Uses
Indications
Spiromide 40/50 is indicated in the following cases:
Pharmacy
Spironolactone is the most important steroidal lactone in clinical application: it acts as a diuretics and low pressure by Aldosterone's inhibition effects, and partial inhibition of adore cortisol biosynthesis of Aldosterone.
pharmacokinetic
spironolactone
Spironolactone is absorbed through the digestive tract, reaching the maximum concentration in the blood after 1 hour, but still with the concentration that can be measured at least 8 hours after 1 dose. Relative bioavailability is over 90% compared to the bioavailability of Spironolacton solution in polyethylene glycol, the best absorption. Spironolacton and its metabolites are excreted mainly in urine, partly through bile.
Furosemid
Furosemid is well absorbed by oral, diuretic effects appear quickly after 1/2 hours, reaches the maximum concentration after 1-2 hours and maintains the effect from 6 - 8 hours. Anti -hypertension effect is longer. Furosemid's absorption can be extended and can be reduced by food. One third of the absorption drug is excreted through the kidneys, the rest is eliminated through the urine, mainly in the form of non -metabolic, completely excreted drugs for 24 hours. Furosemid passed the placenta fence and into breast milk.

How to use
Spiromide is used orally. To increase biological activities, use Spironolactone with meals.
Dosage
Spironolactone dose varies depending on clinical disease.
In the treatment of secondary Aldosterone or high blood pressure, 50 - 100 mg/day, the dose can be increased to 400 mg/day in diagnosis and treatment of primary Aldosterone. The maximum effect occurs after a few days of medication.
With oral Furosemide usually starts at a dose of 20 - 40 mg/day, 1-3 times/day, and increases the dose depending on the treatment response. In general, oral doses from 160 - 320 mg/day and not exceeding this dose. However, in patients with renal failure, oral doses can reach 1000 mg/day. In children, orally 1 - 3 mg/kg/day.
Venous treatment usually starts at a dose of 20-40 mg used once to 2 times/day. Depending on the response of the treatment, it may increase the dose as the dose treatment. Especially in patients with progressive renal failure, bureaucracy increases to 1000 mg of Furosemide/day that can be used in intravenously. To avoid toxicity on the ear, the transmission speed should not exceed 4 mg/min.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? Increase K+ blood and lower Na+ rare blood occurs. Symptoms often lose when stopping drugs, compensation and electrolytes, appropriate hypertension treatment.
High doses of Furosemide can cause loss of fluid and severe electrolytes, causing a decrease in internal volume, which can cause posture lowering and reducing glomerular filtration.
Reducing the volume because Furosemide will activate the Renin - Angiotensin - Aldosterone system and this way K+ takes a lot through urine. Treatment includes additional NaCl and water, if necessary intravenously.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
