Esomeprazole is the isomorphic form of omeprazole, proton pump inhibitors. Esomeprazole prevents stomach acid secretion by inhibiting specific H +/K +- ATPase in the gastric cell wall. Because of the special impact on proton pump inhibition, Esomeprazole prevents the final level of acidic excretion, thus reducing the acidity in the stomach.

    pharmacokinetic

    absorption

After taking the peak concentration in plasma (CMAX) achieved after about 1.5 hours (TMAX). CMAX increases proportional to increase. The area under the curve (AUC) increased 3 times when using the dose of 20 to 40 mg. At the daily dose of 40 mg daily, the whole body uses about 90% when compared to the single dose of 40 mg is 64%.

The area under the curve (AUC) after taking a single -dose of 40 mg Esomeprazole decreases 43 - 53% after use with food when compared to fasting conditions. Esomeprazole should be used for at least 1 hour before meals. The food slows down and reduces the absorption of Esomeprazole, but this does not significantly change its effects and stomach acid secretion.

    Distribution

Esomeprazole clings to 97%plasma proteins. The integral distribution in a stable situation on healthy volunteers is about 16L.

    Metabolism

Esomeprazole is completely transformed through the Cytochrome P450 (CYP) system. The metabolite of Esomeprazole lacks anti -secret activity. The main part of Esomeprazole metabolism depends on isenzyme CYP2C19, forming Esomeprazole's hydroxy and desmethyl metabolites. The rest depends on CYP3A4 to form sulphone metabolites.

    Elimination

The main semi -discharged time in the plasma of Esomeprazole is approximately 1 - 1.5 hours. Less than 1% of the drug in constant form is eliminated into the urine. About 80% of the doses of Esomeprazole are taken in the form of inactive metabolites in the urine, the rest is found in the form of inactive metabolites in the feces.
