Uses
indications
Savi Spirono Plus is indicated in the following cases:
Furosemide component inhibits the Na+/K+/2 Cl- transportation system in the thick section of the branch on the Henle straps and prevents the reabsorption of sodium, potassium and chlo ions; Therefore increasing the amount of sodium and the amount of water excreted in the urine, and also causes potassium loss. The Spironolactone ingredient inhibits the reabsorption of sodium in the exchange of potassium in the distance by inhibiting competition with aldosterone, the result increases the excretion of sodium and water, reducing the secretion of potassium and magnesi ions caused by Furosemide.
spironolactone
Spironolactone is a mineralocorticoid antagonist that is acting through competition inhibitors with aldosterone and mineralocorticoids that are different from the result of increasing sodium and water secretion. Spironolactone reduces the excretion of potassium, ammonium ions (NH4+) and H+. Both diuretic and anti -hypertension effects are even through that mechanism. Spironolactone starts to work relatively slowly, it takes 2 or 3 days to achieve the maximum effect and the drug reduces slow effect for 2-3 days when the drug is stopped. So do not use Spironolactone when needed to cause quick cards.
Spironolactone and its main metabolites (7 alpha - thiomethyl - spironolactone and canrenone) have anti -mineralocorticoid effects.
Spironolactone reduces both systolic and diastolic blood pressure, so the necessary dose is adjusted according to the treatment response. Spironolactone does not cause hyperuricemia or hyperglycemia, as happened when using high -dose thiazide diuretics.
Furosemide
Furosemide is a diuretic diuretics sulfonamide belonging to a strong, fast, dosage -dependent effect group. The drug acts on the upward branch of the Henle straps, so it is classified as a group of diuretics. In addition to the main mechanism that increases the elimination of Na+, K+ and Cl-, the drug also increases the elimination of Ca2+ and Mg2+. The diuretic effect of the drug is strong, thus leading to hypotension, but often weak. In patients with pulmonary edema, Furosemide causes increased venous volume, thus reducing the money burden for left ventricle before seeing the diuretic effect.
pharmacokinetics
spironolactone
Spironolactone is absorbed through the digestive tract, reaching the maximum concentration in the blood after 1 hour, but it can still be measured at least 8 hours after taking 1 dose. Relative bioavailability is over 90%. Spironolactone and its metabolites are excreted mainly in urine, partly through bile.
Furosemide
Furosemide is good at oral absorption, diuretic effects appear quickly after 1/2 hours, reaches the maximum concentration after 1-2 hours and maintains the effect of 4-6 hours. Anti -hypertension effect is longer. When intravenous injection, the effect of the drug is shown after about 5 minutes and lasts about 2 hours. For people with severe edema, the bioavailability of the drug is reduced, possibly due to the direct effect of reducing the absorption of the gastrointestinal tract.
Furosemide's absorption may be extended and may be reduced by food. One third of the absorption drug is eliminated through the kidneys, the rest is eliminated through the urine, mainly in the form of non -metabolic, completely eliminated drugs for 24 hours.
Furosemide passes the placenta and into breast milk.
Before taking Savi Spirono-Plus 50mg/20mg Treatment of Definitely Hypertension (2 blisters x 10 tablets)
How to use
oral medication.
Dosage
Adults: Take 1-4 capsules daily according to the patient's response. For patients who have stabilized in advance, a higher dose is needed than Spironolactone and Furosemide.
Children: Spironolactone and Furosemide are not suitable for children.
Elderly: Spironolactone and Furosemide can excrete slower in the elderly.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Spironolactone
Expression: Anxiety, confusion, muscle weakness, shortness of breath.
Treatment: Lies stomach, use activated carbon.
Check the electrolyte balance and kidney function.
Support treatment. If hyperboly hyperboly has an electrocardiogram change: sodium bicarbonate intravenous injection, calcium gluconate, oral ion exchange resin to seize potassium ions, reduces blood potassium concentration.
Furosemide
Manifestations: Water and electrolyte imbalance including headache, muscle weakness, cramps, thirst, blood pressure, anorexia, fast pulse.
Management: In return, the amount of water and electrolytes lost.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Savi Spirono Plus, you may experience unwanted effects (ADR).
spironolactone
The reactions are not related to the dose of the day and treatment time. The risk of reaction is low when using a dose lower than 100 mg. The risk of hyperkalemia is low when using a dose of less than 100 mg/day in people with normal renal function, provided that they do not use more potassium and must control the reception of potassium through diet.
Common, ADR> 1/100
Uncommon, 1/1000 Rare, ADR Furosemide Unwanted effects mainly occur during high doses (accounting for 95% of harmful reactions). The most common is the electrolyte imbalance (5% of patients have been treated), which occurs mainly in patients who reduce liver function and with patients with renal impairment when treating high doses. Common, ADR> 1/100 Uncommon, 1/1000 Rare, ADR Instructions on how to handle ADR spironolactone Hypoglycatrological reduction is often manifested: dry mouth, thirst, fatigue, drowsiness. This must be carefully considered, especially when used in combination with other diuretics. Need to adjust the dose and periodically check the electrolytes. Furosemide Signs of electrolyte imbalance include headaches, hypotension and cramps, or occurs when high doses, prolonged doses, need to check regularly. The addition of potassium or used with diuretics holding potassium can be prescribed for patients with high risk of developing potassium hypotension. The use of strong diuretics like Furosemide can cause anemia in the brain. Therefore, it is not used to treat hypertension for the elderly.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
Savi Spirono Plus drugs contraindicated in the following cases:
Hypersensitivity to sulfonamide or with sulfonamide derivatives, such as sulfamide cure diabetes.
Be cautious when using
Caution in patients who are in the shortage of electrolytes (due to diuretics or diarrhea).
This drug should also be used cautiously in patients with diabetes, enlarged prostate, low blood pressure and hypogs (hypovolemia).
Patients are at risk of hyperkalemia if impaired renal function or in combination with other common diuretics.
Metabolic acidosis due to hyperporing blood (recovery, often accompanied by hyperkalemia) can occur in patients with cool cirrhosis even though normal kidney function.
Diuretics generally contraindicated in pregnant women, unless the heart disease is prevented from preventive medicine and can not be cured due to pregnancy toxicity and the drug also reduces the perfusion of the placenta.
Be cautious for patients with prostate hypertrophy or difficulty urinating because they can promote urinary retention.
The ability to drive and operate machinery
avoid operating machinery or driving when using the drug.
Pregnancy
spironolactone and metabolites can cross the placenta fence. With Spironolactone, femaleization has been observed in the fetus of male rats. The use, spironolactone in pregnant women requires consideration of benefits with possible risks for mothers and fetuses.
Animal monster research shows that Furosemide can cause abnormalities (defects) for the fetus. Therefore, Furosemide should only be used in women of reproductive age when contraception is properly taken or if the potential benefits justify the potential risks for the fetus.
Thiazide, thiazide diuretic and diuretics strap diuretics evenly through the fetal fence into the fetus and causes water and electrolyte disorders for the fetus. With Thiazide and substance, many cases of platelets in infants have been notified. This risk also appears after the use of diuretics such as Furosemide and Bumetanide. Therefore, in the last 3 months of pregnancy, the drug is only used when there is no alternative medicine, and only at the lowest doses in a short time.
breastfeeding period
The metabolites of spironolactone are detected in breast milk. If it is necessary to use Spironolactone, the method of replacement for infant feeding.
Furosemide is excreted through breast milk and nursing women should stop treatment. Using Furosemide during breastfeeding is at risk of inhibiting milk secretion. This case should stop breastfeeding.
