Uses

Indications

Spinolac 50mg drug is indicated in the following cases:

  • ascites due to cirrhosis, liver edema, kidney edema, heart edema when other drugs are less effective, especially when there is suspicion of increasing aldosteron.

Spironolacton is a mineralcorticoid antagonist, which is used by inhibition of painting with aldosteron and other mineralocorticoids, which are mainly in the distance, resulting in increased sodium and water secretion. Spironolacton reduces the excretion of potassium, ammonium ions (NH4+) and H+. Both diuretic and anti -hypertension effects evenly through that mechanism.

Spironolacton starts to work relatively slowly, it takes 2 or 3 days to achieve the maximum effect and the drug reduces the slow effect for 2-3 days when the drug is stopped. So do not use Spironolacton when it is necessary to cause fast cards. The increase in magnesi and potassium excretion of thiazid diuretics and straps (Furosemid) will be reduced when used simultaneously with Spironolacton. Spironolacton and its main metabolites (7 alpha-tiomethyl-spironolacton and canrenon) have anti-mineralocorticoid effects.

Spironolacton reduces both systolic and diastolic blood pressure, the maximum hypotension effect is achieved after 2 weeks of treatment. Because Spironolacton is an antagonist competing with aldosteron, the necessary dose is adjusted according to the treatment response. Increasing rare Aldosteron. Increased secondary aldosteron occurs in secondary edema due to cirrhosis, kidney damage syndrome and prolonged congestive heart failure and after treatment with conventional diuretics. The diuretic effect is enhanced when used in combination with conventional diuretics, spironolacton does not cause hyperuricemia or hyperglycemia, as happened when using high -dose thiazid diuretics.

    pharmacokinetic

    absorption

Spironolacton absorbs rapidly through the digestive tract, reaching the maximum concentration in the blood after 1 hour.

    Distribution

more than 90% of spironolacton is linked to plasma proteins when entering the blood.

    Metabolism

Spironolacton metabolizes rapidly through the liver and kidneys. The pharmacological metabolic substance is 7-alpha-tiomethyl-spironolacton and canrenon.

    Elimination

Spironolacton and its metabolites are excreted mainly through urine, partly through bile.

    How to use

    Oral drugs.

Can be used when hungry or full.

    Dosage

    Adults

Diuretic when anti -anti -cirrhosis due to cirrhosis, kidney syndrome, congestion heart failure, especially suspected of increasing aldosteron

The initial dose is taking 1-4 tablets/day, dividing 2-4 times, taking at least 5 days, maintenance dose is 2-8 tablets/day, divided 2-4 times.

    Anti -blood pressure

Initial dose taken 1-2 tablets/day, divided 2-4 times, used for at least 2 weeks, maintenance dose adjusted by each patient.

    increase aldosteron

Dosage 2 - 8 capsules/day, divided 2-4 times before surgery. The lowest dose is effective for long -term maintenance for patients who cannot surgery.

    Children

diuretic or hypertension, ascites due to cirrhosis

Initial doses take 1 - 3mg/kg/day, use 1 time or divide 2-4 times, the dose is adjusted after 5 days.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

    Expression

Anxiety, confusion, muscle weakness, shortness of breath.

    Handling

    Stomach washing, using activated carbon.

Check the electrolyte balance and kidney function.

    Support treatment

If hyperkalemia has an electrocardiogram changes: sodium bicarbonate intravenous injection, calcium gluconate, ion exchange resin (sodium polystyre sulfonate) to seize potassium ions, reduce blood potassium concentration.

What to do when forgetting a dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

The reactions are not related to the dose of the day and treatment time.

The most common is big breasts in men due to increased prolactin levels but often recovered after treatment.

Hyperbonemia must always be considered in those who reduce renal function. This risk is low when using a dose of less than 100mg/day in people with normal renal function, provided that they do not use more potassium and must control the reception of potassium through diet.Common

  • Body: Fatigue, headache, impotence , Sleeping.
  • Endocrine: Increasing prolactin, big breasts in men, Milk flow, menstrual disorders , menstrual loss, postmenopausal bleeding.
  • digestive: diarrhea, nausea.

    Less

  • Skin: Red Red, Red Red, Redor.
  • Metabolism: hyperkalemia, hypoglycemia.
  • nerve: Cramps spasm, paresthesia.

    Urinary genitals: Increased serum creatinine.

    Rare

  • Blood: granulocytes, platelets.
  • Instructions on how to handle ADR:

Hypoglycatrological reduction is often manifested: dry mouth, thirst, fatigue, drowsiness. This must be considered carefully, especially when used in combination with other diuretics. Need to adjust the dose and periodically check the electrolytes.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Spinolac 50mg is not used in the following cases:

  • Acute renal failure.
  • Severe renal failure.

    Precautions when used

The risk of hyperkalemia like when impaired renal function and when combined with other common diuretics.

Metabolic acidosis due to recovery hypertension (often accompanied by hyperkalemia) may occur in loss of loss due to normal kidney function.

Diuretics generally contraindicated in pregnancy, unless heart disease.

The ability to drive and operate machinery

The drug can cause drowsiness, should be noted when used.

    Pregnancy

Do not use for pregnant women unless pregnant women have heart disease.

    Breastfeeding period

    Drug interaction

simultaneous use of spironolacton with enzyme inhibitors (ACE - 1) can lead to severe "hyperkalemia", life -threatening, especially in people with kidney failure.

The anticoagulant effect of cooumarin, or indandion or heparin is reduced when used with spironolacton.

Non -steroid anti -inflammatory drugs, especially indomethacin, reduce anti -hypertension effects of spironolacton.

Simultaneous use of lithium and spironolacton can lead to lithium poisoning, due to reduced clearance.

Timeless use of potassium -containing drugs with Spironolacton increases potassium.

Digoxin waste sale time and cardiac glycosides may increase when used simultaneously with spironolacton.

  • Storage

Cool dry storage, avoid light, temperature below 30⁰C.
