Indications
Scanax 500mg drug are indicated in the following cases:
Ciprofloxacin antibacterial by the inhibition of DNA Gyrase and Topoisomerase IV, the enzymes needed for the Bacterial rebirth process of bacteria.
In vitro, Ciprofloxacin has antibacterial activity on some Gram -negative aerobic bacteria: Escherichia Coli and Citrobacter, Entobacter, Klebsiella, Proteus, Salmonella, Serratia, Yersinia. Pseudomonas Aeruginosa and Neisaeria Gonorrhoeea, H. Influenzae, H. Influenzae, Moraxella Cataxella ... and Gram -positive aerobic bacteria: Staphylococci, including bacteria varieties with or unable to produce penicillinase Most anaerobic bacteria, including Bacteroides Fragilis and Clostridium difficile, resistance to ciprofloxacin, although there are some clostridium spp. Also sensitive.
Dynamic pharmacy
ciprofloxacin absorbs fast and good through the digestive tract. Oral bioavailability is about 70 - 80% and the peak concentration in serum is achieved after 1-2 hours about 2.4mcg/ml after taking the dose of 500mg.
The ratio of plasma protein binding is from 20-40%. Ciprofloxacin is widely distributed in the body and penetrates well. The drug appears in cerebrospinal fluid but the concentration is only about 10% of the serum concentration when the meninges are not inflamed. Ciprofloxacin passes the placenta and is distributed in breast milk. High concentration of medication in bile.
The sale time of the drug is about 3-5 hours and shows the accumulation of drugs at a normal level. Selling waste time may be longer in patients with renal impairment (about 8 hours were reported in patients with end -stage renal failure) and the elderly. However, usually do not need to adjust the dose in these patients except when the disease is seriously ill.
ciprofloxacin is excreted mainly in the urine, but the excretion of the kidneys takes about one third of the excretion, including the metabolism of the liver, the excretion through the bile and excreting through the mucosa into the intestine. Oxociprofloxacin is the main metabolite through the urine and sulfociprofloxacin is the main metabolite through the stool. About 40 - 50% of oral dose is excreted in the form of unchanged urine and about 15% in the form of metabolites. About 20-35% of oral dose is excreted in feces after 5 days.

Needs to reduce the dose in people with severe renal failure. In case the person has renal function, does not need to reduce the dose if the dose is low. In high doses, doses should be adjusted based on creatinine clearance or serum creatinine levels.
dose ≥ 750mg x 2 so it decreases to 500mg x 2 Patients with liver failure No dose adjustment. Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? An overdose of 16g has been reported to cause acute renal failure. Symptoms Fatigue, convulsions, hallucinations, confusion, abdominal discomfort, liver failure, kidney failure as well as urinary crystals and bleeding. There have been reports on the poisonous kidneys that can recover. Handling Should empty the stomach by causing vomiting or gastric lavage. Patients should be carefully monitored and supported, including renal function monitoring and may give extra antacids containing magnesi, aluminum or calcium to reduce the absorption of ciprofloxacin. The hydration process must be maintained. Only a small amount of ciprofloxacin ( What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using scanax 500mg you may experience unwanted effects (ADR).
Common, ADR> 1/100
Digestive: Nausea, vomiting, diarrhea, abdominal pain.
Metabolism: temporarily increase the concentration of transaminase.
Uncommon, 1/1000 Systemic: headache, drug fever. Blood: Eosin hypernagus, lymphocytes, polyclinic leukopenia, anemia, thrombocytopenia. heart - circuit: tachycardia. Central nerve: agitation. Digestive: digestive disorders. Skin: rash, itching, shallow veins. Metabolism: temporarily increased creatinine, bilirubin and alkaline phosphatase in the blood. skeletal muscles: Again in joints, swelling. Rare, ADR Systemic: Anaphylaxis or anaphylaxis. Blood: Hemolytic anemia, leukemia, thrombocytopenia, changes in prothrombin levels. Central nerve: convulsions, confusion, mental disorders, paranoia, insomnia, depression , peripheral sympathetic disorders, visual disorders including hallucinations, hearing disorders, tinnitus, taste disorders and smell, increased intracranial pressure. digestive: fake colitis. Skin: Skin syndrome - mucosa, vascular inflammation, Lyell syndrome, skin erythematosus, diverse erythematosus. Liver: There have been reports on some cases of necrosis of liver cells, hepatitis, and jaundice. muscle: muscle, tendonitis (heel tendon) and surrounding tissue. There are several cases of tendon broken, especially in the elderly when used in combination with corticosteroids. Urinary - genital: There is an urinary crystal when alkaline urine, bleeding, acute renal failure, interstitial nephritis. Other: sensitive to light when exposed to sun, laryngeal edema or pulmonary edema, shortness of breath, bronchial spasm. Instructions on how to handle ADR advise patients to rest and limit exercise when the first sign of tendonitis or tendon rupture (such as pain, swelling, or tendonitis, weakness or unable to move joints), stop taking the drug and contact the clinician to consider changing to another antibacterial drug Fluoroquinolon. If the rash, jaundice or other hypersensitivity signs occur while taking ciprofloxacin, should stop the drug immediately and take appropriate support measures (such as epinephrin, corticosteroids, maintain air ventilation, adequate oxygen, maintain blood pressure). should maintain enough drinking water during the use of ciprofloxacin and avoid making urine too alkaline because of the risk of urinary crystals. If suspected or identifying colitis or diarrhea due to Clostridium difficile, it may be necessary to stop ciprofloxacin. Some mild cases can respond only to the stop. Cases from moderate to severe should be treated by supplementing fluid, electrolytes and proteins, treating antibacterial antibacterial drugs against Clostridium difficile (such as oral metronidazol or vancomycin), and surgical assessment when clinical indications.

Pregnancy
should only use ciprofloxacin for pregnant women in cases of severe bacterial infections without other antibiotics instead, forced to use fluoroquinolon.
Breastfeeding period
should only use ciprofloxacin for pregnant women in severe bacterial infections without other antibiotics instead, forced to use fluoroquinolon.
Drug interaction
using fenbufen with fluoroquinolon may increase side effects on the central nervous system of fluoroquinolon. Side effects on the nervous system are also reported on patients taking Naproxen and Chloroquin that is indicated for ciprofloxacin.
The absorption of Ciprofloxacin and other fluoroquinolones is reduced by aluminum or magnesium antacids or by calcium, iron and zinc salts. Similar to other fluoroquinolones, ciprofloxacin should be taken at least 2 hours in advance or after 6 hours of taking these drugs.
Ciprofloxacin increases the effects of anticoagulants such as warfarin and oral medications for diabetes of Glibenclamid. Lowering serious blood glucose, sometimes deadly occurring in patients taking glibenclamid.
Temporary increase in serum creatinine occurs when using ciprofloxacin with ciclosporin.
Concomitance use of ciprofloxacin and theophyllin may increase the concentration of theophyllin in serum.
Ciprofloxacin's absorption may be reduced half if used simultaneously with some cytotoxic drugs (cydophosphamkj, vincistin, doxorubicin, cytosin arabinosid, mitozantron).
Take drinking simultaneously sucralfate will reduce the absorption of ciprofloxacin significantly.
Ciprofloxacin serum concentration is significantly reduced if used with Didanosin.
