Uses
Indications
Treatment of local seizures (including seizures: Simple, complex local seizures and local all -transmitters) and all epilepsy of all primary spasms - vibration, in adults and children.
Pharmacokological
Oxcarbazepine is a anti -convulsed drug, chemical structure and similar effect on carbamazepine. Although the exact mechanism of oxcarbazepine is unknown, In vitro physiological studies show that the drug can stabilize the membrane of the nerve cells that are stimulated, inhibit the repetitive discharge of nerve cells and reduce the impulse transmission through the SYNAP by clasping the sodium channel depending on the voltage, leading to preventing the spread of seizures. In addition, Oxcarbazepin also activates potassium channels and varies high voltage calcium channels (HVA-Calcium Channel).
pharmacokinetic
absorption
Oxcarbazepine is completely absorbed through the gastrointestinal tract.
Distribution
MHD metabolites after rapid metabolism is distributed throughout the body (e.g. MHD is 49 liters), about 40% linked to plasma proteins (mainly albumin).
Metabolism
The drug is quickly metabolized in the liver to form the metabolic substance that still has anti-epileptic effect as the 10-Monohydroxy form (MHD).
Elimination
Oxcarbazepine is excreted mainly through urine in the form of metabolites, less than 1% is eliminated in a constant form. Oxcarbazepine and metabolites through the placenta and breast milk. The sale time is 2 hours for oxcarbazepine and 9 hours for MHD.

How to use
Sakuzyal medicine is taken orally. Food does not affect the bioavailability of the drug.
Use single or when combined with other anti -epileptic drugs.
Dosage
Adults
The starting dose of 600 mg/day (8 - 10 mg/kg/day), divided twice. Good treatment effect is seen at a dose of 600 - 2400 mg/day.
Children
Start with a dose of 8 - 10 mg/kg/day, divided twice. In multi -therapies, good treatment effects are seen with an average maintenance dose of about 30 mg/kg/day. If necessary, the dose may be increased by a maximum amount of 10 mg/kg/day every week from the beginning of the drug to the maximum of 46 mg/kg/day.
Not for children
Patients with liver failure
No dose adjustment for patients with mild and medium liver failure.
Patients with renal failure
For patients with renal function (creatinine clearance below 30 ml/min), oxcarbazepine should be started at a dose equal to half the common dose (300 mg/day) and gradually increase the dose to be clinically responded to the desired.
The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? Currently, there is no specific antidote to the case of overdose.
What to do when forgetting a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.
Side Effects
When using Sakuzyal medicine, you may experience unwanted effects (ADR).
Common, ADR> 1/100
Uncommon, 1/1000 Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Patients with hypersensitivity to oxcarbazepine or any ingredients of the drug.
Be cautious when using
should not use oxcarbazepine for patients who have known to be allergic to carbamazepine, unless the benefits are greater than the risk.
When a skin reaction is found in people who are using oxcarbazepin, treatment must be stopped because it is likely to have Steven-Johnson syndrome and poisoned epidermal necrosis. The average time begins to show showing 19 days.
Be cautious with patients at risk of hypoglycatry hypoglycemia such as use with diuretics or nonsteroidal anti -inflammatory drugs.
When prescribing anti -convulsions, it is necessary to consider the risk of committing suicide behavior with anti -convulsions benefits.
Sudden anti -seizure oxcarbazepine (or other anti -convulsions) may increase the frequency of epilepsy. Need to stop the drug should reduce the dose slowly.
Do not take alcohol while taking the drug because it can increase ADR on TKTW.
The ability to drive and operate machinery
Do not drive or operate the machine while taking the drug because of the adverse effects on the TKTW may affect the job.
Pregnancy
Not for pregnant women.
Breastfeeding period
both oxcarbazepine and metabolic products are still active through breast milk. Stop taking the drug while breastfeeding, or stop breastfeeding when taking the drug, paying attention to the effects that can be caused to the mother.
Drug interaction
anti -convulsions (phenobarbital, phenytoin): Oxcarbazepine inhibits metabolic due to CYP2C19 inhibitors increase the concentration of these drugs in plasma.
Oral contraceptive pills: Oxcarbazepine reduces the effectiveness of the drug.
Calci channel blockers (Felodipin, Verapamil): Oxcarbazepine causes an enzyme induction, reducing the effectiveness of these drugs.
