2 times higher than the mother subtract to H. Influenzae (14-hydroxy metabolites have twice as strong as the mother).

On In vitro, Remeclar usually works on the following bacteria strains:

GR+: Staphylococcus aureus (methicillin sensitive); Streptococcus Pyogenes (Beta blood soluble streptococcus group A), Alpha-Hemolytic Streptococci (Viridans Group); Streptococcus (diplococcus) pneumoniae; Streptococcus Agalactiae; Listeria monocytogenes.

GR- bacteria: Haemophilus influenzae, Haemophilus parainfluenzae, Moraxella (Branhamella) Catrhalis, Neisseria Gonorrhoeae; Legionella Pneumophila, Bordetella Pertussis, Helicobacter Pylori; Campylobacter Jejuni.

    Mycoplasma: Mycoplasma pneumoniae; Ureaplasma Urealyticum.

Other strains: Chlamydia trachomatis; Mycobacterium avium; Mycobacterium leprae.

Anaerobic bacteria: Bacteroides Fragilis sensitive to macrolide; Clostridium perfringens; Peptococcus species; Peptostreptococcus species; Propionibacterium acnes.

Clarithromycin has bactericidal effects on some bacterial strains such as: Haemophilus influenzae, Streptococcus pneumoniae, Streptococcus pyogenes, Streptococcus agalactiae, Moraxella (Branhamella) CatVrhalis, Neisseria Gonorrhoeea, H. Pylorae, H. Pylorae and H. Pylorie Campylobacter spp. The anti -pylori effect of clarithromycin in the neutral pH environment is greater than the acid pH environment.

    pharmacokinetic

Clarithromycin is absorbed quickly and well through the digestive tract after drinking. Metabolic agent 14-hydroxyclarithromycin, after metabolism, still acts on bacteria. The time to use Remeclar does not depend on the meal because the food does not affect the bioavailability of the remeclar tablet. Food slows down the initial absorption and metabolism formed into a 14-hydroxy metabolic form of clarithromycin. The pharmacokinetics of Clarithromycin are not linear; Stable state achieved within 2 days of medication. With a dose of 250 mg 2 times/day, 15-20% of the drug is in the form of the urine. With a dose of 250 mg x 2 times/day daily use, the amount of drug excreted in urine is larger (about 36%). 14 -Hydroxyclarithromycin is the main form of metabolism found in urine and accounts for about 10-15% of the dose. Most of the remaining parts are eliminated through feces, mainly through bile. 5 - 10% of original pure drugs were found in the feces.

When using Clarithromycin 250 mg 3 times/day, the clarithromycin concentration in plasma increases corresponding to the dose of 250 mg x 2 times/day.

The remoclar concentration is achieved in a few times higher than the concentration of drugs in the blood. Increased concentration is also seen in tonsils and lung tissue. At the treatment dose, 80% Clarithromycin binds to plasma proteins.

Remeclar penetrates into the stomach mucous membrane. Clarithromycin concentration in stomach mucous membranes and in the stomach tissue when used in combination with clarithromycin with omeprazole is higher when using only clarithromycin.
