Uses

Indications

Rostor 20 mg drug is indicated in the following cases:

  • Increasing cholesterol primary (type IIA including hyperlested hyperlemical hyperlly heterozygous) or mixed blood lipid disorders (type IIB): is a supportive therapy for diet when the patient does not fully respond to diet and other non -drug therapies (exercise, weight loss). For diets and other lipid reduction treatments (blood enlightenment) or when these therapies are not appropriate. Methylglutaryl Coenzyme A into Meevalonate, precursor of cholesterol. The main acting position of rosuvastatin is the liver, the target organs reduce cholesterol.

Rosuvastatin increases the number of LDL receptors on the surface of liver cells, thus increasing the absorption and catabolism of LDL, inhibiting VLDL synthesis in the liver, reducing VLDL and LDL components.

    pharmacokinetic

    absorption

Rosuvastatin's peak plasma concentration is about 5 hours after drinking. Absolute bioavailability of about 20%.

    Distribution

Rosuvastatin is widely distributed in the liver, which is the main place to synthesize cholesterol and clear the LDL - C. The distribution of about 134 L. About 90% of Rosuvastatin combined with plasma proteins, mainly with albumin.

    Metabolism

Rosuvastatin is less metabolized (about 10%). The main metabolites are identified as N - Desmethyl and Lacton. N - Desmethyl metabolites have a weaker activity about 50% than rosuvastatin while lacton form is not clinically active. Rosuvastatin accounts for more than 90% of HMG inhibitors - CoA Reductase in circulation.

    Elimination

About 90% of the dose of rosuvastatin is eliminated in a constant form, the rest is excreted into urine. Selling time for plasma is about 19 hours. The average plasma clearance is about 50 l/hour. Like other HMG - Coa Reductase inhibitors, the transportation of rosuvastatin through the liver requires the transportation through Oatp - C. This transportation is important in eliminating rosuvastatin through the liver.

    How to use

Take tablets Rostor 20 mg pills with filtered water.

    Dosage

Before the beginning of treatment, the patient must follow a standard diet of cholesterol reduction and continue to maintain this regime during treatment.

The recommended starting dose is 5 mg or 10 mg, once a day. The starting dose depends on the cholesterol level of each patient, the future of cardiovascular risk as well as the possibility of unwanted side effects.

Can adjust the dose every 4 weeks if needed. Dosage of 40 mg should only be considered in patients with severe blood cholesterol hyperplation that is at high risk of cardiovascular disease (especially patients with hypertonic blood cholesterol) without achieving treatment goals at the dose of 20 mg and should be monitored regularly in these patients.

    Old people

Should use the starting dose of 5 mg/day.

    Patients with renal failure

    Creatinine clearance (CLCR) 30 - 60 ml/min

    Initial doses 5 mg/day/day and a maximum dose of 20 mg/day/day.

    Creatinine clearance (CLCR)

    Contraindications.

    Children

Safety and effectiveness have not been established.

    Coordination treatment

Maximum limit of rosuvastatin dose 10 mg/day/day when used simultaneously with Atazanavir, combined Atazanavir and Ritonavir, combined Lopinavir and Ritonavir.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

When an overdose, patients should be treated with symptoms and apply supportive measures when necessary. Should monitor liver function and ck concentration. Blood decomposition may not benefit.

What to do when forgetting a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

    Side Effects

When using Rostor 20 mg drug , you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • headache , dizziness, nausea, abdominal pain, muscle aches, weakness.
  • Hypersensitivity including angels, muscle disease, muscle pilot, joint pain, liver enzyme , multiple neurological disease, cognitive impairment (memory loss, confusion ...), hyperglycemia, HBA1C.

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

  • Patients who are sensitive to the ingredients of the drug. mechanics.
  • is using cyclosporin.

    Precautions when used

Needs to reduce the dose in patients with renal failure and monitor kidney function during monitoring of patients who have been treated at 40 mg.

    Caution for elderly patients.

Do not use rosuvastatin in patients with acute serious condition, suspicion due to muscle disease or lead to secondary renal failure due to muscle prize (bacterial infection, hypotension, great surgery, trauma, metabolic disorders, hormonal and severe conversion, uncontrolled convulsions).

do liver enzyme test before treatment and in case of clinical indications for testing later.

Consider monitoring Creatin Kinase (CK) in the case:

  • Before treatment, CK tests should be conducted in the following cases: impaired renal function, hypothyroidism, self -history or family history of genetic muscle disease, a history of disease due to statin or fibrat before, a history of liver disease and/or drinking lots of alcohol, elderly patients (> 70 years old) have special risks of medication and patients. In these cases, the benefits/risks should be considered and monitor patients clinically when treated with statin. If the results of CK test> 5 times the upper limit of normal levels, do not start treatment with statin.
  • During statin treatment, patients need to notify when there are muscle manifestations (muscle pain, muscle stiffness, muscle weakness ...). When there are these manifestations, patients need to test CK to take appropriate interventions.
  • The ability to drive and operate machinery

Dizziness may occur so caution when driving and operating machinery.

    Pregnancy

    Contraindicated in this object.

    Breastfeeding period

    Contraindicated in this object.

    Drug interaction

    Vitamin K antagonistic (Warfarin)

can increase Inr when used simultaneously.

Increase the risk of muscle lesions when used simultaneously with the following drugs

gemfibrozil , other fibrat blood cholesterol medications, high doses of niacin (> 1 g/day), colchicin.

    antacids

Concentrated with aluminum anti -aluminum oral oral fluid containing Magnesi Hydroxid reduces about 50% of the plasma rosuvastatin levels.

Simultaneous use of rosuvastatin with erythromycin reduces about 20% AUC and 30% cmax of rosuvastatin, may be due to erythromycin, which increases intestinal motility.

    Orth's contraceptives/hormone replacement therapy

will increase 26% AUC of Ethinyl Estradiol and 34% AUC of Norgestrel when used simultaneously.

Concentrated with HIV and hepatitis C inhibitors (HCV) can increase the risk of muscle damage, most serious is muscle pattern, kidney damage leading to kidney failure and can cause death

Maximum limit of rosuvastatin dose 10 mg/day/day when using Atazanavir simultaneously, combined atazanavir and ritonavir, combined Lopinavir and Ritonavir.
