Uses

indicated

Ringer Lactate Transmission is indicated in the following cases:

  • Used in the case of severe dehydration (mainly foreign dehydration) cannot be taken by oral (patients with coma, drinking immediately, vomiting). Learning

ringer lactate is the isometric solution of electrolytes. The composition and concentration of the drug are suitable for plasma.

The pharmacological properties of the drug are prescribed by the ingredients of the drug (water, sodium, potassium, calcium, lactat and chloride). The main effect of Ringer Lactat is to increase the volume of extracellular compartments, including interstitial compartments and circuit fluids.

lactate is converted into bicarbonate mainly in the liver and has a plasma alkalization effect.

ions such as sodium, circulating through cell membranes using different transport mechanisms, among them are sodium pumps (Na+/k+ - atpase). Sodium plays an important role in nerve and heart physiology and even in its kidney metabolism.

Potassium is essential for many metabolic and physiological processes including neurotransmitter, muscle contraction and acid -base balance. Normal potassium concentration in plasma is about 3.5 - 5.0 mmol/l. Potassium is an intracellular cation, mainly found in muscles, only about 2% are present in extracellular fluid. Potassium into cells and retain the concentration requires transportation through Na+/K+ - ATPase enzyme.

About 99% of calcium is inserted into the bone, the remaining 1% is found in tissues and body fluids, and essential for normal neurotransmitter, muscle and coagulation activity.

chloride is an extracellular anion found at low concentrations in bones and at high concentrations in some components of connective tissue such as collagen. The intracellular chloride has high concentrations in red blood cells and gastric mucosa. The balance of anion and cation is adjusted by the kidneys. Chloride reabsorption is usually followed by sodium reabsorption.

    pharmacokinetics

    Distribution

volume and ion composition of extracellular compartments and intracellular compartments:

    extracellular compartments:

  • about 19 liters.
  • sodium (mmol/l): 142; Potassium (mmol/l): 5; Calcium (mmol/l): 2.5; Chloride (mmol/l): 103.
  • intracellular compartment:

  • about 23 liters.
  • Sodium (mmol/l): 15; Potassium (mmol/l): 150; Calcium (mmol/l): 1; Chloride (mmol/l): 1. Sodium slowly distributed into red blood cells and nerve cells; Sodium is very slowly distributed into the bone.

    Elimination

Sodium is mainly excreted through the kidneys, but there is a large reabsorption in the kidneys. A small amount of sodium is eliminated in feces and sweat.

  • Before taking Ringer Lactate solution introphy and electrolyte (20 bottles x 500ml)

    How to use

    intravenous infusion.

It is necessary to check the sensory solution before transmission.

Use only if the solution is clear, there are no visible sub -stools and the packaging is not damaged. Immediately transmitted after connecting the bottle to the transmission line.

Do not use plastic bottles in serial transmission. Such use can cause gas embolism due to excess air is drawn from the first bottle before completing the infusion from the second bottle.

The pressing of the transmission contained in plastic bottles to increase the flow rate may cause embolism if the residual air in the bottle is not completely pushed out before transmission.

Using intravenous lines with ventilation holes with catheter at an open position can cause gas embolism. The intravenous line set with ventilation holes with pine holes in the open position should not be used with plastic bottles.

    Dosage

The number and the speed of infusion depend on clinical and biochemical symptoms (electrolytes - map, hematocrite, urine amount ...).

Treatment of severe dehydrated diarrhea in children

Can be recommended by the World Health Organization:

Intravenous infusion immediately, at first 30 ml/kg for 1 hour (children under 12 months old) or 30 minutes (children over 12 months to 5 years old), then 70 ml/kg for 5 hours (children under 12 months) or 2 hours 30 minutes (children over 12 months to 5 years old).

1-2 hours to re -evaluate the condition of the patient.

Dengue shock treatment (level III and IV)

20 ml/kg for 1 hour, then re -evaluate the condition of the patient.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when using overdose?

Severe: acute pulmonary edema, acute heart failure.

If you see edema under the skin, especially shortness of breath, stop transmitting immediately. For appropriate treatment (intravenous diuretics, oxygen breathing ...).

In an emergency, call the 115 emergency center immediately or go to the nearest local health station.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
