Indications
Rodilar indicated to treat cough caused by throat and bronchi stimulated when the cold cold or when inhaling the stimulant; cough without sputum, chronic.
Pharmacokology
Pharmacological group: A dry cough drug
Code ATC: R05D A09
dextromethorphan is a derivative of morphine, which reduces the cough due to the impact on their center in the brain. The drug has no pain to relieve pain and generally have very little sedative effects.
Dextromethorphan is used to reduce temporary cough due to mild stimulation in the bronchial and throat as usual cold or inhalation of stimulants. Dextromethorphan is most effective in the treatment of chronic cough, without sputum. The drug is often used in combination with many other substances in the treatment of upper respiratory symptoms. The drug has no effect on phlegm.
The effect of dextromethorphan is equivalent to the effect of codeine in the treatment of chronic cough. Compared to Codeine, dextromethorphan has the effect of reducing the cough as irritated similar to Codeine, less side effects in the digestive tract. With the treatment dose, the anti -cough effect of the drug lasts 5-6 hours. Low toxicity, but with very high doses that can inhibit the TKTW system.
pharmacokinetics
dextromethorphan is quickly absorbed through the digestive tract and works within 15 - 30 minutes after drinking, lasting about 6-8 hours. The drug is metabolized in the liver and excreted through the urine in the form of constant and Demethyl metabolites, among them dextrorphan also has a slight cough reduction effect.
Before taking Rodilar 15mg treatment for cough caused by throat and bronchi is stimulated (10 blisters x 10 tablets)
How to use
Tablets for oral tablets.
Dosage
Adults and children over 12 years: 1-2 tablets (equivalent to 15-30 mg dextromethorphan hydrobromid) every 4-6 hours. The maximum dose must not exceed 8 tablets/day (equivalent to 120 mg dextromethorphan hydrobromid).
Elderly: adult dose.
Do not take the drug for more than 3-5 days without the physician's prescription.
Physicians should not be appointed to use dextromethorphan for more than 2-3 weeks.
Handling drugs before and after use: There is no special requirement on drug treatment before and after use.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when using overdose?
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
When using Rodilar often has unwanted effects (ADR) such as:
Common, ADR> 1/100
Rare, ADR
Instructions on how to handle unwanted effects:
In case of respiratory failure and inhibiting the TKTW system, using 2 - 10 mg dose of Naloxon, intravenous injection can be restored.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Rodilar drugs are contraindicated in the following cases:
Precautions when using
alcoholics.
Patients with coughing with too much phlegm and chronic cough in smokers, asthma or alveoli
Patients are at risk or respiratory decline.
Using dextromethorphan is related to histamine release and should be cautious with children with allergies.
Abuse and dependence on dextromethorphan, can occur (though rare), especially due to prolonged high doses.
Using dextromethorphan with alcohol and other central neurological inhibitors can increase the effects on the central nervous system and toxic to relatively small doses.
Dextromethorphan is metabolized by the liver's cytochrom P450 2D6. The activity of this enzyme is determined in genetics. About 10% of the normal population is poor metabolism with CYP2D6. Poor metabolic people and patients use simultaneously with CYP2D6 inhibitors can seriously and/or prolong the effects of dextromethorphan. Warnings should be given in patients who are inferior to metabolic with CYP2D6 or use CYP2D6 inhibitors.
Preparations containing lactose, cautious when used for patients with rare genetic disorders in galactose tolerance, lactase lactase deficiency or glucose-galactose absorption disorders.
Preparations containing TartraZin Lake color can cause allergies in some sensitive people.
Preparations containing methyl hydroxybenzoate and prudity propyl hydroxybenzoate can cause allergic reactions (may occur late).
Using drugs for pregnant and lactating women
Pregnancy
There is no authentic data for teratogenicity in animals.
In clinical research, the results of epidemic research on a small number of women seem to eliminate the special malformation of dextromethorphan.
At the end of pregnancy, high doses, even when treated short, can cause respiratory depression in infants.
During the last three months of pregnancy, the use of chronic dextromethorphan in the mother, regardless of the dose, may be the cause of drug withdrawal syndrome in infants.
Therefore, occasionally using dextromethorphan should be considered during pregnancy only when really necessary.
Breastfeeding period
Dextromethorphan secretes into breast milk: Some cases of tone reduction and respiratory condenser have been reported in children, after being breastfed from the mother using other central anti -drugs at higher doses than conventional doses. Therefore, the use of this drug is contraindicated during breastfeeding.
The effect of drugs on driving, operating machinery
Dextromethorphan can cause fatigue, dizziness, affecting cognitive ability, need to be careful to use with people who perform dangerous jobs and need concentration.
Interactive, taboo of drugs
Interaction of the drug:
Dextromethorphan is metabolized by Cytochrom P450 Isoenzyme CYP2D6, so interact with these enzyme inhibitors such as Amiodaron, Haloperidol, Propafenon, Thioridazin, Quinidine reducing the metabolism of dextromethorphan in the liver, increasing the concentration of this substance in the rod and hypercens Dextromethorphan.
Avoid simultaneous use with Mao inhibitors because it can cause harmful interaction reactions.
Concentrated with TKTW inhibitors can enhance the Inhibition of TKTW of these drugs or of dextromethorphan.
Valdecobid increases the concentration of dextromethorphan in serum when used together. Dextromethorphan used with Linezolid causes Serotonin syndrome syndrome.
Memantin can increase both the frequency and unwanted effects of Memantin and Dextromethorphan, so avoid combining use. Do not use in combination with moclobemid.
The cavalry of the drug:
Due to the absence of studies on the correlation of the drug, not mixing this drug with other drugs.
Storage
Leave a cool place, avoid light, temperature below 30⁰C.
